Employee walked to the rear of the haul truck to check lights for the pre-trip inspection. EE twisted ankle on the berm behind the truck. Upon examination, EE has edema and bruising in the heel. Employee placed in a CAM boot, modified duty until MRI is taken.
EMPLOYEE WAS WALKING FROM THE WASTE OIL TANK TO THE SERVICE TRUCK. EE SLIPPED AND FELL AND TRIED TO CATCH SELF. WHEN EE TRIED TO CATCH SELF, EE TORE THEIR BICEP.
Employee jumped off of a service truck to the ground instead of using the steps on the truck.
Employee was walking in the mine. Employee slipped and fell, when employee fell employee put arm out to catch self and jammed shoulder. Employee initially did not seek medical attention but after recently having an MRI, it shows damage that will have to be operated on.
Employee got a foot stuck in mud, they tripped and fell forward hitting head on a rock. Hard hat prevented any lacerations but they strained their neck muscles upon impact.
Employee was refueling a loader, when finished employee stepped off of the machine, missed the last step and fell.
Employee was opening up the entrance gates to the quarry, slipped on the ice and fell, hitting his head causing a laceration.
Employee took the radiator cap off of the machine while it was still under pressure. To escape the spray he jumped backwards, hitting his arm and back on the machine carriage.
Employee was dragging a water hose, he slipped and fell, injuring his shoulder.
Employee was climbing into his truck at the beginning of his shift. He had only one hand on the ladder. When he stepped up he lost his balance and was hanging on with the one arm causing a sprain/strain to his shoulder.
Employee was climbing down, off of the machine, when he stepped down his foot slipped and he fell against the catwalk of the machine.

Employee was riding in a truck. When he went to step down out of the truck he missed the grab rail with his right hand, but was able to catch himself with his left hand. When he caught himself he thinks he pulled a muscle or something.

Employee reports recurrence of symptoms from a previous incident dated 8/16/04 in which he missed 39 work days. His chiropractor documented that the employee could have a potential of flare ups with work activities.
EE WAS ATTEMPTING TO REMOVE AN OBSTRUCTION IN THE TRANSFER BIN TO THE CONVEYOR. WHILE DOING THIS TASK, HE ENTERED THE BIN WHEN HE EXITED THE BIN HE SLIPPED AND FELL 10 TI 11 FT. TO THE GROU ND BELOW. THIS AREA HAD LOOSE SHOT ROCK (10"MINUS) ON THE GROUND WHICH HELPED INFLICT HIS INJURIES.
THE STEP ON BULLDOZER BROKE UNDER WEIGHT OF THE EMPLOYEE.
EE WAS EXITING VEHICLE, HE PUT HIS FOOT ON THE FUEL TANK FROM WHICH THE TREADPLATE WAS MISSING, HE SLIPPED & HE LANDED ON HIS ANKLE, CAUSING AN ANKLE SPRAIN. TREADPLATE HAS SINCE BEEN REPLACED .
EE WAS WORKING REPAIRING CONVEYOR BELT. EE STATED THAT WHEN HE JUMPED TO THE GROUND HE EXPERIENCED PAIN IN HIS BACK AND LEG.
EMPLOYEE WAS UNLOADING CONVEYOR ROLLERS; HE TRIPPED OVER A ROLLED UP TARP, TWISTING HIS KNEE AS HE DID.
