Climbing onto loader to work on the engine. Slipped and fell onto left shoulder & left chest.

Standing on edge of berm turned to walk back down to road right boot stayed planted in soft material as rest of body turned knee popped.

Employee was recording hour meter and checking fire ext on GR113 when EE went to climb down off grader his foot slipped off step, fell backwards to the ground landing on molboard of machine.

Employee was walking down a cat walk when he stumbled on a step. He landed on his his left hand and thumb fracturing a bone in his thumb.

Employee walked down a ramp along side of a screen, at the bottom of the ramp is one step down to a platform. When the employee stepped onto the platform his left knee gave out on him. Employee stated there was no material on the platform and he was looking at the platform when he stepped onto it. The doctor diagnosed him with a sprained left knee.

EE WAS CHANGING MESH OR SIZING SCREEN - HE ATTEMPTED TO CROSS TO OPPOSITE SIDE OF SCREEN DECK AND SLIPPED DURING TRANSITION. HE FELL (SAME LEVEL) ON SCREEN METAL FRAME PUNCTURING HIS LEG. EXISTING WALKWAY SHOULD HAVE BEEN USED.

EMPLOYEE HAD BEEN ASSISTING IN SMALL LOGGING/CHOKE SETTING OPERATION. SEVERAL LOGS HAD BEEN STACKED ONTO ONE ANOTHER, AND EMPLOYEE WAS STEPPING DOWN OFF OF LOG STACK. AS EMPLOYEE STEPPED ONTO BOTTOM-MOST LOG, HE STEPPED ONTO A SECTION OF THE LOG WITH NO BARK. THE LOGS WERE ALSO SLIPPERY DUE TO RAIN WEATHER. EMPLOYEE SLIPPED AND LANDED WITH RIGHT LEG BENEATH HIM INJURING RIGHT KNEE.

EMPLOYEE WAS RU3NING A ROAD GRADER.WHILE GETTING OFF OF GRADER HE SLIPPED ON A WET TIRE.HE CAUGHT HIMSELF INSTEAD IF FALLING.HE COULD NOT STRAIGHTEN UP.HE COMPLETED REST OF SHIFT BEFORE GOING TO DOCTOR. 3

EMPLOYEE WAS SH2VELING DIRT OFF A CONVEYOR BELT WALKING BACKWARDS RAPIDILY HE STEPPED OFF OF THE WALKWAY BACKWARDS FALLING 2 OR 3 STEPS CATCHING HIMSELF WITH HIS ARMS ON THE HAND RAIL
