The conditions contributing to his death are still under investigation, there were no direct witnesses to the death of our employee. We know that employee was picking up a bulk load of lime at Graymont. We know that a Graymont employee found him on top of the trailer in a safety harness unresponsive. CPR & AED were administered on site. EMS transported employee to the Hospital.
Bottom stair tread broke while employee was entering crusher pit. The next day ankle was sore and got X-Ray that showed nothing broken only a sprain.
Employee was exiting the #1 tube mill and lost his footing causing him to fall off the scaffold platform. When employee fell he broke his left middle finger.
WALKING DOWN STAIRWAY WITH CONCRETE RAKE AND SHOVEL. TRIPPED OR MISSTEPPED. WAS USING HAND RAIL BUT WHEN CATCHING BALANCE CONCRETE RAKE WEDGED BETWEEN HIS RIGHT KNEE AND STAIRWAY.
GOING DOWN STAIRS TO QUARRY PUMP. BASEMENT FLOOR DAMP--FELL BACK ON HANDS HURTING BACK.
