The injured employee was working on top of the #1 Crystallizer at MVO. As ee took a step ee lost balance and put right hand down to help steady self. EE hand flexed open a thin sheet metal joint and right hand ring finger went into/under the sheet metal cutting the tip and side of finger.
The employee was washing down the bottom floor of her work area. She attempted to step over a rolled up wash hose that was still connected and hung her foot, causing her to fall and land on her right shoulder.
The injured employee was walking up the stairs to the area control room. The area's external process control alarm sounded, startling the employee, causing him to fall at the top of the stairs and land on his knees, arms and leg.

THERE IS APPROXM A 10 INCH DROP FROM THE CONTROL ROOM FLOOR TO THE WORK AREA FLOOR, AND AS HE MADE THE STEP HE LANDED ON HIS LEFT ANKLE, RESULTING IN A MILD SPRAIN.
EE WAS FUELING A FRONT-END LOADER AT THE FUELINGSTATION. HE TURNED TO WALK TO THE PUMP AND AS HESTEPPED OVER THE FUEL LINES HIS LEFT FOOT CAUGHTTHE LINE CAUSING HIM TO LOSE HIS BALANCE. AS HE FELL ALL OF HIS WEIGHT SHIFTED TO HIS RIGHT KNEEWHICH TWISTED UNDERNEATH HIM, CAUSING A STRAIN.
THE INJURED WAS CLIMBING DOWN A FIXED LADDER IN OUR PRODUCT PARKING AREA. AS HE EACHED THE BOOTOM OF THE LADDER HE MISSED THE LAST WRUNG, CAUSING HIM TO PUT ALL OF HIW WEIGHT ON HIS LEFT LEG K NEE AND LEG.
THE INJURED WAS INSPECTING THE LID OF A CLACINER, THAT WAS LOCATED IN THE SHOP. THE LID HAD BEEN TURNED UPSIDE DOWN SO THAT IT COULD BE INSPECTED AND NEW REFRACTORY WORK PERFORMED. AS THE INJU RED STEPPED OFF THE LID ONTO THE FLOOR HE STEPPED ON A PIECE OF BROKEN REFRACTORY CAUSING HIM TO FALL BACKWARDS. AS HE FELL HE CAUGHT HIMSELF WITH HIS HANDS, RESULTING IN INJURY TO R PINKY.
