While climbing off of the Mine Cat explosive carrier the miners tools became entangled on the tractor causing the miner to lose his balance and step down hard on the left leg. The miner reported hearing his knee pop and felt pain.
Employee was stepping off his drill and rolled his ankle. Seen at the ER and released to restricted duty for one day.
Main door on bus was locked and employee tried to enter bus through the emergency door, which is approximately 3 feet off the ground. The employee slipped and cut his left index finger. He was seen at the ER and the laceration was stitched. Released to full duty.
Employee walking on steep slope with uneven and floating rocks all day on the 26th. Her ankle began to hurt, the next day her entire right leg from ankle to low back was hurting. Diagnosed with sprain ankle and inflamed sciatic nerve. Restricted duty for 7 days.
Employee was running out rods and slipped and fell dropping rod on foot.
IW was descending escape ladder in conduit, foot slipped off rung and he fell about 5 feet, landing on metal grating. Right ankle buckled under him. Xrays negative, placed in immobilizer boot. Released to full duty.
Walking out to service truck in ice and snow, slipped and fell, broke left leg and ankle.
IW operating dozer, felt pain in knee. No accident. Claim accepted as work related knee strain on 06/14/2011 after independent medical evaluation. IW had one day restricted duty on 04/14/2011 after receiving a steroid injection.
Employee was getting off bus to start night shift. He caught his left heel on the edge of the concrete pad, causing him to twist his left ankle. He was seen at the ER and subsequently released to restricted duty. Investigation complete on 01/18/2011, claim accepted by workers' comp on 01/25/2011.
Driver was climbing up onto work truck to close bay door, stepped off and landed the wrong way causing ankle to break.
Stepping off a water truck, missed step, fell to the ground. Seen at ER, diagnosed with head injury, released to full duty. Contacted safety on 9/3/10, was having pain in head and left shoulder. Seen 9/17/10,diagnosed with a shoulder strain, released to full duty and referred to physical therapy. He is not progressing. Medical reportable for treatment due to physical therapy.
Operator was exiting haul truck and when she reached the bottom of the ladder she turned around on the platform to exit the bottom portion of the steps and she fell down the steps to the ground and hit and cut her right knee.
Employee was fueling a dozer and he states that he turned quickly to shut off the fuel when he heard a snap in his right knee and fell to the ground. He was taken to NNRH where he was diagnosed with a fracture and underwent surgery.
EE was coming down a ladder on a haul truck and when he got to the last rung he dropped to the ground, twisting his ankle. He was taken to NNRH and given a full duty release but was still in pain on 06/20/10 so he was looked at again at the hospital and was told to take 3 days off and then to start physical therapy. This injury became a lost time on that day (06/20/10)
Employee fell off of the track on a dozer while working on the fire suppression system. It had been raining and the area was wet and muddy.
Employee got off of a trailer with his left leg first and then the right. The employee's left foot slid down the berm in loose gravel, twisting his leg. He was seen on 05/10/10 and through a MRI was diagnosed with a partially torn ACL and was put on restricted duties. No restricted duty so he became a Lost Time.
Employee was lifting platform frame and feet slipped in snow, causing employee to fall to ground. The platform frame struck employee on the left side of head.
Employee was walking back to his truck when he tripped over a rock and landed on his left arm and elbow. During a follow up visit on January 14, 2010 it was discovered that the employee had a chip fracture to his elbow.
Employee was climbing down from a boom truck. His right foot was on the ground and when he started to put his left foot down his boot caught on the bumper and he felt a pop. It slid off the bumper and it popped again. Went to Battle Mountain Hospital and was released with Restricted Duty.
While bending over to pick up a suction hose employee slipped causing him to loose his balance. Sticking out his arm to maintain balance, his hand struck the shotcrete rib.
Employee was getting off of the bus at the mill and fell down the stairs. He was seen at the ER and was diagnosed with a fractured rib. Released to full duty.
Employee was loading core bins on back of a boom truck. As employee was un-hooking the rigging, he decided to jump off truck instead of using the ladder that is mounted on truck. The result was, employee landed off balance and used his left hand to catch himself, which resulted in a broken bone in hand.
After fueling a CAT D6R Dozer, the worker dismounted from the dozer and in doing so stepped into a pre-existing dozer track (impression in the ground) and sprained his right ankle.
While walking from the ready line to the shop, the employee twisted his left ankle and fell. He was taken to NNHR. He had a follow up visit on the 21st and was classified as LTI at this visit.
Employee was washing off the pond balls in a the dump truck with a high pressure sprayer. He lost his footing and fell off the top of a 4 ft. ladder. He hit his head on the floor. He was taken to NNRH and found he had a concussion He was given a full duty release but was made a LTI at the follow up on October 21st.
