While coming down from changing breathers on hyd tank, employees left foot slipped from bottom step of the ladder. He landed on the edge of his foot causing it to twist to the left. He was taken to the hospital and given a full duty release but on a follow up exam (05/06/08) it was discovered that he had soft tissue damage and was put on Restricted Duty.

While employee was walking ground gave way and employee fell backwards into a hole and left ankle was caught between two rocks and twisted
Employee was walking between core tables and to avoid some water she stepped over to the right side. The grate was sticking up a 1/2" above the others and her toe caught on the grate. She tried to catch herself as she was falling but she twisted her body and fell on her right side. She went to the doctor on 5/27/08 and this was when it became a Lost Time.
Employee slipped on ice and fell to ground fracturing a bone in his right foot.

EE WAS GETTING OFF THE 451 TIGER, HIS FOOT SLIPPED OFF THE STEP AND LANDING ON HIS RIGHT KNEE HESTRAINED HIS KNEE.

EMPLOYEE WAS WORKING IN FIRE ROOM OF ASSAY LAB WHEN SHE EXPERIENCED AN APPARENT SEIZURE AND FELL STRIKING HER HEAD AND CAUSING A 1" LACERATION WHICH REQUIRED ONE SUTURE TO CLOSE.
HELPER SLIPPED ON PLYWOOD DECK PLACED TO WORK ABOVE MUD. HE TRIED TO CATCH HIMSELF AND STRUCK HIS ARM AGAINST TUBE RACK.

EE WAS STEPPING OFF OF SHOVEL LADDER WHICH WAS NOT FULLY EXTENDED AND HE DROPPED TO THE GROUND CAUSING A CRACK FRACTURE OF HIS LEFT HEEL. HE WAS PLACED ON LIGHT DUTY.
EE WAS WALKING UP STAIRS WHEN HE TWISTED HIS RIGHT KNEE. EE'S CONDITION TO DETERIORATE & EE HAD ORTHOSCOPIC SURGERY ON 4/11/01 & WAS PLACED ON LIGHT DUTY UNTIL 6/7/01. EE WAS ON LIGHT DUTY FOR A PERIOD OF 26 DAYS.
EMPLOYEE WAS HOPPING ACROSS A PUDDLE WHEN HE TWISTED HIS LEFT ANKLE CAUSING A SPRAIN AND POSSIBLE FRACTURE. EMPLOYEE WAS PLACED ON LIGHT DUTY AND RETURNED TO WORK ON NEXT SHIFT.

EMPLOYEE WAS EXITING SERVICE TRUCK WHEN HE STEPPED ONTO SOME LOOSE ROCKS AND TWISTED HIS RIGHT ANKLE. EMPLOYEE WAS EVALUATED BY A PHYSICIAN WHO DETERMINED THE ANKLE WAS SPRAINED AND PLACED EMP LOYEE ON LIGHT DUTY FOR 2 DAYS.
EE WAS HELPING LOAD TRAILER AS JOB WAS COMPLETED. HE WAS STANDING ON THE TRAILER GUIDING THE BACKHOE IN, WHEN HE STEPPED TO THE SIDE AND STEPPED OFF THE TRAILER. FELL ABOUT 7 FEET TO GROUND.

EMPLOYEE WAS EXITING A BOBCAT LOADER WHEN HE SLIPPED AND FELL CAUSING A SCRAPE ON HIS RIGHT ASHIN. EMPLOYEE WAS SEEN BY A DOCTOR ON 3-30-99 AND THE WOUND WAS FOUND TO BE INFECTED. THE DOCTO R PRESCRIBED ANTIBIOTICS AND PLACED THE EMPLOYEEON LIGHT DUTY.
HE WAS ATTEMPTING TO PUT A BEARING CASE ON A 48 IN. ROLLER. THE HOUSING MOVED, CATHCING HIS LEFT LITTLE FINGER BETWEEN THE TABLE EDGE AND SHARP EDGE OF BEARING HOUSING. REQIRED 6 STITCHES.
WHILE PULLING ON A WRENCH EE SLIPPED, HITTING HIS HEAD ON THE DRIVELINE OF TRUCK 504.
EMPLOYEE WAS TRANSFERRING TIE POINNT FROM ONE LOCATION TO ANOTHER. HE SLIPPED ON A 1 1/4" PIPE, FELL OFF THE END OF A PLATFORM AND LANDED ON A PIPE RACK BELOW.
EE WAS GETTING OFF HER TRUCK AT QUITTING TIME WHEN SHE SLIPPED FALLING TO THE GROUND & LANDING ON HER HEEL

HE STARTED TO SLIDE BACK DOWN THE COLUMN BUT COULD NOT CONTROL HIS SPEED. THE ROPE GRAB FUNCTIONED, HIS MOMENTUM CAUSED HIM TOBECOME INVERTED AND HE HIT HIS HEAD ON A GRATINGFLOOR 2' BELOW HIM.
EE WAS WALKING AROUND FORMS AND TRIPPED OVER REBAR. EE FELL ON HIS RIGHT KNEE.
CREW JUST HAD A SAFETY MEETING. THEY WERE IN THE PROCESS OF MOVING THE STATIC LINE TO RE TIE OFF AT THE RIDGE LINE TO INSTALL RIDGE VENTING. THERE WAS AN 18" WIDE OPENING ALONG THE ENTIRE RIDG E LINE. EE WALKED UP TO THE RIDGE LINE FACING IT, GOT TO THE EDGE OF THE OPENING THEN TURNED AROUND & STEPPED BACK INTO IT & FELL THROUGH.
EMPLOYEE WAS UNLOADING REFUGE AT LANDFILL WHEN SHE SLIPPED & FELL TO THE GROUND. SHE WAS STANDING ON THE GROUND WHEN SHE LOST HER FOOTING. THE FALL RESUTLTED IN A FRACTURE TO HER LEFT WRIST.
