Employee was spoiling cable from spoil reel. Employee slipped and fell on sled grating. They were taken to town for evaluation. Left wrist was fractured.

Employee stepped down off of the continuous miner onto uneven ground and rolled their left ankle. This resulted in an injury to the left fibula.

Employee carrying vacuum hose down belt galley tripped and fell receiving a cut on right arm. Employee was transported for evaluation and was shown to have a fracture of their rib.

Employee walking to enter the admin building. Tripped on curb stepping up on to the sidewalk. Fall resulted in fracture of upper arm.

Employee was working to reset manlift in Unit 5. While attempting, employee stepped through manhole opening resulting in laceration above the knee. Employee was taken to town and received 7 stitches.

Employee walking towards Unit 5 in area that appeared to be mud. Stepped on a spot, right foot slid into hole which was full of condensate. Removed boot, condensate burnt area around right ankle. Employee was treated by site EMR, transported to town to be evaluated. Burn was cleaned, cooled, wrapped. Returned to work on next scheduled shift.

Employee was working on a belt line. The employees foot slipped and they felt a twinge in their back. Employee finished shift and later pain worsened and they went to the ER.

Employee was taking trash to hopper and twisted knee in a pothole.

Employee walked over grating and slipped on the corner of grating and fell on arm. This caused a laceration on the arm.

Employee was up on the deca prepping for welding. Employee slipped on a pipe and fell on right hip.

Leaving work area tripped on grading, hurt right ankle.

While walking across wet ground, slipped on mud. Attempted to brace himself with arm resulting in a sprained wrist. Employee was taken to local clinic for observation and released back to full capacity work.

Employee was carrying a piece of pipe across a trench. He tripped on grating covering trench, causing him to fall, resulting in a broken arm near the wrist.

Operator was opening a discharge valve, stepped off of the pump base and on uneven ground and slipped. Felt pain in their right elbow and right ankle. They were taken to emergency room for evaluation and returned to work with restrictions.

Employee was exiting a shuttle car. The individual missed a step and their right leg went to the ground. The employee felt a twinge in their left knee but felt they were ok to finish the shift. The next shift they reported the knee was worse, so they were taken to the ER for evaluation, recommended to a specialist and given a release to return to duty with restrictions.

Employee was exiting powerhouse building and lost footing on ice. Employee fell and as they were attempting to catch themselves landed on their left wrist. Employee was taken to doctor to be examined and was referred to a specialist where it was determined they needed surgery.

Operator had hooked a steam hose up to a washout on a suction line of a pump. The operator was stepping over a pipe to access the washout and tighten the steam hose and stepped into a hole and rolled his ankle spraining it. He was taken to the doctor to be evaluated and was allowed to return to work with restrictions.

On 10-04-2013 An Employee was standing diagonal to the inclined walkway and was moving a super sucker hose to the side when he felt pain in his knee. He was taken into town to the doctor to be examined. He returned to work with no restrictions. On 10-29-2013 the knee was not getting better so the employee returned to the doctor, at this time he was given work restrictions.

Truck Driver was tarping down his load. He Pulled on a bungee cord and the bungee cord broke in half. Truck driver fell to the ground hurting his shoulder and ribs. He was transported to Sweet water Memorial Hospital for observation.

Employee stepped on a rock that was next to the rib. When rock shifted, employee lost his balance and hit his left shoulder on rib. Employee had bruising and redness to left shoulder and returned to work. On 11/8/12 employee received medical treatment for shoulder pain and received work restrictions.

Employee was pulling feeder breaker power cable when he stepped on a rock, causing him to fall. Received medical attention for injury to left knee.

Employee was working in the East Mine Fan Tunnel. He slipped and fell while carrying a large pry bar. The pry bar crushed the tip of left ring finger, requiring sutures to repair.

Employee was walking near conveyor belt and twisted ankle.

While walking, at a rapid pace, around a corner employee slipped and hit the ground landing on his shoulder. His boots had some mud on them.

While entering a bobcat skidsteer employee slipped on the step pad and hit his shin on one of the forks. He received an abrasion on his shin and later complained of knee pain on his right leg.

Employee was greasing in #1 Crusher, got foot caught in air hose and fell backward hitting his back on a rock. Employee received medical care and was not able to return to work on 12/14/09.

Employee was walking to change house at end of shift. Employee slipped on concrete walkway and fell on seat. Employee received medical care and was unable to return to work full duty.

While EE was on his way back to the I & E shop from getting a pop, he took a sip of the drink, choked on the drink, caughed and passed out just short of the mandoor at the south end of the I & E shop. The EE fell to the ground, striking his head, resulting in two small scalp lacerations and four stitches.

Employee entered area with hot crystals on the floor to assist another employee who was burned. Employee slipped and received burns to leg and arm.

EE was working to repair pressure filter (f507). As he was exiting filter, he reached up to hold the top of the door frame of the filter. EE lost his footing, slipped and caught all of his weight with right arm & shoulder causing injury to his shoulder.

Employee was dragging curtain into crosscut #2 in CM05 area. He stepped on a rock causing his ankle to give way and causing employee to fall.

Employee went to the parking lot to retrieve an item from her vehicle. Employee slipped on an ice patch and fell, landing on her right knee.

EE was watching continuous miner pick up wind row, and cable from offside shuttle car (SC39) knocked EE off his feet. EE struck his head on a rock.

Employee was walking down steps from rake (6RC01) and lost footing. While trying to regain balance, the tip of his boot caught and rolled ankle.

While cleaning up a polymer spill, employee slipped on some of the spilled material and fell. Employee's statement on results of incident was "no injury". Employee continued work. An infection developed from a pre-existing condition and resulted in lost work days. On 8/3/06 company doctor stated that the fall could have contributed to employee's infection.

Employee was loading rail cars. As he attempted to seal a lid on the car, he stepped from the walkway to the car top and rolled his ankle.

EE WAS WALKING DOWN CONVEYOR BELT AREA, STEPPED IN A HOLE & FELL.

While making adjustments to cooling tower pumps in refinery until #1 evaporator, employee stepped on some uneven concrete floor-twisted her knee.

Employee was inspecting boxcar to evaluate space for pallets and slipped as he was turning around to leave.

EE STEPPED ON FLOOR GRATING NEAR 6E01 (EVAPORATOR), SLIPPED AND HIT HIS HEAD ON THE FLOOR GRATING.

EMPLOYEE WAS PUSHING A LADDER TOWARDS P2000 TANK WHEN HE SLIPPED AND TWISTED HIS KNEE.

EE was standing on a ladder to remove a shuttle car cable, from an anchor point and fell, from the ladder as cable moved. complain of headach and sore right knee. Recent surgery and absence from work until july 13, missing work and eventually had surgery. lost work days will be reported from this date.

EE WAS WALKING DOWN THE LEFT WALKWAY BETWEEN 4C41A AND 4C41B AND HIS RIGHT FOOT SLIPPED AND HE FELL ON HIS LEFT ANKLE.

WHILE MAKING A BELT SPLICE THE EMPLOYEE STEPPED ON THE BOTTOM BELT AND THE BELT MOVED CAUSING THE EE TO TRIP AND FALL. HE FELT PAIN IN HIS LEFT ANKLE.

EE WAS WALKING NEAR THE GARBAGE COMPDATION IN UNIT 4 AND GOT HER FOOT CAUGHT IN SOME BALING WIRE AND STUMBLING ON FELT AND PAIN IN HER KNEE.

EE WAS WALKING IN THE REFINERY AND HE STEPPED ON SOME GRATING AND HIS FOOT GOT CAUGHT, TWISTING HIS RIGHT KNEE.

EE WAS RIDING ON A CUSHMAN AND THE OPERATOR TURNED TO THE RIGHT AND THE EE FELL OFF, STRIKING HIS HEAD ON THE GROUND.

EMPLOYEE WAS STEPPING DOWN OFF THE MINER WHEN HIS LEFT FOOT HUNG UP CAUSING HIM TO FALL AND TWIST HIS KNEE.

EE WAS ON A 4' PLATFORM LADDER HANGING A COME-A-LONG. THE HOOK OF THE COME-A -LONG HOOKED THE BOTTOM OF THE LADDER, CAUSING EE TO SHIFT HIS WEIGHT. WHEN THE LADDER FELL, IT CAUSED EE TO FALL.

EMPLOYEE WAS RESTACKING BAGS FROM PALLETIZER/ WRAPPER PLATFORM, WHEN HE LOST FOOTING AND FELL 36" TOT HE FLOOR.
