EE was washing Tower #2 back pad, EE shut the fire hose off went to change locations & started to slip, falling backwards, but caught self with left hand on a pillar causing a strain to right neck & shoulder. Injury Type: Neck Strain. EE didn't request med. treatment at the time. Was seen by a medical provider on 12/26/23 when injury became reportable due to restrictions.

EE was pulling a 1 1/2 inch fire hose to clean off piece of equipment, as EE was walking backwards, tripped in rut and fell to ground.

On 04/04/2022 EE climbed down the ladder on the loader that EE was operating to close the gates to the haul road. As EE stepped down on the lower rung of the ladder, it broke away from this support. EE grabbed the ladder as EE fell straining left arm bicep when EE caught self from falling. PT wasn't ordered until 06/13/2022.

Employee was climbing ladder onto haul truck, hands slipped, twisted left side back.

EE dismounting loader ladder, boot tread got caught in the bottom rung of step, fell to the ground, striking left knee on concrete floor. EE fell on 3/4/20, reported to clinic on 3/5/20. On 3/16/20 EE given physical therapy.

Employee was exiting loader with ladder, right foot already on group, when left foot became caught on bottom step, causing employee to lose balance and fall to ground. Employee tried to catch self and fracturing left wrist.

EE was walking to haul truck when EE slipped and fell on packed snow straining Rt. knee.

Employee was doing a pre-shift inspection on Loader unit #15-415. He was inspecting the z bar links, pins and bushings on the left front of the machine. He leaned over to inspect the machine, slipped on snow that had fallen the night before. He fell and his left side (upper chest) fracturing three ribs when he contacted the lift arms on the loader.

Employee was washing the HP 400 crusher decks off with a fire hose before scheduled crusher maintenance. He tripped on a small rock and fell to the crusher deck dislocating his left shoulder.

Employee fell and strained his right knee while running to the break room to avoid getting wet when it began to rain.

While hoisting a 3'x20.5' sheet of corrugated metal siding, the sheet was caught by a gust of wind causing the clamps to fail. The 20#sheet fell to the ground from a height of approx. 10' striking injury. in his attempt to get out of the way he fell to the ground. He suffered a bruise to his abdomen. Slight damage to the sheeting/siding.

EE had finished washing out material from under the conveyor and was in the process of stowing the fire hose when he slipped backwards extended his L hand to caught himself and dislocating his left pinkie finger or pip joint. Note:EE was wearing his hard hat,safety glasses,high visibility garment,gloves & steel toed boots.

EE was walking backwards while carrying screen on screening tower platform. He tripped on water line and landed on his right side on top of support beam. Employee had been working in same area and had made numerous trips.

Employee slipped and fell and dislocated his right pinkie finger.

EE WAS CLIMBING THE LADDER TAKING A WATER HOSE TO THE TOP OF THE TWINSCREW TO START TO WASH DOWN THE LADDER SHIFTED POSITIONS HOSE PULLED HIM BACK CAUSING A FALL.

EE WAS WALKING ACROSS OFFICE, TRIPPED ON LEG OF CHAIR, FALLING FORWARD ON FACE BREAKING HER NOSE AND CHIPPING LEFT ELBOW.

EXTREME ICE CONDITIONS - EE WAS GOING FROM THE SHOP TO THE PLANT - SLIPPED ON ICE FELL BACKWARDS - LOWER BACK WENT OUT OF PLACE.

SLIPPED/LOST FOOTING CLIMBING LADDER GETTING INTO FRONT END LOADER

EMPLOYEE WAS CHANGING SCREEN SLIPPED OFF SIDE OF SCREEN AND FELL AGAINST HAND RAIL ON LOWER PART OF RIGHT SIDE UNDER RIBS.

HELPING OTHER EMP WELD SCREENS LSIPPED PUT HAND ON HOT SCREEN TO CATCH SELF

THE EMPLOYEE WAS CLEANING AROUND CONVEYOR- SLIPPED WITH SHOVEL AND HIT SIDE OF CONVEYOR.

EMPL GETTING UP FROM DESK,GOT FEET TANGLED IN TELEPHONE CORD AND FELL FORWARD ON BOTH KNEES.

EMPLOYEE WAS MOVING CASING FROM A SHOT PATTERN-SLIPPED AND HIT LEFT KNEE ON ROCK.

EMPLOYEE WAS CHANGING SCREENS STEPPED OFF SIDE OF SCREEN AND TWISTED LEFT ANKLE.

EMPLOYEE WAS WALKING DOWN TO THE JAW CONV TO SPOT TAILDRUM TO REMOVE ROCK FROM TAILDRUM.

EE WAS MOVING 2OSE FOR DETROIT DIESEL DRIVEN MOTOR PUMP. WATER SHOT OUT CAUSING WET SLIPPERY CONDITIONS.
