Operator positioning a light plant on the bench, when EE got out of EE's pickup EE rolled EE's ankle. EE felt popping and pain was evaluated by the EMT then taken to the hospital. X rays revealed EE had fractured EE's right fibula. The employee was given a splint and crutches then sent home.

Mostrando los 4While working on a manlift, dismounted onto the ladder misjudged the steps fell on right side shoulder. Initially said it was ok, shoulder continued to be sore, started physical therapy on 3/10, did not inform the company of this activity until 3/24. The diagnosis, strained separation in the shoulder, the employee has not missed any scheduled shifts.
Operator dismounted rubber tire dozer. Slipped on ice. Employee felt they did not need any medical care at that time. On 4-3-17, employee informed company that on 3-31-17 employee received results of x-ray and MRI. Employee will be receiving physical therapy treatment on 4-10-17, for continued numbness in left arm. To date, employee has not missed any scheduled shifts.
