Employee was walking to the picnic table for lunch and twisted right ankle. X-rayed and given Ibuprofen and an ace wrap and told to keep elevated with ice. Released to regular duty.

Mostrando los 26EE was walking down a ramp to a water pump. EE stepped on the hose and slipped and fell, twisting right leg. EE underwent surgery to repair multiple fractures.
Employee was climbing up ladder and lost his grip, falling against his right shoulder. Diagnosed with right shoulder contusion.
Employee was straightening a conveyor belt frame. The chain broke causing the conveyor frame he was standing on to shift. He fell down across a piece of 6" channel iron breaking two ribs.
Crew was changing the screens on the secondary lime deck and they had a loader positioned to pull screens from the bucket. Loader bucket bled off and it dropped, causing the railing to fall onto the EE, knocking him into the screen. EE insisted he was fine and not hurt. Next day requested vac day. He worked following day, but called in 9/22/08, wanting medical attention.
Employee was climbing out of loader and his hand slipped off the ladder and he twisted his back. Employee reported slip but refused treatment until 10/02/07. Diagnosis was just low back pain and he could return to work with no restrictions.
Employee slipped and fell down a hill and cut his right knee, requiring seven stitches.
Employee was walking into the scale house, slipped on a slick step and fell. He reported to others that he was fine and proceeded to drive to town for repair parts. When he returned, an employee noticed he ran into a boulder placed in front of our scale house and went out to check on him. He complained that his left arm was unresponsive. He was taken to local ER.
EMPLOYEE WAS TIGHTENING THE BOLTS ON THE IMPACTOR WITH A WRENCH. THE WRENCH FELL, HE SPUN AROUND AND LOST HIS BALANCE AND FELL (APPROX 8 FEET) TO THE GROUND AND LANDED ON HIS FEET, FRACTURING BOTH FEET AND REQUIRING SURGERY.
EE WAS CLIMBING DOWN THE LADDER OF LOADER AND MISSED A STEP AND FELL AGAINST A CONCRETE SLAB FRACTURING THE TIP OF RIGHT ANKLE
Employee was pulling on new belt over tail pulley and slipped on the roller. He fell from the conveyor and cut his hands on the guard on the tail pulley, requiring a few stitches.
Employee was gathering water samples for environmentatl testing and slipped on loose rock and hurt his back. Did not report injury until 10/11/04. Dr diagnosed as lumbar strain and put on restricted lift/pull/push and no bending.
EE was just walking and felt a sharp pain in ankle-doesn't remember twisting or stepping on anything. she was diagnosed with a sprained ankle. she wasn't scheduled to work again until 10-4-04. she was checked by our dr and given permission to go on to work.
Employee was unloading anfo bags from the powder truck and he slipped and fell, landing across the wooden bed frame. Diagnosis was contusion of right chest.
EMPLOYEE WAS CLIMBING THE LADDER INTO THE LOADER, SLIPPED AND FELL, LANDING ON HIS LEFT LEG CAUSING STRAIN TO HIS BACK AND LEFT BUTTOCK. HE WAS EXAMINED, TREATED WITH MEDICATION AND RETURNED TO WORK WITH NO RESTRICTIONS. HE RETURNED AFTER A FEW WEEKS COMPLAINING IT WAS NOT COMPLETELY HEALED. AN MRI AND EPIDURAL STERIOD INJECTION WAS GIVEN ON 5/4 AND ANOTHER INJECTION ON 5/24/04.
EE WAS CLIMBING OFF HIS DOZER AND SLIPPED AND FELL AND LANDED WITH ALL OF HIS WEIGHT ON HIS RIGHT WRIST. A WRIST BRACE WAS APPLIED AND HE WAS ORDERED OFF WORK DUE TO WRIST STRAIN DISLOCATION.
EE WAS WALING ON SCREEN DECK HOOKING CHAINS AND SLIPPED ON THE FROST AND CUT HIS RIGHT SHIN AGAINST THE METAL EDGE OF THE SCREEN REQUIRING A FEW STITCHES.
EE WAS CLIMBING OUT OF HIS HAUL TRUCK AND MISSED THE LAST STEP, FALLING APPROXIMATELY 4 FEET TO THE CONCRETE FLOOR. NO WITNESSES. HE REFUSED MEDICAL TREATMENT BUT COMPLAINED OF SORE ELBOW. ON 2/18/02 HE VERBALLY REPORTED HURTING HIS BACK & RELATED IT BACK TO THAT FALL.
EE WAS CLIMBING UP THE LADDER INTO HIS LOADER AND MISSED THE LAST STEP AND FELL-SPRAINING HIS RIGHT ANKLE.
EE WAS CLIMBING OVER ROCKS TO CLIMB ONTO LOADER LADDER AND TRIPPED AND FELL INTO THE LOADER STEP-CUTTING HIS LEFT EYEBROW.
EE WAS CLIMBING INTO HIS DRILL. THE TOP RUNG OF THE LADDER BROKE AND HE SCRAPED HIS LOWER FRONT RIGHT LEG.
EMPLOYEE WAS CARRYING A JUG OD OIL AND SLIPPED ON THE PLATFORM OF THE SCALEHOUSE AND SCRAPED HISLOWER LEFT LEG. HE DECLINED MEDICAL TREATMENT FOR TWO DAYS AND WENT TO THE DOCTOR ON 6-6-01, TREATED FOR POSSIBLE INFECTION.
EE WAS CHECKING THE RADIATOR ON AN R-22 TRUCK AND SLIPPED ON THE CIY BUMOPER AND IT JERKED HIS RIGHT ARM AND SHOULDER, STRAINING IT.
EE WAS GETTING OFF HIS LOADER AND GOT HIS LEG H UNG UP ON THE TRACK AND HE FELL DOWN CAUSING A MILD SPRAIN TO HIS RIGHT KNEE.
EE WAS PUTTING SUCTION HOSE IN SUMP AND THE HOSE PUSHED HIS FEET OUT FROM UNDER HIM. HE FELL--DISLOCATING HIS FOURTH FINGER ON THE LEFT HAND AND MULTIPLE ABRASIONS.
EMPLOYEE WAS WORKING ON THE CRUSHING PLANT AND GOT HIS FINGER CAUGHT BETWEEN TWO PIECES OF IRONAND HE SLIPPED AND FELL CAUSING STRAIN AND SUPERFICIAL LACERATION TO HIS LEFT MIDDLE FINGER.
