Employee was assisting with core tube retrieval activities at the drill when an issue occurred. During the process of resolving the issue, the employee lost balance and contacted the drill feed frame with EE's hand, resulting in a hand injury.

Mostrando 50 de 90EE picked up a pump to place it back on a timber car. EE's foot was against the rail and when EE turned EE stumbled and felt a sharp stabbing pain in EE's back. EE did not want to see a DR. After EE's days off, EE stated EE's back wasn't getting better. EE was taken to the doctor on 09/23/2025 and was placed on lite duty by the physician.
While bolting the back, loose material fell causing the miner to move backwards. This caused miner's machine to fall toward miner, knocking backward. As miner fell backward, left hand came into contact with the post of the bolt deck which resulted in a hematoma on the hand. The miner was placed on restricted duty by the provider.
A labor support tripped while crossing a grizzly cover and fell causing a dislocation of the shoulder. The injured person was placed on light duty by the provider
While descending the stairs out of the main office after shift, the injured miner missed a step and fell down the stairs, twisting their knee. The injured miner declined going to the doctor. On December 26th the miner stated it wasn't getting better. We took EE to the doctor and the miner was placed on restricted light duty after being diagnosed with a sprain in the knee.
While cleaning up muck from the track level, the motorman stepped over the hitch of a train car to get to the other side of the train when they slipped, twisting their ankle. This resulted in a sprained ankle
Miner was preparing to bolt when the air line on the jack leg failed. While trying to shut the air off to the hose, the miner slid against a mat on the rib causing a laceration to left forearm. The miner was taken to the hospital where the miner received stitches.
Employee was walking down ramp when EE tripped and caught self with left hand. This resulted in a broken wrist. Initial x-ray was negative. Did not receive confirmation of broken bone until 04/03/2024.

An employee lifted a piece of 40-lb rail with another miner. As they were walking the rail to where it was going to be installed the employee tripped on a 1" hose that was laying in the walkway. The employee fell and the rail landed on his right shin causing a severe hematoma.
Employee was cleaning the ground floor of the rock house with a 1" water hose. He was walking forward near the base of the E-45 conveyer foundation while pulling the 1" hose. The employee's left foot caught the foundation and he tripped and fell on to his left side and on to the concrete floor. He was walking east towards the rock house room.
EE was examining the cleaning cells at the mill which sit on a slightly elevated floor. The floor is constructed of metal grates and there is a 6" step up to the elevated floor. When she left the area she misjudged the step as she was leaving the elevated floor. This caused her to fall on her knees resulting in a L. ankle sprain, and a sprain and a laceration to R. knee.
He had just finished moving the electricians utility truck from it's covered parking area so that he could have access to move a fan out of the shop with the forklift. He parked the pickup in an uncovered area. When he stepped out of the vehicle his foot slipped on the icy surface. His partner pulled the steel out of the chuck and as he stood the machine up he lost his footing and fell backward pinching his hand between the machine and the corner of a rib mat. This caused a laceration to the web between his fore-finger and thumb which required sutures.
The employee was cutting timber for a raise crew. As he lifted a scrap piece of square timber 12"x12"x5' long he got off balance and when he caught himself he felt pain in the lower abdomen mid-line down to the groin.
EE was cleaning threads on a cone crusher while other crew members were extracting the mantle. A worker yelled at EE to free the chain on the chain hoist and he stepped into a 1' diameter circular hole that had not been covered after the extraction. This injury was reported as a minor injury and because of persistent symptoms we referred him to a Dr. on 8/17/12.
The employee had placed a jack leg drill on the muck pile which was on an inclined plane. He then stepped back to retrieve some rock bolts and when he turned back around he slipped and his chin struck the crows foot on the leg which because of the slope was a little over waste high.
The employee was installing a header and tried to gain elevation by standing on a wood block appx. 10 x 10 x 18 inches. The Block rolled which caused him to fall. He put his arms out to break his fall which caused a hairline fracture to the radial head when his arm contacted the concrete floor.
