Performing an inspection inside of a tank, coming out of the tank from the side opening, feet first and had pressure to EE's biceps pulling self through the opening, lost grip with EE's right hand, used EE's left hand and arm to catch self and immediately felt pain and burning sensation in EE's left bicep.

Mostrando los 50An employee was walking and stepped on a rock and rolled right ankle but continued working through the shift. Medical treatment was sought and an MRI was obtained on July 20th which determined that they had ligament issues in the ankle then employee was placed on restricted duty July 22nd.
Track laborer was using a track wrench on a joint bar bolt. EE slipped and twisted left ankle.
EE was walking between the A track and Runner switch at the Landing at Solvay when EE slipped on a piece of ice falling on buttocks. EE felt a pain in lower neck/mid back area and a tingling in left hand. EE was taken to the ER where EE was advised a follow up with EE's primary physician, restricted to light duty and discharged.
A roof bolt operator fell, striking head, resulting in a laceration.
An employee sustained a distal right radial splinter fracture of the left wrist. EE was outside dumping the ball mill dumpster, slipped in the mud, losing their balance, struck arm on the side of the dumpster while trying to catch self from falling. Following a full medical evaluation with the company doctor on April 4th it has been determined that surgery will be required.
Slipped opening ranger door and sustained pressure and pain in left shoulder. Employee had an MRI on 1/5/16, reviewed by company physician, inconclusive. A 2nd opinion with an orthopedic surgeon, 2nd MRI was prescribed with contrast 1/19. Jan 20, company learned that the employee had suffered a dislocated shoulder resulting in a tear of the labrum that will require surgery.
Due to a product spill by a delivery truck driver, an employee sustained a laceration to the posterior region of his scalp, requiring 14 staples to close, due to a fall. He did not recognize that flocculent had been spilled on the ground creating a slipping hazard. As he proceeded across the spilled area he slipped striking his head on the concrete.
Due to the body positioning and the location chosen for footing an employee fell forward cutting his right forearm requiring 6 stitches. When his arm landed on the sheet metal.
(See Attached)EE was walking down the pipeline cross over stairs and when he stepped of the bottom step he felt a pop in his left knee.The knee began to swell and he had pain,could not put his full weight on knee.EE had prior knee surgery.Xrays taken,recomm. a brace.EE RTW 8/28 On 9/3EE felt knee pop on 9/6 sent him home EE missed work on 9/6.9/11 EE had MRI torn ligaments.
Tripped and fell down walking from the lunch niche
A mine production employee working in 7 Bore Miner panel sustained a sprained ankle when he stepped off the auxiliary fan skid back onto the mine floor. The employee had stepped up onto the skid to reach a miner cable that was secured from the mine roof by a J-hook. The employee severely rolled his ankle thus was unable to report to his normal job duties.
While climbing on the back of the roadway to get a rock out of the chute, the employee stepped on one of the wheels on the roadway and fell. He tried to catch himself and sustained a sprained to his left shoulder and a small laceration on his left forearm. Employee worked until obtaining an MRI on 4/20/2012 showing a tear in his shoulder. He had surgery on 4/23/2012.
While carrying wooden planks down a set of stairs, an employee lost her footing and fell on the stairs. As a result, the employee felt pain in the lower portion of her back. She continued to work until 6/30. On 7/1 the employee underwent an MRI. 7/6 the employee was directed to stay off work until further tests can be carried out which are scheduled on Aug 6.
An employee was positioned on the 2nd rung of a 4ft ladder applying foam to the outside of a stopping lost her balance and fell. On 2/23 an orthopedic surgeon diagnosed her with having herniated disk and place her on restricted duty. Further medical reviews have indicated that the fall and the employees job position could have reaggravated a pre-existing back injury.
The employee was performing repairs on the longwall shear which required him to be on the top of the shear body. While climbing down off of the shear, he stepped down onto the Bretby tray. His right foot slipped causing him to lose his balance. He sustained stretched ligaments in the left ankle and will see a specialist on September 28th.
While an electician was working on a limit switch on a filter and he went to retrieve parts. An operator filled the filter with hot water to flush the filter system out. When the electrician returned he began to adjust the limit switch, hot water sprayed out of the ram, causing the employee to fall from the ladder. He sustained a laceration over his left eye requiring 3 sutures.
The employee was looking at a valve in the transporter building when the valve opened causing dust to blow out. He turned away and slipped on some product that was on the floor causing him to fall. He landed on his left wrist causing pain and swelling. He was taken to seek medical attention and found he fractured his left wrist.
Installing a pie shaped piece of 1/4" plate in the duct work on the CA-2 calciner discharge. The plate hung up and while manipulating the 1/4" plate, employee fell down the incline plane and fell into the basin, receiving a broken left arm, 3 ribs were broken and a fractured pelvis.
Employee was stepping out of a water truck. When she stepped to the ground her left foot rolled to the inside and caused injury to her ankle. Employee was new to the piece of equipemnt and not used to the step height.
As the employee was walking down the rail tracks, he reached over to release the handle on a rail car, when he slipped on the ice and fell to the ground. On 3/28/08 he was still having abdominal pain. He was taken to seek medical attention and was diagnosed with an abdominal hernia.
The employee was lifting a belt roller off of a pallet when the pallet broke and the remaining rollers fell and struck the employee. The employee fell causing bruises to his face and left leg as well as pain and swelling to his right shoulder. He was taken to seek medical attention and was put on restricted duty for one day.
Employee was walking through the high bay when he felt a pop in his right knee. He was taken to seek medical attention and was put on restricted duty until the results of the MRI are known.
While climbing down off the track mobile, the bottom step broke causing the employee to hyper extend his left knee. He was taken to seek medical attention and was put on Restricted Duty until his MRI on 2/5/07.
While the employee was fueling, he stepped on the metal cover for the bulk fuel tanks and slipped. When he fell he twisted his lower left leg. He was taken to seek medical attention and received 7 to 10 days restricted duty to allow his sprain to heal.
While bolting right curve, the employee was tramming when the plates fell off the bolter. The employee stepped off the bolter and stepped on something the caused him to fall and twist his left ankle. He was taken to seek medical attention on 9/11/06 and was put on restrictive duty until 9/19/06.
While the employee was inspecting the #1 production shaft he twisted his right knee on a build up of muck. He went to an orthopedic surgeon on 8/30/06 and was scheduled for surgery on 9/11/06.
Employee was walking when she fell forward and landed on crib block. She was taken to seek medical attention and is off work waiting x-rays results.
Employee was doing crane inspections. He was coming down the ladder and slipped on one of the rungs and lost his balance causing him to fall off the ladder to the floor. He was taken to seek medical attention and received 12 stitches in his forehead.
Employee was washing down the floor by CA- at FN-126. He lost his footing on an oil spill and fell. When he fell he lost control of the hose and was sprayed with hot condensate. He received burns to his right hand, left wrist, back, waist and abdomen. He was taken to seek medical attention and referred to the burn center in Salt Lake City.
The employee was walking into the Longwall panel when he stepped on a rock on the side of the rib. His foot went out from under him and he fell. He was taken to seek medical attention and it was found that he had a broken left leg just above the ankle.

EE WAS ON THE FORKLIFT WHEN A HIGH-PRESSURE OIL FITTING BROKE, SPRAYING OIL AT HIM. WHEN HE STEPPED OFF THE FORKLIFT ON UNLEVELED GROUND HIS LEFT ANKLE ROLLED OUT FROM UNDER HIM. HE FELT AND HEARD A POPPING IN HIS ANKLE AND IT STARTED TO SWELL UP. HE WAS TAKEN TO THE ER AND WAS RELEASED. STILL BOTHERING HIM, ON 6/24 EE RECEIVED 1 WEEK RESTRICTED DUTY.
EMPLOYEE GOT OUT OF THE CAB OF THE VERSATRAC & STARTED TO WALK TO THE FRONT TO HOOK UP A SHIELD WHEN HE TRIPPED FALLING ONTO THE DUCKBILL, INJURING HIS UPPER LEFT ARM/SHOULDER.
WHILE EE WAS PUTING UP THE RAILS FOR THE SNAKE AFTER A BELT MOVE HE STEPPED IN A HOLE AND TWISTED HIS RIGHT KNEE. ON 4/8/04 THE EE WAS STILL EXPERIENCING PAIN. HE WAS TAKEN TO SEEK MEDICAL ATTENTION, AND WAS PUT ON RESTRICTED DUTY UNTIL HE RECEIVES A MRI.
EMPLOYEE WAS WALKING BACK TO HIS LOADER AFTER MOVING A BELT WHEN HE TRIPPED ON A ROCK. WHILE TRYING TO CATCH HIMSELF ON THE LADDER OF THE 988 HE TWISTED HIS LEFT SHOULDER. EMPLOYEE WAS TAKEN T O SEEK MEDICAL ATTENTION AND RECEIVED 1 WEE RESTRICTED DUTY FOR A STRAINED SHOULDER.
ON 10-09-02 EE REPORTED ABOUT A MONTH AND A HALF AGO HIS KNEE WAS HURTING BUT IT WENT AWAY. SINCE THEN HE WENT HUNTING AND IT STARTED TO HUR AGAINS. HE WILL HAVE SURGERY AND WAS PLACCED ON LOS T TIME ON 10-14-02, HE IS ATTRIBUTING THIS TO WORK.
EE WAS USING A SCALING BAR TO MOVE A PIECE OF EQUIPMENT SLIGHTLY FORWARD. THE BAR SLIPPED AND EE FELL TWISTING HIS KNEE. HE SOUGHT MEDICAL ATTENTION AND WAS RELEASED TO SEE AND ORTHPEDIC DOCTO R. HE SEEN THE ORTHOPEDIC DOC ON 10-7-02 AND WAS PLACED OFF OF WORK PENDING FURTHER EVALUATION.
EMPLOYEE WAS DESCENDING STAIRS AND STATED THE "HE EITHER MISSED A STEP OR HIS KNEE JUST POPPED OUT", CAUSING HIM TO FALL. HE SOUGHT MEDICAL ATTENTION AND WAS PLACED OFF OF WORK PENDING FURTHER EVALUATION.
EE STATES HE TRIPPED AND FELL STRAINING HIS BACK. HE SOUGHT MEDICAL ATTENTION AND WAS PLACED ON LOST TIME ON FEBRUARY 12, 2002.
EE WAS CLIMBING A LADDER. YHE WAS ON THE SECOND RUNG WHEN HE SLIPPED AND FELL SPRAINING HIS BACK.
EMPLOYEE WAS TO COMPLETE A PRESHIFT EXAMINATION.HE SLIPPED ON A ROCK AND FELL STRIKING HIS KNEE.ON 9/27/01 HE SOUGHT MEDICAL ATTENTION AND WAS PLACED ON LOST TIME STATUS.
SOLVAY MINERALS WORKERS WERE SHOVELING ROCKS ON FLOOR CREATING A SLIPPERY WORK AREA. EE WAS MOVING A BOTTLE CART. HIS FEET SLIPPED HE FELL CATCHING HIMSELF WITH LEFT ARM AND SHOULDER, HURTING HIS SHOULDER.
EE SLIPPED ON SOME OIL AND TWISTED HIS NECK.

HOWEVER , HE CONTINUED TO HAVE PAIN IN HIS BACK. HE SOUGHT MEDICAL TREATMENT ON 5/26/00 FOR HIS BACK AND WAS PLACED ON LOST TIME STATUS EFFECTIVE 5/27/00. ANTI-SLIP HAS BEEN APPLIED TO THE ENTIRE SHO
