IE was unloading boxes from the tailgate of a pickup truck and carrying them into a Connex storage container. As IE stepped up into the container, EE's foot slipped and EE's right leg came down hard onto the ground causing a sprain to EE's right knee.

While exiting a water truck, the employee missed the second rung from the top of the ladder and fell approximately three feet to the ground. During the fall, the employee attempted to regain balance by grabbing the door, which caused EE's body to shift to the right. Upon impact, EE landed on EE's right elbow and reported pain.

Operator jumped off equipment from approx. 3 ft after incident. Felt pain in right knee from twisting motion when contacted ground

Plant Tech was walking a leach pad doing routine checks when they tripped on a dripper hose falling onto their knees

Employee was dismounting dozer in the wash bay. The foot on the dozer arm slipped and they fell backwards getting right leg hung up between the track frame and the dozer arm for a short time. After leg came free, employee fell to the ground. X-ray revealed a fracture at the end of the femur bone where the ligament attaches above the right knee.

Employee was walking with a short flathead shovel into the tool room at the process plant when EE tripped over a cushion mat on the floor, while trying to catch themselves the shovel handle made contact to the front teeth causing the left one to chip in half.

A haul truck operator slipped on icy ground injuring their right shoulder and arm.

An employee slipped and fell on icy ground. Employee worked employee's scheduled shifts and full duty until March 5th when employee did not report to employee's scheduled shift.

A driller walking on the pattern at night stepped into a drill hole and twisted EE's knee. After medical treatment and observation, it was determined EE needed surgery. EE missed EE's next scheduled shift on March 8th.

The operator lost his footing falling down the stairs; the operator attempted to catch himself on the hand railing above the radiator and impacted his left arm on the radiator housing.

Crusher Operator was dismounting the frame of a Tertiary Crusher when his foot slipped and his other foot hung up on the arm guard of the cone causing his knee to hyper-extend.

Employee was walking down traction type plastic on a slight grade and he felt his knee give out. It was determined to be a tear to ACL and Meniscus, worked until surgery on 12-6-2012.

Operator was walking from his haul truck to the shop. The operator slipped on the ice and fell to the same level. The operator was assessed at the emergency room with a fractured left humorous bone.

Employee was walking along a stream bank when he slipped on moist vegetation. When he attempted to catch his fall with his hand, his hand contacted a tree branch, causing a laceration.

Employee was helping to lift up a hood on a straight truck when he planted his right foot on uneven ground causing his knee to pop and cave in. An MRI revealed a torn ACL and the employee was advised on 09/05/2007 that surgery was required to repair the ACL. A date for the surgery has not yet been scheduled.

Employee was putting oil in a Cat 16G Motorgrader. when he turned to walk on tandem housing. When he placed his foot on the tire he slipped and fell to the ground. Weather conditons were snowing. Slight oil spill may have contributed to the accident. Injury consists 3 fractured ribs.

Employee was exiting the cab of a drill. He was walking on a platform when his left ankle was caught between the cab and a deck wrench lying on the deck. Employee twisted his ankle causing a sprain.

Employee was dismounting from a 100T haul truck. When he stepped down on the ground with his right foot, he felt a pop in his right knee. Shortly after, the knee began to ache and swell.

Employee was priming blastholes on a blast pattern that had received 3 to 4 inches of fresh snow the night before. After priming a blasthole, the ee stepped back to kick some drill cuttings away to bury the down line. When he stepped back he fell into a blasthole that had been re-drilled. He experienced pain in right knee but did not know it was a serious injury.

Employee was walking to a pick up when he slipped on snow packed ground spraining his ankle.

EE was checking pipe line connections on leach pad #4 when he slipped & fell on some ice. He immediately felt sharp pain in his lower back. Did not seek medical treatment until 03/15/04.

WHILE PRESSING A BEARING INTO THE BOOM ON A CAT 992 LOADER, THE TOOL BROKE. THIS RESULTED IN THE HYDRAULIC HOSES HITTING EE CAUSING LOSS OF BALANCE AND HE FELL ABOUT 9.5 FT. HE HAS A BROKEN WR IST ON THE LEFT ARM AND PULLED MUSCLES IN HIS LEFT SHOULDER. HE ALSO HAS A CUT ON THE FOREHEAD WHICH TOOK SEVERAL STITCHES.

EMPLOYEE WAS CLIMBING A LADDER WHEN IT SLIPED OUT FROM UNDER HIM. EMPLOYEE GRABBED ONTO A PIECE OF SHEET METAL WHICH CAUSED A LACERATION TO HIS INDEX FINGER. IT REQUIRED FIVE STITCHES.

EMPLOYEE WAS PULLING IN AND HOOKING UP DRIPPER PIPE ON LEACH PAD. HE STEPPED ON ICE WITH AN AIR POCKET UNDER IT AND HIS LEG BROKE THROUGH THE ICE. A PIECE OF ICE PUNCTURED HIS RIGHT LEG NEAR T HE KNEE. THREE STITCHES WERE REQUIRED TO CLOSE THE WOUND.

EE WAS CLIMBING STEPS INTO CAB OF DRILL WHEN HE LOST HIS GRIP ON THE HAND RAIL. HE FELL APPROX 7 FT TO THE GROUND. EXPERIENCED SEVERE PAIN IN LOWER BACK.

EMPLOYEE WAS DISMOUNTING FROM THE REAR OF A CATEPILLAR 16G MOTORGRADER. HIS RIGHT FOOT BECAME MOMENTARILY CAUGHT IN A BRAKE LINE CAUSING HIM TO LOSE HIS BALANCE AND FALL OFF OF THE MACHINE. EM PLOYEE LANDED ON HIS LEFT ELBOW. PAIN AND SWELLING, AND POSSIBLY FRACTURED ELBOW.

EE WAS HELPING ANOTHER CARRY A HEAVY OBJECT OUT OF A DRY POND HE STEPPED OVER A 2 IN PIPE AND STUMBLED, CAUSING THE OBJECT TO HIT HIS KNEE./

EE WAS ASSISTING A MECHANIC FIX THE BUCKET LEVELON THE LOADER. HE STEPPED FROM THE FENDER TO THEDECK WHEN HE SLIPPED OFF THE DECK AND FELL TO THE GROUND. HE FRACTURED HIS LEFT WRIST IN THE FAL L.

EMPLOYEE WAS CLIMBING OFF A RENTAL CRUSHER UNIT WHEN HE SLIPPED AND FELL APPROXIMATELY FIVE FEET CAUSING A HAIRLINE FRACTURE TO HIS ELBOW.

EE WAS CARRING A COOLER OF SAMPLE BOTTLES TO THE WAREHOUSE WHEN HE SLIPPED AND FELL, HURTING HIS RIGHT ANKLE. X RAYS PROVED NEGATIVE AND THE EE DID NOT MISS ANY WORKSHIFTS. ON 3-3-97 EE RETURN ED TO THE DR WHO PLACED AN AIR SPLINT ON THE RIGGHT ANKLE.

EE WAS WALKING AROUND HIS PERSONAL VECHICAL IN THE EE PARKING LOT TO GET HIS LUNCH PAIL WHEN HE SLIPPED AND FELL. APPROX. ONE HOUR LATER HIS NECK BEGAN TO STIFFEN UP AND BOTHER HIM. EE LATER W ENT TO A CHIROPRACTOR FOR TREATMENT.

EMPLOYEE WAS WALKING ON THE TRACK OF THE LOADER WHEN HE SLIPPED & FELL TO THE GROUND FRACTURING HIS LEFT WRIST & INJURYING HIS BACK

EE WAS ASSISTING ANOTHER EE REPLACING A TAIL PULLEY GUARD. HIS FOOT SLIPPED AND AS HE FELL HIS MIDDLE FINGER WAS CAUGHT BETWEEN THE GUARD AND TAIL PULLEY. MIDDLE FINGER ON RIGHT HAND WAS FRACT URED.

EMPLOYEE WAS STANDING ON A 15 GALLON BUCKET TO CHANGE OIL FILTER ON DEMAG SHOVEL. THE BUCKET COLLAPSED CAUSING EMPLOYEE TO FALL. EMPLOYEE'S RIGHT FINGER STRUCK AGAINST EDGE OF BUCKET.

EMPLOYEE WAS DISMOUNTING 85 TON HAUL TRUCK WHEN HE STEPPED OFF THE BOTTOM SWING STEP HE STEPPED INTO A RUT & TWISTED HIS ANKLE

EMPLOYEE WAS CLIMBING DOWN FIXED LADDER AND SLIPPED ONT HE LAST RUNG AND HIT HIS SHIN BRUISING THE MUSCLE AND CAUSING SWELLING.

EMPL WAS DISMOUNTING HAUL TRUCK. WHEN HE STEPPED OFF LADDER HE FELT PAIN IN HIP & BACK.

A SURVEYOR WAS WALKING TO A SURVEY POINT ON A SIDEHILL. HE SLIPPED AND HIS ELBOW HIT A ROCK, CAUSING A FRACTURE TO HIS LEFT ELBOW.

OPERATOR WAS ASCENDING STEPS ONTO HAULAGE EQUIPMENT SWING STEP WAS MISSING A BOLT WHICH CAUSED OPERATOR TO KEEP HIMSELF FROM FALLING AND OPERATOR STRANED LOWER BACK

CLIMBING INTO I9 DRILL, FOOT SLIPPED AND FELL ON WALKWAY OF DRILL. LACERATION ABOVE AND BELOW RIGHT EYE RESULTING IN EIGHT STITCHES ABOVE RIGHT EYE.

STEPPING FROM I3SIDE OF CONE TO THE OUTSIDE SLIPPED WHEN TRYING TO BREAK FALL & THREW BACK OUT.

EMPLOYEE WAS CH9CKING SCREENS AND SLIPPED AND FELL OFF OF LADDER.EMPLOYEE PULLED MUSCLE FROM BONE IN LOWER LEFT LEG.

PULLED TOO HARD3WHEN PULLING PLASTIC OVER DITCH CAUSING HIM TO FALL. FELL AGAINST CULVERT.

RAINY WET AND M6DDY CONDITIONS CONTRIBUTED TO THE ACCIDENT RT KNEE WAS INJURED

EMPLOYEE SLIPPED WHILE CLIMBING OVER SNOW COVERED ROCKS TO CHECK THE WATER LEVEL HE FELL APPROX FOUR FEET AND LANDED ON HIS BACK THE LOWER LEFT SIDE OF HIS BACK WAS BRUISED

EMPLOYEE STEPPED ON A PIECE OF METAL PLATE COVERED OVER WITH SNOW, SLIPPED AND FELL CUTTING THE SMALL FINGER OF HIS LEFT HAND ON A ROCK. REQUIRED 5 STITCHES.

EMPL ASSISTING MOVING PIECE OF ANGLE IRON WHEN HE SLIPPED AND FELL.LITTLE FINGE ROF LEFT HAND PINCHED BETWEEN IRON AND ROCK BREAKING HIS FINGER.
