On May 4, 2011, EE allegedly injured ankle while walking on floor of a building. She completed her scheduled May 4 shift. The Ohio County Coal Co. is unaware of any medical provider who determined that she had to miss work as a result of alleged event on 5-4

Employee stated that while he was walking by the Joy CM-15, he tripped and fell into the miner. His right right left (upper thigh) struck a miner bit, which caused a laceration on his thigh.
EE carrying roof bolts to front of pinner, stumbled and fell into bolter. Pin plate struck left forearm, causing a small laceration that required 5 stitches to close.
Employee hanging pinner cable stepped into a rut and sprained left ankle.
