EE HAD GOT OFF HIS RIDE TO SHUT THE AIRLOCK DOORS HE SLIPPED AND FELL CATCHING HIMSELF WITH RIGHT HAND. BENT INDEX FINGER BACK. SWELLING.

EEG stated that appx. two weeks ago on 6.18.2013 he was climbing off the loader and stepped on uneven ground and twisted his left knee. NOTE: Reported incident after chosen for random alcohol/drug screening on 6.26.13. EEG has worked regular duties until 6.26.13--started missing work 6.26.2013
