While coming down from changing breathers on hyd tank, employees left foot slipped from bottom step of the ladder. He landed on the edge of his foot causing it to twist to the left. He was taken to the hospital and given a full duty release but on a follow up exam (05/06/08) it was discovered that he had soft tissue damage and was put on Restricted Duty.

Employee was assisting loading blast holes when he fell backwards into a 4 foot deep hole. He remained working for about one hour, and then collapsed. He was diagnosed with a massive concussion.

While employee was walking ground gave way and employee fell backwards into a hole and left ankle was caught between two rocks and twisted

Employee was walking between core tables and to avoid some water she stepped over to the right side. The grate was sticking up a 1/2" above the others and her toe caught on the grate. She tried to catch herself as she was falling but she twisted her body and fell on her right side. She went to the doctor on 5/27/08 and this was when it became a Lost Time.

Employee slipped on ice and fell to ground fracturing a bone in his right foot.

Employee was climbing ladder to empty dust pots and slipped. He spun around and slid down the ladder. His arm got caught on a bolt causing a laceration.

The operator was climbing up on his machine and slipped off causing the operator to land on the back of his heel twisting his ankle. He wasn't reportable until a follow up visit on 9/18 showed he had a torn achilles tendon.

The employee climbed up onto the tie rod on one of the Liebherr T282B haul trucks so he could tigthen an overhead fitting. The employee slipped on the tie rod, lost his balance and fell approximately 4 feet to the ground, landing on his right foot.

Tripping rods in. After connection, stepped back and stepped into deck hole for rods. Fell against stool straining back.

Individual was on a ladder, tightening a hydraulic fitting when the fitting broke, releasing hydraulic oil. This startled the individual which caused him to step back off the ladder, he then fell to the ground and injured his right elbow and wrist.

EE was walking towards 505 water truck to work on it when he slipped and fell on the ice.

EE was getting off work she went to load into the Cortez Van when she slipped and fell on the ice causing a strain to her right shouder. Resulting in modified work

While walking up the stairs at mine operations,he reached the top step and slipped on the top of the walk way falling on his right knee and hitting his right shoulder against the building causing(A strain to his right shoulder at the rotater cuff)

The lifting eye pulled out of the cyclone causing the cyclone to fall, EE backed up stumbeled striking his right elbow on the grating causing a laceration.

In adverse weather conditions, a driller helper stepped down from a water truck, slipping and twisting his knee.

Employe was washing down in #8 agitator when he slipped in the mud and hit his left hand against a support beam causing a laceration to his L index finger.

Got out of pickup, slipped on ice, pulled leg muscles. 2 days sedentary duty.

EE SAID HE WAS DISMOUNTING DRILL #26 WHEN HE SLIPPED CAUSING A STRAIN TO HIS LEFT WRIST.

Tripping pipe out of hole, he slipped and bent his finger back. He hyperextended his finger.

Ee was walking to the pickup truck when he fell & injured his shoulder. Snow on the ground may have contributed to the fall. Ee can give no explanation for the fall & there are no witnesses.

Employee was putting away a tool in upper tool box on water truck. His foot slipped off the step, then his hand slipped off the hand rail and he fell about 3 feet to the ground. He put out his left hand to catch his fall and fractured his left wrist.

Wind gust blew up edge of liner, EE was standing on the liner which caused him to lose his footing.

EE SLIPPED ON A WORK DOCK 1HIL CARRYING A CASING SUB. SUB THEN LANDED ON HIS ANKLE, INJURING A LIGAMENT. ORIGINALLY THOUGHT TO BE A FRACTURE, XRAY SHOWED NO FRACTURE.

EE WAS PUMPING FUEL INTO TANKS ON BACK OF PICKUP. HOSE WAS TANGLED WHILE STRAIGHTENING HOSE, HE SLIPPED AND CUT LEFT SHIN ON SIDEBOARD OF PICKUP BOX, REQUIRED 3 STITCHES.

EE WAS GETTING OFF THE 451 TIGER, HIS FOOT SLIPPED OFF THE STEP AND LANDING ON HIS RIGHT KNEE HESTRAINED HIS KNEE.

EMPLOYEE WAS REMOVING ROCK FROM DOZER ARM WHEN HE STEPPED BACK, LOST HIS FOOTING AND FELL LANDING ON HIS OUTSTRETCHED RIGHT ARM CAUSING FRACTUREOF RIGHT WRIST. EMPLOYEE PLACED ON LIGHT DUTY FOR ONE DAY THE RELEASED TO FULL DUTY WITH BRACE.

WHILE STEPPING OFF AN 830E 240 TON HAUL TRUCK THE EMPLOYEE TWISTED HIS LEFT KNEE. THE PAIN CONTINUED AND THE INJURY FINALLY REQUIRED SURGICAL REPAIR ON 8/24/01. EMPLOYEE RETURNED TO WORK ON NE XT SHIFT LIGHT DUTY.

EMPLOYEE WAS WORKING IN FIRE ROOM OF ASSAY LAB WHEN SHE EXPERIENCED AN APPARENT SEIZURE AND FELL STRIKING HER HEAD AND CAUSING A 1" LACERATION WHICH REQUIRED ONE SUTURE TO CLOSE.

HELPER SLIPPED ON PLYWOOD DECK PLACED TO WORK ABOVE MUD. HE TRIED TO CATCH HIMSELF AND STRUCK HIS ARM AGAINST TUBE RACK.

EE WAS WALKING OUTSIDE THE MILL BUILDING WHEN SHE SLIPPED IN THE SNOW AND INJURED HER LEFT ANKLE. XRAYS SHOWED A FRACTURE OF THE LEFT ANKLE. FRACTURE WAS CARED FOR AND EE PLACED ON LIGHT DUTY.

EE WAS STEPPING OFF OF SHOVEL LADDER WHICH WAS NOT FULLY EXTENDED AND HE DROPPED TO THE GROUND CAUSING A CRACK FRACTURE OF HIS LEFT HEEL. HE WAS PLACED ON LIGHT DUTY.

EE WAS WALKING UP STAIRS WHEN HE TWISTED HIS RIGHT KNEE. EE'S CONDITION TO DETERIORATE & EE HAD ORTHOSCOPIC SURGERY ON 4/11/01 & WAS PLACED ON LIGHT DUTY UNTIL 6/7/01. EE WAS ON LIGHT DUTY FOR A PERIOD OF 26 DAYS.

EMPLOYEE WAS HOPPING ACROSS A PUDDLE WHEN HE TWISTED HIS LEFT ANKLE CAUSING A SPRAIN AND POSSIBLE FRACTURE. EMPLOYEE WAS PLACED ON LIGHT DUTY AND RETURNED TO WORK ON NEXT SHIFT.

EE WAS STEPPING OFF FROM DRILL ONTO GROUND WHEN HE TWISTED HIS LEFT ANKLE. EE WAS EVAULATED BY ADR & WAS ON LIGHT DUTY FOR 1 DAY WITH ANKLE SPRAIN

EMPLOYEE WAS EXITING SERVICE TRUCK WHEN HE STEPPED ONTO SOME LOOSE ROCKS AND TWISTED HIS RIGHT ANKLE. EMPLOYEE WAS EVALUATED BY A PHYSICIAN WHO DETERMINED THE ANKLE WAS SPRAINED AND PLACED EMP LOYEE ON LIGHT DUTY FOR 2 DAYS.

EE WAS HELPING LOAD TRAILER AS JOB WAS COMPLETED. HE WAS STANDING ON THE TRAILER GUIDING THE BACKHOE IN, WHEN HE STEPPED TO THE SIDE AND STEPPED OFF THE TRAILER. FELL ABOUT 7 FEET TO GROUND.

EMPLOYEE WAS LIFTING A ROTARY DRILL BIT ONTO THEDRILL, WHEN HE SLIPPED ON THE MUDDY SURFACE AND TWISTED HIS LOWER BACK. EMPLOYEE WAS SEEN BY A DOCTOR FOR MUSCLE STRAIN AND PLACED ON LIGHT DUTY

EE WAS STEPPING OVER END OF PALLET AT THE WAREHOUSE WHEN HE FELT PAIN IN HIS LOWER BACK. EE WAS EVALUATED BY A DR. WHO RECOMMENDED 1 DAY OF LIGHT DUTY.

EMPLOYEE WAS EXITING A BOBCAT LOADER WHEN HE SLIPPED AND FELL CAUSING A SCRAPE ON HIS RIGHT ASHIN. EMPLOYEE WAS SEEN BY A DOCTOR ON 3-30-99 AND THE WOUND WAS FOUND TO BE INFECTED. THE DOCTO R PRESCRIBED ANTIBIOTICS AND PLACED THE EMPLOYEEON LIGHT DUTY.

HE WAS ATTEMPTING TO PUT A BEARING CASE ON A 48 IN. ROLLER. THE HOUSING MOVED, CATHCING HIS LEFT LITTLE FINGER BETWEEN THE TABLE EDGE AND SHARP EDGE OF BEARING HOUSING. REQIRED 6 STITCHES.

WHILE PULLING ON A WRENCH EE SLIPPED, HITTING HIS HEAD ON THE DRIVELINE OF TRUCK 504.

EMPLOYEE WAS TRANSFERRING TIE POINNT FROM ONE LOCATION TO ANOTHER. HE SLIPPED ON A 1 1/4" PIPE, FELL OFF THE END OF A PLATFORM AND LANDED ON A PIPE RACK BELOW.

EE WAS GETTING OFF HER TRUCK AT QUITTING TIME WHEN SHE SLIPPED FALLING TO THE GROUND & LANDING ON HER HEEL

WHILE STANDING ON A LADDER & LIFTING AIR BOX FOR INSTALLATION, EE LOST FOOTING & TWISTED LEFT KNEE. INJURY RESULTED IN SURGERY ON LEFT KNEE.

EE WAS CLIMBING DOWN OFF THE 82 TON CRANE WHEN HE SLIPPED & FELL, PINNING HIS RIGHT LEG UNDER HIM.

EE SLIPPED ON MATERIAL & FELL AGAINST STRUCTURE. INJURING HIS COLLAR BONE.

EE WAS CLIMBING A COLUMN IN MILL BUILDING AROUNDGRINDING GLAD WATER TANK AREA. EE HAD CLIMBED ABOUT 20' WHEN HE DECIDED HE COULD NOT MAKE THE REST OF THE CLIMB. HE STARTED TO SLIDE BACK DOWN THE COLUMN BUT COULD NOT CONTROL HIS SPEED. THE ROPE GRAB FUNCTIONED, HIS MOMENTUM CAUSED HIM TOBECOME INVERTED AND HE HIT HIS HEAD ON A GRATINGFLOOR 2' BELOW HIM.

EE WAS WALKING AROUND FORMS AND TRIPPED OVER REBAR. EE FELL ON HIS RIGHT KNEE.

CREW JUST HAD A SAFETY MEETING. THEY WERE IN THE PROCESS OF MOVING THE STATIC LINE TO RE TIE OFF AT THE RIDGE LINE TO INSTALL RIDGE VENTING. THERE WAS AN 18" WIDE OPENING ALONG THE ENTIRE RIDG E LINE. EE WALKED UP TO THE RIDGE LINE FACING IT, GOT TO THE EDGE OF THE OPENING THEN TURNED AROUND & STEPPED BACK INTO IT & FELL THROUGH.

EMPLOYEE WAS UNLOADING REFUGE AT LANDFILL WHEN SHE SLIPPED & FELL TO THE GROUND. SHE WAS STANDING ON THE GROUND WHEN SHE LOST HER FOOTING. THE FALL RESUTLTED IN A FRACTURE TO HER LEFT WRIST.
