Miner slipped as they were getting into the haul truck due to icy conditions. Miner caught themselves from falling and pulled muscles and tendons in their arm and shoulder area.
The miner was working on a walkway greasing the plant and stepped backwards into a small opening next to the motor and lost his balance. He fell directly onto the motor which had a eye bolt sticking out of it. The miner fell only backwards landing on the motor and eye bolt.

EMPLOYEE WAS CLIMBING ONTO THE LOADER WHEN THE STEP BROKE. THE CABLE STEP HAD RUSTED IN SIDE THE LADDER, WHEN EMPLOYEE STEPPED ON THE LADDER THE CABLE BROKE, HE FELL TO THE GROUND SEPARATING H IS LEFT SHOULDER.
HE WAS ASSEMBLING SURGE BIN. HE WAS STANDING ON H-IRON ON THE REAR OF BIN 7 FT. FROM GROUND WHEN HE LOST HIS GRIP AND JUMPED BACKWARD TO GROUND INJURING RIGHT HEEL WHEN LANDING ON GROUND. LADD ERS WERE AVAILABLE ON THE SITE.
