Worker slipped on some ice when getting out of his vehicle and landed on his shoulder. Got up and brushed off and went to work. No accident report was filled out and no lost time for the injury. Throughout the year his shoulder was giving him trouble and was traced back to the fall he had in December. He held off Surgery until December 28 2015. No lost time until now.
Setting rails, backed up and fell over
Employee was changing the direction of the diamond wire saw, which was stuck in the stone. He was pulling on it w/a rope, which separated and employee fell backwards.
Employee was cutting a block with it tipped and he fell and hit the granite.
