Employee was going to grab something off the desk in the PM tent. Tripped and hit the corner of the desk. They informed a coworker who brought them to H.E. Shop. Employee went to the supervisors office and then to the clinic

IP was walking and slipped. IP attempted to break the fall by sticking out the right arm. Struck right hand on rocks on the ground, resulting in a laceration to the right middle finger requiring sutures.

Rainy and Muddy conditions caused a slip and fall when employee stepped on the bumper of the pickup. Employee struck employee's arm causing shoulder to dislocate.

Employee was walking to Water Treatment Plant #3. As the employee was approaching the building, slipped and fell to the ground landing on their left side.

Employee slipped on pickup side step and landed on one leg with all body weight, injuring left knee. Employee was getting into the pickup and left foot slipped off the running board landing on to left leg. Employee was escorted to the medic for further evaluation and was determined that a possible meniscus injury.

While moving PQ core tube, the drill helper lost footing on a clear work area. The tube fell from EE's grasp and EE re-grabbed the tube while it fell. EE's hand came in contact with a pipe 12" above deck level (door frame pipe). This contact with EE's hand between the tube and pipe resulted in a fracture to EE's Right Ring Finger.

While installing delineators on the new B-line ramp, an operator sprained an ankle on the uneven ground by the berm. Operator did not see a rock and stepped on it which sprained the ankle.

Mechanic was preparing to dismount the tracks of a CAT 336 excavator. As EE was dismounting the grease line hung on the track. Mechanic attempted to shake the hose loose and kick it free; ultimately losing balance and making contact with the ground below with the right shoulder.

Wash bay attendant was pulling the fire hose back into the bay. This was being done by pulling and backing up at the same time and tripped over the hose falling and hitting EE's head.

Employee was assisting removing the trunnion liner which broke loose of its interference fit and fell uncontrollably. The employee stumbled back and EE's foot became trapped between flange and floor.

Employee was walking through the Dry Storage Warehouse. Attempting to walk around and over the fork, right boot caught the fork, stumbled and tripped. Employee reached out right had to catch themselves and grabbed a part of the rack that had a steel burr that cut the right 5th finger.

Employee was crossing over a 10" and 26" HDPE pipe. Employee stepped onto the 10" pipe to get over the 26" pipe slipped and right knee landed on 26" HDPE pipe.

Employee stepped off the last step of a staircase onto pavement and twisted/sprained left ankle.

While exiting the steamer compartment of a steam truck, miner stepped down onto the ground and rolled right ankle.

While hosing down a floor, worker tripped over the hose, fell forward onto the concrete floor injuring right shoulder and left forearm.

While carrying a support beam out the main door of the fabrication shop with a co-worker, employee lost footing while transitioning from inside to outside of the shop causing the employee to fall and pinching right hand causing a minor laceration.

While using a pipe wrench an employee slipped and fell into the end of the wrench injuring his rib. Initially when seen at the clinic he required no medical treatment and was released to go back to work with no restriction. Two days later he returned to the clinic and was given restrictions.

Employee rolled his ankle while walking up stairs.

Employee was walking to office when he slipped on ice and obtained an ankle fracture.

Employee was walking back to the job site after lunch, he was walking up a flight of stairs and felt a pain in his knee.

As the employee was exiting out of the top of a culvert he slipped. He caught himself on the top of the culvert where there was a sharp edge. The employee was not wearing cut resistant gloves. Employee was wearing cut gloves earlier but took them off because they were torn.

A surface crew operator lost his balance entering the cab of a 966 loader. As he put his arm out to catch himself he hit his finger on the door frame.

While refueling a road grader the operator was exiting the machine coming down the ladder and slipped off the step and fell onto his right wrist.

On 04/14/13 Operator slipped on step causing an abrasion on his left shin. On 05/22/13 abrasion became infected requiring medical treatment.

While moving a hose, the workers foot caught the lip of a floor I beam, this action caused him to fall backwards and land on the floor. When he got up he noticed that his left hand was bleeding and his latex glove was ripped. A safety pin for a camplock fitting was in his hands as he fell. The safety pin twisted in his hand puncturing his glove and causing the laceration.

A mill operator strained her left shoulder while falling backwards in the chair when one of the chair legs fell through the floor grating.

A Concentrator Operator was attempting to open a suction valve. In the process of going back and forth from the control panel to the pump area, which was flooded with slurry, the Operator stepped off the side of a down sloping ramp and slipped backwards onto the step. The Operator suffered a bruised back and elbow and sprained his right shoulder.

Haul truck Operator lost balance and slipped while walking from pit trans. bus to Haul Truck#7721. This happened approx. 100 feet from the bus. No other operators on the ready line witness the slip. The operator left foot caught traction and as the rest of his body was in motion falling to the ground he twisted his left ankle.

A drill crew, with the assistance of an A-Star helicopter, was attempting to move a bundle of rods. The pilot carried the load of rods to the new site. The helper extended his arms to receive the load and steady it on the drill deck, but the helper stumbled. The helper grabbed hold of the suspended load of rods and then fell from the suspended load breaking his right leg.

While the employee was mixing mud on top of the mud deck, he removed a bucket of Quik Trol that was covering the bung access hole in the grating of the mud tank. The employee moved the empty bucket out of the way; this removed the cover on the access hole. The helper mixed mud when his right foot slipped into the access hole. He then lost his balance and twisted his ankle.

Icing conditions on the floor of a conveyor tube located under a coarse ore stock pile caused an employee to loose balance, traction, grip. The employee did not fall to the ground but reported joint discomfort. The employee was able to work his remaining rotation days, 8 days, then sought offsite medical evaluation confirming ligament/tendon strain.

A heavy equipment mechanic had just completed maintenance on a Caterpillar 777D and moved the truck outside onto a concrete staging pad outside the shop. The mechanic exited the cab and decsended the driverside ladder to the bumper level when he lost balance, traction and grip. He landed on the concrete floor, his left heel taking the force of the fall.

Employee was dismounting from the bed of a flatbed delivery truck, placed right foot on the ground, the ground was uneven and as a result the employee rolled right ankle.

A 36"x42" piece of grating was removed in a walkway in 2025. An employee was walking along this walkway and stepped through the open hole onto a valve, 16" below the deck. He then caught himself with his elbow on the toe board stopping his fall.

Operator was standing on the bottom rung of a ladder when he stepped back off the ladder he made contact with the joy compressor frame which caused his foot to roll over and twist his ankle.

Employee was walking on a slippery surface and fell twisting his left knee.

Employee tripped while he was crossing over a cement road barricade, injuring his arm.

Employee was climbing up the steps to the Caterpillar Compactor when he lost grip of the handrails and fell back onto the ground. The employee received lower back contusion from the fall.

While looking for stock items a warehouse employee stepped across a beam on a storage rack, slipped and went through two separate openings which caused scraping to the legs. Freezing rain had caused a slippery surface on the beam.

Employee stepped in a small ground depression and twisted his left foot. Employee continued to work favoring him left foot. This action of favoring his left foot caused employee's right knee to injury months later requiring surgery.

Mill Operator slipped on ice outside door entry which caused an ankle fracture.

Fell on icy surface twisting right knee. Employee continued to work normal activities until 2/15/2007. Employee was put on restrictive duty 2/15/2007.

Unloading fuel at mine site, tripped on a piece of steel and twisted knee of right leg. While hooking up the fuel line to tanker.

Employee was unloading some hose out of a pickup when he slipped on the tail gate. He received a cut on his right thumb and it later got infected, so he was sent off site and became an LTA.

EE was carrying some hose and tripped, landing on an old pump pedestal injuring his right knee.

Employee tripped over a wheel chock and sprained his left wrist.
