Mostrando los 5Dimensional stone extraction crew miner needed a tool from another area of the ledge. Instead of using provided access to that area, miner tried to walk across a small (approx 10" wide) ledge of a column to get there that was roped off. Miner slipped on rocks/mud and fell down approx. 7 feet and landed on shoulder causing pain.
While measuring for future cuts on the ledge, Employee attempted to step over a barrier rope (approx. 30" high). Employee's foot was tripped by the rope and employee fell over the rope landing on left knee, resulting in broken patella.
While beginning to mark a turned block, employee noticed a chunk of stone debris attached to the side of the block. Employee used left foot to stomp downward on excess piece. Excess piece was barely attached & fell easily but downward momentum & lack of resistance by the stone caused employee to lose balance, falling down onto block landing on right shoulder.
Conditions contributing to Injury: uneven working surface, inattention to task status. Resulting in losing balance, falling on backside while attempting to break fall with hand, sprained wrist.
