While coming down from changing breathers on hyd tank, employees left foot slipped from bottom step of the ladder. He landed on the edge of his foot causing it to twist to the left. He was taken to the hospital and given a full duty release but on a follow up exam (05/06/08) it was discovered that he had soft tissue damage and was put on Restricted Duty.

Employee was walking between core tables and to avoid some water she stepped over to the right side. The grate was sticking up a 1/2" above the others and her toe caught on the grate. Employee slipped on ice and fell to ground fracturing a bone in his right foot.

EE WAS GETTING OFF THE 451 TIGER, HIS FOOT SLIPPED OFF THE STEP AND LANDING ON HIS RIGHT KNEE HESTRAINED HIS KNEE.

EMPLOYEE WAS WORKING IN FIRE ROOM OF ASSAY LAB WHEN SHE EXPERIENCED AN APPARENT SEIZURE AND FELL STRIKING HER HEAD AND CAUSING A 1" LACERATION WHICH REQUIRED ONE SUTURE TO CLOSE.
HELPER SLIPPED ON PLYWOOD DECK PLACED TO WORK ABOVE MUD. HE TRIED TO CATCH HIMSELF AND STRUCK HIS ARM AGAINST TUBE RACK.
EE WAS WALKING OUTSIDE THE MILL BUILDING WHEN SHE SLIPPED IN THE SNOW AND INJURED HER LEFT ANKLE. XRAYS SHOWED A FRACTURE OF THE LEFT ANKLE. FRACTURE WAS CARED FOR AND EE PLACED ON LIGHT DUTY.
EE WAS STEPPING OFF OF SHOVEL LADDER WHICH WAS NOT FULLY EXTENDED AND HE DROPPED TO THE GROUND CAUSING A CRACK FRACTURE OF HIS LEFT HEEL. HE WAS PLACED ON LIGHT DUTY.
EE WAS WALKING UP STAIRS WHEN HE TWISTED HIS RIGHT KNEE. EE'S CONDITION TO DETERIORATE & EE HAD ORTHOSCOPIC SURGERY ON 4/11/01 & WAS PLACED ON LIGHT DUTY UNTIL 6/7/01. EE WAS ON LIGHT DUTY FOR A PERIOD OF 26 DAYS.
EMPLOYEE WAS HOPPING ACROSS A PUDDLE WHEN HE TWISTED HIS LEFT ANKLE CAUSING A SPRAIN AND POSSIBLE FRACTURE. EMPLOYEE WAS PLACED ON LIGHT DUTY AND RETURNED TO WORK ON NEXT SHIFT.
EE WAS STEPPING OFF FROM DRILL ONTO GROUND WHEN HE TWISTED HIS LEFT ANKLE. EE WAS EVAULATED BY ADR & WAS ON LIGHT DUTY FOR 1 DAY WITH ANKLE SPRAIN
EMPLOYEE WAS EXITING SERVICE TRUCK WHEN HE STEPPED ONTO SOME LOOSE ROCKS AND TWISTED HIS RIGHT ANKLE. EMPLOYEE WAS EVALUATED BY A PHYSICIAN WHO DETERMINED THE ANKLE WAS SPRAINED AND PLACED EMP LOYEE ON LIGHT DUTY FOR 2 DAYS.
