Emp was working with two other emp installing erosion control matting and grass seed on a dirt slope. While placing rock and staples for holding the matting the emp right foot slid into a washout causing them to fall. Their foot was wedged in the washout and remained stationary as they fell resulting in twisting to their ankle.

Employee was turning around, tripped and fell on flat ground, resulting in abrasions to left leg and fracture to right ankle
Employee exiting truck lost footing/balance letting go of ladder with right hand leaving left hand on railing briefly supporting body weight. Employee taken directly to local medical center. Injuries recieved; straining of three fingers on left hand, left wrist, and left shoulder. Employee released back to work on 5/05/2010 under limited duty, with therapy.
Employee was injured during the completion of a walk around pre-shift inspection of a CAT 966 wheel loader. At inspection of the coolant level, lost balance and fell forward, bruising left shoulder on the engine compartment. Employee taken to hospital and released back to work on modified duty on the same day. Employee has not been yet released back to full duty.
Dismounted vibrating screen improperly resulting in impaction of left ankle.
EMPLOYEE WAS LOADING BOULDERS OFF OF A LADDER AT SEMO STONE. HE WAS BETWEEN 5-6 FEET OFF THE GROUND WHEN HE LOST HIS BALANCE AND FELL. HIS LEG WENT THROUGH THE LADDER, CAUSING HIM TO STRAIN MUSCLE IN HIS LEG AND BACK.
