The HVAC vent covers in the floor of the office building/ lab were removed for floor cleaning and waxing. Mill operator was exiting the lab area when EE stepped on said opening, making EE loose EE's balance, leading to a fall. Employee was transported to hospital via local ambulance service. Employee was seen by hospital ED for pain in knee. Xray's indicated no broken bones.

Mostrando los 27Mechanic was crawling out of under packers at 4Spout when feet got tangled up in each other. EE fell from packer platform on hands and knees approximately 30" from packer platform to concrete floor below onto L knee. EE reports no pain and no injury other than a 2" knot above L knee. Mobility is not restricted. EE does not want to go to ER or leave work.
Shipper had released a truck and was walking backwards. Tripped over a wheel chock & fell, striking buttocks, hip, & right shoulder on the ground on 9/26. They did not report it immediately. 9/28 reported pain to management. Physical therapy given, but could return to work with no restrictions. Follow-up 10/28, restriction due to ongoing pain in R shoulder.
The EE placed his body against the safety chain at the north loading dock door & leaned out of the opening to look south in order to determine if there were any trucks waiting to be loaded. The bolt at the chain anchor point broke, & the EE was pitched forward out the loading dock door opening. HE traveled downward approximately 4', landing on his feet in an upright position.
EE was retrieving a garden hose from the well pump house and was reaching for the water hose spigot when he encountered the snake. The snake struck at him, causing him to jump back to avoid being bitten. The employee landed awkwardly on his left leg and fell to the ground. Upon attempting to stand, he experienced severe pain in his left knee. The result was a patella fracture.
EE was descending a vertical ladder when his foot slipped off a ladder rung. The ee had his hands on the outer frame of the ladder and was able to catch himself before hitting the ground. The ladder is approx 10 feet in height and the ee slid approx 5 feet before catching himself. The ladder is exposed to weather, and it was raining at the time of the incident.
EE was lifting a bag of product from the weigh scale to the pallet, when he tripped on a hydraulic line below the pallet press. EE fell onto the pallet and cut his left thumb knuckle on the pallet.
Employee had descended a ladder when he tripped over a piece of conduit channeling welded to the top of the bin. Employee fell to the bin top flooring injuring his left wrist, both shoulders, and collarbone.

EE STEPPED ON A LOOSE ROCK, SLIPPED AND FELL ONTO THE WALKWAY STRIKIN G HIS BACK ON ONE OF THE ROCKS CAUSING A CONTUSION TO HIS LOWER BACK THE EE WAS TAKEN FOR A MEDICAL EVALUATION, PROVIDED MEDICATION AND WAS PLACED ON MODIFIED DUTIES.
EE TWISTED HIS RIGHT KNEE WHILE TURNING CORNER WHILE WALKING ALONG WALKWAY CONSTRUCTED OF STEEL GRATING. THE SPRAIN RESLUTED IN RESTRICTIVE DUTIES.
EE STEPPED INTO THE 6 INCH GAP BETWEEN THE WOOD PALLET AND THE EDGE OF THE PALLET LIFT (PALLET JACK) WITH HIS LEFT LEG. THE EE SPRAINED HIS LEFT KNEE CAUSED FROM THE LEG ENTERING THE THE SMAL L AREA. EMPLOYEE HAS BEEN PLACED ON LIGHT DUTY.
EE WAS DESCENDING A THREE STEP LADDER ON THE VIT-GOBE BLENDER OPERATION WHEN HIS FOOT SLIPPED OFF OF THE METAL RUNG. THIS CAUSED HIM TO FALL BACK AND STRIKE HIS RIGHT ELBOW ON A METAL HANDRAIL RESULTING IN A BRUISE TO HIS ELBOW. THE EE WAS PLACED ON MODIFIED DUTY.
EE'S FOOT SLIPPED WHILE ASCENDING THE LADDER TO THE PALLETIZER, CAUSING HIM TO FALL AND CATCH HIMSELF ON THE RUNG OF THE LADDER. THE WEIGHT OF HIS BODY CAUSED HIM TO STRAIN HIS LEFT WRIST DURI NG THIS MANEUVER.
EMPLOYEE MISPLACED HIS FOOT ON BOTTOM STEP WHILEDESCENDING STEPS. HE FELL FROM MACHINERY CAUSINGEMPLOYEE TO STRIKE HIS HAND AFTER STRIKING THE FLOOR, CAUSING A BRUISE. HE WAS PLACED ON RESTRIC TED DUTY UNTIL PAIN AND SWELLING SUBSIDED.
EE WAS DESCENDING STAIRS WHEN HE TRIPPED AND FELL AGAINST THE HANDRAIL FOR THE STEPS CAUSING A BRUISE TO HIS LEFT SIDE AND RIBS. X RAY TAKEN WAS NEGATIVE. CONTINUED PAIN RESULTED IN RESTRICTED DUTIES.
EE WAS WELDING A COVER PLATE ABOVE S-1SCREEN DRIVE SHAFT. HE HAD TO GO INSIDE OF MACHINE TO DO THIS. THERE IS UNEVEN FOOTING INSIDE OF THE SCREEN. EE WAS STANDING WITH EACH FOOT ON TOP OF TW O LIFTTING LUGS FOR SUPPORT. THE NEXT DAY HE WAS COMPLAINING OF HIS FOOT AND THIGH HURTING. AFTER CONTINUED PAIN IN HIS THIGH EMPLOYEE AS TAKEN TO DR AND DIAGNOSED WITH MUSCLE STRAIN AND WAS
WORKER HAD STEPPED UPON A SIX(6) INCH WOODEN PALLET TO GET SHIPPING SUPPLIES. AS HE WAS LEAVING, HE TURNED AND STEPPED, FORGETTING ABOUT BEING ON A PALLET. THE SUDDEN DROP CAUSED HIM TO STRAIN A MUSCLE IN HIS NECK.
EMPLOYEE WAS STEPPIGN DOWN ONTO A WET FLOOR AFTER SERVICING EQUIPMENT. AS HIS FOOT HIT THE SLIPPERY SURFACE, HE LOST HIS FOOTINGANDHIT HIS KNEE ON A BRACE CAUSIG HIM TO TWIST HIS LOWER BACK. THE TWISTING ACTION CAUSED A STRAIN TO HIS LOWER BACK.
WORKER WAS MONITORING EQUIPMENT CONTROLS WHEN HE STEPPED OFF THE EDGE OF A BAR GRATE AND RUBBER MATTING APPROXIMATELY 2 INCHES TO THE CONCRETE FLOOR TWISTING HIS RIGHT ANKLE.
EMPLOYEE WAS CLIMBING DOWN FROM DUST COLLECTOR AFTER CLEANING WHEN HE SLIPPED AND WHEN HE GRABBED THE HANDRAIL,HIS THUMB BECAME HYPEREXTENDED CAUSING THE INSIDE TENDON TO BECOME DETACHED.THIS HAD TO BE SURGICALLY RECONNECTED.THE EMPLOYEE WAS ABLE TO RETURN TO REGUALR WORK DUTIES.
EMPL STEPPED OFF OF FORKLIFT WAS WALKING THROUGH WAREHOUSE. HE SLIPPED & FELL ON WET FLOOR WHICH HAD SMALL AMOUNT OF OIL ON IT FROM FORKLIFT. HE HIT FINGER ON FLOOR.
EMPLOYEE WAS WASHING DOWN AREA IN CLASSIFIER ROOM. HE STEPPED ON DUST COLLECTOR BAG, THAT WAS BEING USED AS A WATER STOP IN THAT AREA, HIS FOOT SLIPPED ON THE BAG AND HIS HEEL CAUGHT CAUSING E MPLOYEE TO FALL DOWN. HE TRIED CATCHING HIMSELF,CAUSING INJURY TO HIS RIGHT SHOULDER.
I,NAME LOST HIS3FOOTING WHILE WALKING ON WORK PLATFORM. AS HE FELL, HE TRIED TO CATCH HIMSELF ON A NEARBY FORLIFT. HE MISSED AND JAMMED HIS KNUCKLE ON HIS LEFT HAND AGAINST THE FORKLIFT. THE N EXT DAY, I,NAME3CLAIMED THE HAND WAS DAMAGED. HE WAS SENT TO DR. ON 6-9-87.
