An employee was standing on a 17 inch two step working platform performing maintenance work. As EE applied force to the box end wrench, the wrench slipped causing EE to lose balance and fall. In falling, EE's right knee contacted a ceramic liner, resulting in a fractured knee cap.

Mostrando los 10On February 11, 2025, a mine maintenance employee was performing routine maintenance on a borer miner. While stepping onto the bore miner rotor bit block, EE's foot slipped, resulting in a fall and a laceration to EE's left calf from contact with the borer miner bit. The injury required sutures.
An employee was standing on the FCT ramp for a belt move and slipped on the fines. Once the employee slipped on the fines, they felt pain in right knee. MRI was completed 1/27/2025 and was determined to be reportable on 2/7/2025.
Employee was assisting in putting a piece of culvert back into place on BC-57. The cover was lifted over the handrail, set on the deck and then lifted to move into place. The employee positioned self to assist placement. In the process of pushing the cover into place EE leaned against a handrail. The handrail gave way and the employee fell approximately 10' to the ground.
A mine production crew pulling excess miner cable back to the power center had an employee watching the cable being pulled so they could stop them when the new cut location was reached. When finished, the employee stepped backward, stumbled and fell. In an attempt to catch themselves, they extended their arms backward while falling from ground level and injured right wrist.
Exiting feet first from a boiler EE had been working on, through an opening at the traveling grates, EE's feet were not on the outer ledge, so EE slid off the traveling grates, through an opening between the front ledge and the traveling grate that was twelve (12) inches wide, falling in the ash hopper approximately nine (9) feet below and suffered an ankle injury.
An employee was working on a roof replacing metal roofing. EE got to the end of the SRL and it pulled EE back. Causing EE to trip. EE tried to catch self with left arm.
Employee was attempting to clear the suction of an underflow pump that had plugged. The plug cleared and then they slipped while exiting the area and fell to their knees in the hot liquor on the ground, receiving burns to legs and arms.
Employee was walking down a flight of stairs on Cooling Tower 3 (CT-3). While descending the stairs, utilizing the handrail, EE stumbled, and caught self in a twisting motion, while applying weight to EE's left leg and knee. An MRI was completed indicating a torn meniscus, a fresh tear, as well as some other historical injuries. Surgery was performed.
Cleaning the kitchen floor with a mop and bucket, some of the soapy water had slushed out of the bucket. As EE was mopping the floor, EE slipped and fell on left side and shoulder.
