Ee was coming down off a scissor deck, put EE's right foot on the ground, went to put other foot down and EE's right foot rolled off the tire trench EE was standing on. EE felt immediate pain, was taken to the clinic, diagnosed with an ankle sprain and released to modified duty, non-weight bearing and required to wear a medical boot.

Mostrando los 45Ee in wash bay washing truck. When ee went to get down off bed of truck, foot tangled in hose, and ee tripped, falling to ground. Seen at ER, laceration to left hand, contusion to left knee, and right thumb sprain. Released to restricted duty.
Ee stepped off a scissor deck, boot caught, and twisted ankle. Ee was seen at the ER on 08/31 and then by a specialist on 09/01, where they were given a boot, crutches, and released to sedentary duty. Follow up on 09/07 continued the sedentary duty.
Stepped outside to put the mop over the railing, when stepping back around, overstepped the landing and fell backwards hitting back and head on the step and ground. L-2 fracture in back and cut on back of head requiring 5 staples.
The employee was lowering the end gate of a pipe truck, he dropped the retaining nut on the ground, when he stepped off the day-bed to get the nut he let go of the end gate. the end gate hit him in the shoulder and knocked him down when he contacted the day-bed with his chin. resulting in a laceration to the chin requiring 8 stitches to close.
Helper was climbing into a pipe truck. He put his right foot on the first step, grabbed the handles to help pull himself up. When he lifted himself off the ground he put weight on his right foot. His right foot wasn't straight with the rest of his body which caused his knee to twist as he put weight on it.
Employee strained left knee while cleaning borehole. Employee got off balance, straightened out his left leg and felt a pop behind knee. Employee was placed on restricted duty.
Helper was adding a 5' quill rod on a -67 degree. He had the rod lifted up to add it to the string and the rod slipped and was going to fall. He lost his grip and while trying to keep the rod from falling he stumbled around and strained his back.
Employee was working on UG-34 and was walking on the landing of the deck platform With his foot planted flat on the deck, the employee stepped with his right foot and his ankle rolled inside his boot. The employee went to the ER for x-rays. The x-rays were negative and the employee was given a prescription medication.
The driller and helper had just got to the drill station and was getting ready to start unloading the drill mud off the tractor when the helper had stepped off the tractor and twisted his ankle on a rock that was under some drill mud and cuttings.
Employee was walking down 3 sets of stairs while holding a core tube, went to put tube in rack and slipped on mud. While trying to prevent the tube from falling he felt a pull in his lower back.
While moving a DynaPac roller on the job site the operator lost control of the roller. The opertor removed the seat belt and jumped from the roller. The operator fractured her right leg when she hit the ground.
While resetting a cattle guard into place the employee slipped and fell between the rails on the guard. The cattle guard could have been slick due to a small hydraulic leak on the back hoe.

Miner was walking down shaft ramp to the 1715 shop. While walking on the ramp, he came to a slick spot. Left foot slipped forward and over-extended left knee.
MINER PULLED JACKLEG OFF RIB, SLIPPED ON HOSES AND FELL DOWN. THE LEG OF THE JACKLEG FELL ACROSS THE LOWER PART OF MINERS LEFT LEG
MINER WAS REMOVING SPLIT SET OFF BOLTER. MINER SLIPPED AND STARTED TO FALL. HE CAUGHT HIS WEIGHT BY GRABBING RIB WITH RIGHT ARM. MINER FELT PAIN IN HIS RIGHT SHOULDER AND RIGHT ELBOW.
MINER WAS GETTING OUT OF A LHD AND TWISTED LEFT ANKLE.
Miner slipped on snow walking to # 2 shaft, fell down spraining right wrist.
This miner was getting off a tractor when his left ankle rolled.
WHILE CHANGING A FLUSHING HEAD ON 3804 BOLTER, EE SLIPPED IN THE MUD AND LOST HIS FOOTING. HE GRABBED THE BOLTER WITH HIS RIGHT HAND AND TWISTED TO THE LEFT TO KEEP PART FROM HITTING HIS LEFT LEG, AND HURT HIS LOWER BACK.
EMPLOYEE WAS DRIVING UNIT 2728 LUBE TRACTOR UP THE MAD DECLINE WHEN A HOSE BLEW OFF THE GREASE PUMP. AFTER STOPPING THE TRACTOR HE BEGAN TO DISMOUNT FROM THE RIGHT HAND SIDE. AS HE STEPPED DOWN WITH HIS LEFT FOOT, HE TWISTED HIS LEFT ANKLE.
EE WAS WORKING ON A SCREEN. HE NEEDED ANOTHER TOOL, SO HE UNTIED HIS LANYARD FROM HIS SAFETY HARNESS TO DESCEND TO THE GROUND. HE SLIPPED ON SOME ICE AND SNOW, AND FELL APPROX. 4 1/2 FEET, LANDING ON HIS BACK.
THE EE WAS WALKING TO THE SHAFT FROM THE OFFICE, WHEN HE SLIPPED ON SNOW & ICE THAT WAS ON THE ROAD. THE EE LANDED ON HIS BACK WHEN HE FELL. THE EE DID NOT SEEK MEDICAL ATTN. UNTIL MARCH 22,2004 & AGAIN ON APRIL 6, 2004.
EE WAS HELPING A MINER LIFT UP A PLUGGER AFTER THE MINER HAD FINISHED DRILLING A 4 FOOT HOLE. THE EE HAD HIS RIGHT FOOT ON A 2X4 SUPPORT THAT WAS LAYING ON THE 2 INCH SIDE. WHILE PULLING THE PLUGGER OUT OF THE HOLE THE EE'S FOOT SLIPPED OFF THE 2X4. THE EE STATED THAT HE FELT A POP IN HIS RIGHT KNEE.
INJURED EE SLIPPED WHILE DISMOUNTING UG HAUL TRUCK, STRIKING LEFT SIDE ON AIR CLEANER GUARD.
EE WAS WATCHING LOW PRESSURE END OF PUMP DISCHARGE WHEN PLUG IN 4" LINE DISCHARGED, CAUSING 4" RUBBER HOSE TO STRIKE EE IN CHEST, KNOCKING HIM DOWN. HIT BACK OF HEAD ON ROCKS.
EE WAS WASHING OUT BATCH PLANT AREA, STEPPED ON 1" RUBBER HOSE, TWISTING RT. ANKLE.
EMPLOYEE SLIPPED WHILE WALKING ON STAIRS, FELL ON HIS BACK.
EMPLOYEE CLIMBED OFF THE JUMBO CAB TO CHECK THE FACE, IN DOING SO EMPLOYEE SLIPPED ON A ROCK, FELL AND STRUCK HIS RIGHT WRIST CAUSING A LACERATION.
EMPLOYEE HAD FINISHED DUMPING SOME REJECT CRUCIBLES IN A CONTAINER SITUATED INSIDE O CONTAINMENT AREA.AS SHE WAS PREPARING TO WALK AWAY SHE TRIPPED AND FELL OVER THE BEAM INTO THE CONTAINMENT AREA. INJURING HER RIGHT SHOULDER.
EE WAS BOLTING ON SISSOR DECK HIS LEG SLIPPED ON DECK SAID HIS LEG HURT A LITTLE BUT WORKING. EE LEFT WORK WITH THE FLU CALLED SICK WITH FLU FOR 2 DAY. CALLED IN 4/3/00 MON SAID HIS LEG HURT G OING TO DR NO REPORT TO SUPERVISOR OF INJURY.
