Miner was stepping down onto the last rung of the ladder on the CAT 988B Loader (equip#2746), when the weld of the rung broke. Miner fell approximately 2 feet to the ground on his back, followed by his head. Miner felt ok to work and 2 hours later he felt dizzy. Miner taken to the medical clinic to get checked.

While putting away 1/4" screens for 3&4 bottom decks, employee was on one end of the screen bending down and co-worker on the other. Employee, in the process of standing up, tripped on the wood that is used to keep screens off the ground. As he started to fall backwards, he caught himself before falling to the ground. While descending on a fixed ladder from conveyor 54 down to the rooftop of Air Separator 1 (AS1), EE was unaware of the last 2 rungs that were missing (26" gap), as he continued down, he had to catch himself from falling onto the rooftop of AS1 and injured his right shoulder. Employee continued to work and thought the pain would go away. On 4/3/13 EE sought medical treatment.
While walking down C31 catwalk with a empty 1-gallon paint can in his left hand, about 10-15 feet from the head box, employee slipped and broke his fall by holding onto the handrail. EE did not feel any immediate pain. Later on that night employee felt pain to his lower back.
While working on Dozer, equipment number 2550, employee was standing on the Dozer tracks replacing hydraulic hose and o-ring, when his foot slipped off the tracks and he fell about 4 to 5 feet to the ground, landing on his back.
Employee was walking on the wet concrete walkway in the feeder tunnel when he slipped and fell on to the concrete walkway. Employee stated that he fell on his left side of his body, using his left hand to cushion his fall.
Employee was seen wiping the windows on the CAT-773 haul truck and about 5 to 10 minutes later he was seen by the Stockpile Operator, laying on the ground in front of the haul truck. Employee sustained a laceration to his head and broke his right wrist.
While walking on ground level, employee slipped in muddy area and bent his right knee. Employee regained his balance and did not fall to the ground, however he heard a pop noise from his right knee.
Our employee broke his upper shin bone below his knee on his right leg while repairing a 988B Cat Loader for Ameron. The 4 ft. platform he used to gain access to an area to replace a pin collapsed. He is waiting to have surgery.
While working in the tertiary plant, employee stepped onto screen #5 to retrieve a piece of wood when his left leg slipped and twisted causing pain to his left knee.
After punching out for the day, employee was descending down from the stairway when he lost his footing from the middle landing of the steps and tumbled down the remaining 9 steps.
While refueling the D8 Dozer at the fuel station, employee was descending from the equipment with his right hand on the dozer's railing. As he reached out to grab the fuel nozzle with his left hand, his left foot slipped causing a jerking motion unexpectedly to his right arm. Thereafter he started to feel mild pain. While cleaning out the bypass chute behind crusher 2, employee stepped on section of the catwalk that appeared to be rusted and fell partly through hitting his right thigh on the electrical conduits and the tail pulley guard. As a result employee sustained abrasions on forearm, contustion of the thigh and laceration to left palm.
While getting out of the truck to buy lunch, employee slipped and fell to the ground landing on his back.

WHEN CLIMBING DOWN EQUIPMENT #773, HE STEPPED ON UNEVEN GROUND, STRAINING RIGHT KNEE.
EE WAS CLEANING THE RETURN ROLLERS FROM C20 CONVEYOR AND HE SLIPPED. WHILE HE WAS FALLING, HE GRABBED THE SAFETY GUARD AND THE WIRE FROM THE SAFETY GUARD PUNCTURED THRU HIS GLOVE AND BETWEEN H IS TWO FINGERS. LACERATION ON RIGHT HAND LONG FINGER SUTURED IN EMERGENCY ROOM.
