EE CLIMBED ON FORKLIFT AND ONTO THE TOTE BIN TRYING TO ADJUST THE LOADING BULK CHUTE. HE LOST HIS BALANCE AND FELL APPROXIMATELY TEN FEET TO THE CONCRETE ROADWAY.
CLIMBING IN MIXER TRUCK, HE SLIPPED GRABBING THEHAND RAIL AND BROKE FALL WITH RIGHT ELBOW.
