Mostrando los 42Employee was walking to the back of their truck to get something out and did not lift a foot high enough and tripped over the railroad tie that was used for parking spots in the parking lot. They fell on their right shoulder hurting it.
While pulling the lid back on rotex 6 EE stepped in an open spot under the rotex and caught self with EE's left elbow on top of the machine. Did not seek medical attention until 11/20. No restrictions to job duties, but medical treatment (Therapy) was ordered 12/11/23. Opened due to therapy for nerve in the elbow.
E did not want to see the Dr. until 4/18/23 when EE called and said it was sore and wanted to go to the Dr. EE was climbing off the cat-walk of the truck when the side bracket that holds the 2 steps together broke and EE fell from the bottom step to the ground. Seeing Dr. on 4/19/23.
EE was walking from the office and when EE stepped down the steps and turned, EE felt pain in the inside lower part of the left knee. EE did not feel EE needed medical attention until 1/30/23. EE notified HR on 1/27 while EE was on vacation that EE's knee hurt and wanted to see a Dr. when EE got home. Released to full duty but given a brace for the knee at this time.

While shoveling sand EE turned and leaned on a metal rail on a platform at a height of about 15 feet. The railing snapped and EE fell from a platform straight down and hit the cement. EE has missed work days at this time only.
EE was walking across the parking lot and slipped on packed snow.

EEs were installing pipe on the bluff. All ees were tied off by 2 means, the pipe broke and the rope from the tie off was caught on the pipe. When the pipe broke it pulled an ee about 200 yards down the bluff. The second tie-off prevented the ee from being dragged any further. The employee received a contusion to the head and bruising to the entire body.
EE was in JLG starting to exit, his safety harness caught on something causing him to fall back in to JLG. He then put his hand down to break his fall and his hand landed on a HDP Pipe clamp on the floor. Cutting his hand through his glove.
Employee stepped off of a piece of equipment and turned left ankle, causing a sprain to the left ankle.
Employee stepped onto a pallet to go to another area and slipped in the mud and employees left knee gave out, causing strain and sprain.
Pulled tendon by big toe when climbing stairs of locomotive which was wet and icy. Did not slip just stepped down wrong on foot.
