Employee was mounting a CAT 980 Loader when EE's foot slipped on the the step causing EE to catch self with EE's left arm. EE was diagnosed with a torn tendon and muscle and had to have surgery on 5/14/2026. EE's first day missing work was 5/14/2026.

Mostrando los 25Employee was walking from EE's private vehicle to EE's rock truck. As EE was walking across the parking at the truck ditch, EE slipped and fell on the snow and ice. EE was diagnosed with a fractured femur and started missing work on 1/11/2025
Employee was starting equipment prior to other employees running the equipment. EE had started a D11 dozer and when EE stepped off the platform onto the track, EE's foot slipped off the wet track causing left ankle to roll to the side causing a severe strain.

He worked through to 11/10/2014, returned to doctor who stated a possible hernia and has been off work since. Scheduled to see doctor on 10/21/2014 for possible surgery.
Getting on dozer and foot slipped. Caught himself with right arm. Thought he would be ok and finished out the week. On 10/13/2014, he was taken to be examined at Hazard ARH by Safety Director. X-ray showed nothing but doctor thought it could be a possible torn rotator cup. MRI has been requested and victim has been off work since 10/13/2014.
Employee stepped off low boy onto crib block when crib block shifted causing him to fall.
Working in cab on D9N, and started down out of the cab. Stepped down on track and twisted left knee. Heard it pop, but thought it would be ok. Did not feel need to go to doctor. On 8/15/14 he was examined and scheduled for MRI, but returned to work. He missed work 8/19/14 due to pain but returned on 8/20/14 and worked through 8/26/14. MRI scheduled for 9/2/14.
EE was getting off 992 loader at end of shift when he said he hung the toe of his boot on the boot brush causing him to fall onto platform. Had lunch bucket in left hand and fell onto left shoulder.
