The employee was preparing to grease the skid steer with an air grease as he was unrolling the grease line he started walking backwards not realizing how close the skid steer was parked he tripped backwards onto the bucket. The result of fall caused a left hip /lower back contusion

Mostrando los 18On 10/16/12, EE was exiting the cat skid steer. Upon exiting, EE hit her shin on the cab which possibly cause here to slip. EE reported the accident to another EE on 10/23/12. On 10/24/12 EE showed me how she thought she may have hurt her thumb-wrist area-EE was taken to the doctor where she was given a brace-she was cleared to return to her full duty as an operator.
Injured was stepping over the tailgate of his pickup truck, he placed his left foot on the bumper of the truck and his left hand on top at the tailgate. He swung his right leg over the tailgate. His left foot slipped off the bumper causing him to fall on his left side.
EE was adjusting the CB antenna, as he walked back across the walkway of the loader, he tripped over a shovel that was laying on the walkway, lost his balance and hit his knee on the hood handle.
EE WAS RESETTING E STOP (SAFETY SWITCH) AFTER CONVEYOR HAD STOPPED. HE WAS PULLING HIMSELF UP ON CONVEYOR FRAME AND SLIPPED, AND FELL INTO SKID LOADER BUCKET THAT WAS BELOW HIM. CONVEYOR WAS 3 -4' OFF GROUND.
EE WAS WALKING DOWN SLOPED CATWALK GRATED. WHICH WAS FREE OF SNOW & ICE BUT WAS SLICK WITH FROST, & SLIPPED & FELL STRIKING LEFT WRIST.
EMPLOYEE WAS WALKING DOWN STEEL GRATED STEPS AND ONE WAS PARTIALLY DISCONNECTED AND LOOSE FROM FRAME. HE STEPPED DOWN AND HE SLIPPED AND FELL ON HIS BACK. THERE WAS NO VISIBLE INJURY, BU THE W AS SENT TO THE DOCTOR AND HAD A MILD CONTUSION ON THE BACK.
EE WAS STANDING ON A 36" MAINLINE CONVEYOR 3-6' ABOVE GROUND LEVEL & LOST HIS BALANCE. IN TRYING TO AVOID FALL HE SLIPPED AND FELL SPRAINING HIS RIGHT ANKLE.
EE SLIPPED AND FELL APPROX. 10 FT TO THE GROUND WHILE WALKING ON A CONVEYOR BELT WHICH IS A VIOLATION OF CO POLICY CO WORK RULES. AT THE TIME OF THE ACCIDENT WORK WAS BEING DONW ON THE TAIL PU LLEY OF THIS CONVEYOR WHICH IS AT GROUND LEVEL. WHEN HE WAS ASKED WHY HE WAS ON THE CONVEYOR HE STATED ID ONT KNOW WHY I WAS UP THERE POST ACCIDENT DRUG SCREENING WAS POSITIVE FOR COCAINE MET
PATIENT ALLEGES HE TRIPPED OVER CENTER BLOCK AND INJURED HIS LEFT WRIST.
CLAIMANT JUMPED FROM A CONVEYOR & TWISTED HIS BACK
CLAIMANT SLIPPED AND FELL ON METAL STEPS.
EMPLOYEE ALLEGES HE SLIPPED ON ICE, FELL BACKWARD AND HIS HAND STRUCK AN IRON BEAM.
SLIPPED ON SNOW ON STEPS TO TRAILER AND FELL ON STEP ON HIS ARM.
SLIPPED & FELL AGAINST CONVEYOR BRUISING RIGHT ELBOW.
