The IE was carrying a bottle of compressed gas down the longwall face when EE slipped and fell. The IE has pain in the middle part of EE's back.

The IE was using a prybar and it slipped out causing the IE to fall and land on EE's elbow. The IE continued to work until 10/1/2025 when EE received surgery.

Was working on D6T dozer that was down, was climbing down on the left side, holding handrail with right hand and right foot on the push arm, left foot slipped off push arm step, and hit right ribs on the tracks.
I.E. stated EE was using a bottom roller bracket as a handle to pull a bottom roller into a crosscut, as EE was pulling the bracket came of causing EE to lose balance and fall landing on EE's left wrist. X-ray shows a fracture.
I.E. stated they were stepping off of the platform backwards, their left foot was on the ground and their right foot hung on the 5-inch lip on the edge of the platform. I.E. fell backwards hitting their back on the foot of the ATRS. X-rays show a fracture in their L2.
IE was working on a conveyor belt, lost EE's footing, and fell. The IE felt pain in EE's back and was taken off work for the pain in EE's back
I.E. stated EE was getting a bag of rock dust off the roof bolter and slipped hitting EE's ribs and shoulder on the machine.
The IE stepped out of the mine's guard shack and slipped on ice that formed on the wooden steps. As the IE fell, they hit their head and shoulder on the step. The IE continued to work until they went for shoulder surgery on 6/9/2025.
I.E. stated I.E. was helping change out belt on main north 3 and slipped and fell. When trying to catch self I.E. cut left hand on an unknown object on the mine floor.
I.E. stated they had just finished changing canoe liners in HG 10 flow through, as I.E. was walking away from the flow through I.E. slipped in the mud causing right hand to fly up becoming wedged between the belt and the bottom roller. I.E. went to a hospital where X-rays showed no broken bones. I.E. is supposed to follow up with Ortho for full release
EE stated they saw the emulsion line had blown and they were running down the travel road to find a shut-off when they rolled an ankle. Employee went to the hospital with no broken bones but was issued a walking boot and crutches.
I.E. stated was helping add belt for a belt move and stepped on a wet piece of belt causing I.E. to slip and fall hitting right ear on vulcanize table. Employee had stitches put in ear at a hospital.
The employee was in the process of getting out of a track-hoe and the employee's leg buckled causing employee to fall. The employee did land on feet but injured back during the accident. The employee continued to work until surgery on September 18, 2024.
I.E. stated they were checking dust in face fan 37, they had shut off hyd. to the duster raised lid and slipped with right hand getting wedged between agitator and dust hopper. I.E. re-gained footing and pulled hand out. Employee worked until 10/21/2022 then went to doctor for agitation and discomfort in right hand.
IE was roof bolting in the North Mains 5 left entry. IE finished drilling EE's hole and put EE's glue stick in the hole. When IE went to put the bolt in the hole, IE slipped of EE's platform towards the atrs and fell to the ground. The platform was in the elevated position (approximately 5 to 5 1/2 feet) off the ground.
IE was bolting on the right side in the North Mains entry #4. IE was walking through the middle of the DBRB and was going to step up on the bolting platform. IE's foot slipped and struck the bolting platform. IE received a cut to the right leg (shin). IE received 8 sutures to close the cut.
IE was talking to two other co-workers while turning to walk away and caught foot on a transformer slide causing IE to fall. IE landed on right knee, causing an abrasion swell. IE finished the shift and saw own doctor the next day who told IE to take off two weeks with a follow up exam.
I.E. was getting out of the cab of SM02 and caught EE's right foot between the seat and steering lever. I.E. was falling and used EE's right arm to catch self that is when EE felt a pop and immediate pain in EE's right shoulder. IE is waiting to see a specialist. This happen on the surface at the Akin portal, the conditions was dry and still daylight.
IE was lifting a 12" sweeping 90* fitting out of the back of the bed of a pickup truck with the tail gate lid closed. As IE was lifting the pipe bent over, IE's foot slipped off of the back of the bumper causing pain in lower back/leg. IE began receiving chiropractic treatment on lower back on 4/21/2022.
IE was performing maintenance working to a shield on the HG8 longwall. IE was climbing in and out of this shield changing hoses when felt a crack in EE's left knee. IE worked two more hours before reporting it, IE continued to work for a few months before seeking medical attention. Conditions in the area was good and level but wet from washing.
Employee was standing on a rock while taking an air reading when EE lost EE's footing and fell to the ground on EE's wrist. Employee worked to 6/15/2020 then was put on light duty with restrictions
Employee was hanging cable in front of the feeder on HG7. After hanging the cable EE slipped and fell striking EE's back and left ribs. Employee went for medical treatment the next day, x-ray showed EE had received 4 fractured ribs.
employee was on the North side of the cage bottom, the cage was on the surface, the employee opened the North side gate and walked 4.5 feet, (assuming the cage was on the Bottom) and fell 19.5 feet to the grading at the bottom of the shaft, landing on EE's right side. Employee received fractures to EE's back and left leg.
A contractor employee was walking and pulling a rope. While pulling the rope EE tripped over a hose fracturing left ankle.
Employee had a seizure
Walking by stage loader over rib rash, slipped and a piece of rib wire stuck into my left forearm.
Top started felling out as EE was drilling, went to step out of the way and fell backwards off platform landing on EE's back
Employee was carrying extension cords when EE tripped and fell over them causing EE to sprain EE's left ankle.
Employee was coming out of sampler room when they slipped/tripped over belt and one leg went off concrete pad. Cause of injury was leg slipping off concrete body placement. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work 6/8/2018. Scheduled days off 6/15, 6/16, 6/17.
Employee stepped down from a supply trailer and twisted EE's knee
EE pulled slick lines and hooked them up to a pump able crib, turned around and tripped over the slick lines and tried to catch self dislocating left shoulder.
IE was walking in muddy area and tripped over hog panels that were lying in the mud. IE fell on side straining back and hurting ribs.
Employee was in the process of throwing over cable when a section came back and hit employee on the right side of employee's lower right leg. This caused employee's right leg to twist and employee heard employee's right knee pop. This resulted in an ACL tear.
IE twisted knee when IE fell while making a weekly examination.
Employee was standing on the 6th step of a 12' ladder to drop down a belt chain that was tied up to the rib. Employee was leaning against a 4" pipe that was about chest level while on the ladder. The pipe moved some causing employee to lose employee's balance and fall. This resulted in a broken right wrist.
This is being filled due to a citation issued during a Part 50 Investigation. On 7-17-2017 the IE was removing trash from a xc and tripped over a concrete block. While trying to catch themselves from falling. The IE extended arm out and came in contact with the coal rib resulting in pain to shoulder. The IE worked until 2-28-2018 and then had shoulder surgery.
Employee was laying a block for the foundation of a stopping. As employee turned to pick up another block employee heard employee's left knee crack.
I.E. was pumping water in the bleeder. Felt pain in right ankle while walking and EE's foot started hurting. EE didn't report the incident to anyone until 03/08/2017. EE went off for unrelated to work back surgery on 07/17/2017. We were informed on 07/28/2017 that the Dr. connected the ankle injury to the back surgery that EE had went off for.
Employee was hanging curtain in the face. EE slipped & fell from the 3rd step of an 8' ladder. It caused a very slight fracture to a bone in left forearm.
Contractor stepped down from one flap to the second flap and was inspecting their work, when contractor turned around slipped due to a thin sheet of ice on the flap. Contractor then fell about 2ft and landed on right knee and shoulder on the frozen ground. This resulted in a fracture to right knee cap.
Employee was carrying cribs and slipped on a wet 2"X12" board. Employee twisted EE's knee & fell. Employee has missed no work but had to have a procedure done to cut a foreign object out of EE's knee on 12/27/2016. Mine has been on vacation shut down since 12/23/2016 until 1/3/2017.
Contractor was trying to get a 4ft mandoor out of the back of a 6 man MV. The door was stuck so ee stood on the bumper for better leverage. EE jerked up on the door to free it up, when it did ee felt a pop in lower back. EE then let go of the door and fell to the mine floor. This resulted in a strain/sprain to the lower back.
Employee was shoveling at the tailpiece by the bore hole. Another employee was spraying the bore hole and unclogged rocks that were stuck. This caused water, sand, and mud to fall from the bore hole to the mine floor. The material came into contact with employee causing EE to fall which resulted in contusions to right leg and sand in eyes.
Employee was holding rib mesh up with a 6' rod to keep self away from rib while mesh was bolted. EE went to step away and EE's foot was under a loose rock causing EE to fall.
Employee was hanging a block curtain by the feeder on the 3rd step of a 6 foot ladder. A service truck was backing up towards the feeder to drop off a spool of co cable and backed into the ladder knocking the ladder and employee to the ground. This resulted in a strain at the time of the incident and now the employee is receiving medical attention.
Employee was getting out of the mantrip when he got tripped up trying to get out and he fell on a 6" waterline that was on the ground causing him to hurt his ribs.
Employee was helping to install a mandoor in stopping when he slipped over a crib tie and fell cutting his arm on the door.
