While carrying a ladder the wind blew a door open catching the employee foot causing them to fall. employee fell on to a concrete floor striking their left kneecap. Employee did not seek medical until 11/27/23 where they were prescribed an antibiotic.

Mostrando los 13Employee exiting the cab of an excavator/scaler caught foot in the groove of the track and rolled left ankle. Employee went to the doctor on 4/14/23 where x-ray shows a broken ankle.
Employee using a shovel removing spillage on catwalks. Area was provided with handrails to prevent fall hazards. Somehow the employee took a shortcut and positioned self beyond the protection of the handrails, lost control and fell to the ground. Employee was transported to a hospital for evaluation. All xrays and CT scans were negative. Employee was prescribed a muscle relaxer.
While working on a feeder pan the employee slipped causing a welding rod in the welding rod bag to cut EE's side requiring stitches.
While exiting the CAT 140M grader employee was stepping down to the ground when boot hung up in the step. Employee lost balance and began to fall backwards. Employee grabbed the hand hold with right hand and heard a pop in shoulder. Employee was put on restricted duty for shoulder strain to prevent further damage.
Employee was on landing cleaning windows of Kawasaki 115Z7 loader. Somehow fell through step opening on railing 7'8" to the ground. No witnesses. Contusion on right knee. Sprain left hip.
EE started down a flight of stairs in office, slipped and fell 5 or 6 steps to landing. Transported to hospital. Hospital found nothing wrong and released ee to return to work, no restrictions. EE continued to have pain in shoulder. MRI 12/22/2018. On 12/26/2018 doctor informed ee of a rotator cuff tear. EE still is released to work as of 1/7/2019.
EE was loading explosives at the face. EE tripped and fell on his left hip. He told supervisor on 9/11/09. On 9/11/09 EE went to doctors and found out it was a bruised bone in his hip.
Trip and fall; uneven height on stairs. Employee's knee swelled up.
EE was going to get in his drill and slipped on the step and pulled his shoulder, his boots were muddy. Streched tendon. RTW on 5/1/07, but has a few minor restrictions.
Catwalk on the haul truck was a little moist from exiting the mine. When EE was getting down from the catwalk, his foot slipped and he caught himself before he fell, resulting in an injury to his shoulder.
Employee attempted to drag a cardboard box out of the back of a pickup truck. The cardboard ripped and he fell off the back of the truck, hitting his head.
