Employee was dismounting a 16G grader after working on the fuel linkage. Employee was coming down the steps when they stumbled and fell to the ground landing on the left hand (approximately 2'). This resulted in a hairline fracture to the forearm just below the elbow.

Stepped out of bulk truck slipped and fell on ice twisting left knee. Sought medical attention on 2/28/14 full release. Seen specialist on 3/13/14 full release. Still having pain had MRI 5/23/14 was put off for surgery on 6/2/14 with a torn meniscus.
Injured employee was securing equipment to the lowboy when he slipped and fell causing a fracture to his tail bone. He continued to work his shift but went to the ER afterwards where the diagnoses was made. He was released to return to work the following day.
Injured employee was walking the roadway on #1 section when he stepped in a rut and twisted his lower back. He saw his primary care physician 9/28/10 and was referred for an MRI on 10/18/10. On 10/21/10 MRI revealed bugling disc in his lumbar region of his lower back. He has been excused from work until further notice.
