On 5/9/2014 employee reported that she twisted her knee on 5/5/2014. She did not report this event to personnel when it occurred as required by company policy. We have investigated this accident and do not believe this event constitutes a workplace injury. Brahma only files this 7000-1 out of an abundance of caution in order to preclude MSHA enforcement.
A grating covers a concrete tank. When employee walked across the grating one section (approx. 3'x5') slid and fell into the tank. He also fell in the tank from 17' on to the concrete below. He fell on to his right side and fractured 7 ribs, his elbow and shoulder.
Leaving his shift for the day, employee stepped down form the steps onto solid ground and felt his knee give or twist. He went home thinking he had strained the knee. His knee began to hurt and swell. He came in the next day and reported to his supervisor and was referred to our health care provider. No rocks or other material was found on the solid surface
Employee was starting to climb a vertical ladder when he slipped on first step and fell 2' down and landed on his right knee on top of strong back causing a laceration on his knee
Employee standing on drill table slipped and fell backwards hitting his head.
Slip & fall. Initial incident - from snow packed over. Employee fell on right elbow straining elbow. Employee continued to work. Elbow became more painful from the repetitive motions of the work causing more strain to elbow.
EE was walking between work locations, slipped on ice and strained his left knee
EE was performing a pre-walk around on his Haul Truck, he slipped on the ice, his legs went separate ways and he broke his right leg in two places.
Mechanic was climbing down ladder, missed a step and fell 2-3 feet on his foot and broke his ankle

As EE completed his duties he proceeded to exit the cat walk by descending the stationary ladder. As he was descending he experienced an episode of dizziness & lite headiness causing him to lose his grip & falling to the ground. EE fell 6'
Exiting haul truck (740) employee stepped on the lower rung & lost balance causing him to fall backward off of the step. Falling back onto a 3' - 4' rock. Employee suffered a blow to his lower back.
Walking up slope with spooler slipped on surface covered w/dust. Lost balance tried to catch himself with spooler, all of his weight was on spooler/outstretched right arm causing dislocation of shoulder.
Wet / muddy / slippery area. Employee was navigating on access ramp to pump house when he slipped and fell hitting his right shoulder on the ground.

Operator was walking out to get a sample from the barren header when he slipped on ice. While trying to catch himself his right leg locked straight, causing strain to his knee.

Employee was moving the Odex hammer when he stumbled and fell with the hammer landing on his right arm. Diagnosed with right ulnar nerve contusion. Prescribed medication and ice for injuries. Placed on light duty.
Employee was filling water truck from a water fill station. As he was walking down the catwalk he tripped over a bent section of the catwalk which could not be seen because of inadequate illumination. He fell against the hand rail. Employee finished the shift but went to the doctor the following day (4/15/07) Diagnosed with a contusion to right side of chest wall.
A driller was rigging a winch line when he stepped in a hole and strained his ankle. Physical Therapy was prescribed on Feb 16, 2007
A leach pad operator had dismounted a backhoe when she fell on uneven ground and strained her ankle. Physical therapy was prescribed and administered.

EE WAS DESCENDING THE WATER TANK ON A DRILL WHEN HE SLIPPED AND FELL APPROXIMATELY 9 FEET TO THE GROUND. HE WAS EXAMINED THE DAY OF INJURY AT E.R. AND RELEASED TO HOME. HE WAS EXAMINED BY THE AUTHORIZED PROVIDER THE NEXT DAY AND RELEASED TOFULL DUTY. NO MEDICAL TREATMENT WAS ADMINISTERED DURING EITHER EXAM. A MILD CONCUSSION CAUSED HIM TO MISS TIME FROM WORK STARTING 11/23/02.
EE WAS WORKING THREE RUNGS UP ON AND 8 FT.LADDERWHEN THE LADDER TIPPED OVER. THE EE FELL ON TOP OF LADDER, BRUISING HIS RIGHT SHOULDER AND RIBS.PHYSICAL THERAPY WAS PRESCRIBED AND IS BEING PERFORMED. EE HAS BEEN RELEASED TO WORK WITH MINOR RESTRICTIONS THAT FALL WELL WITHIN THE SCOPE OF HIS JOB.
EE WAS INSTALLING DECK GRATING AND BOLTING HANDRAIL. EE WAS WEARING HIS FALL PROTECTION EQUIPMENT BUT HAD TAKEN IT UPON HIMSELF TO DISCONNECT WHILE INSTALLING BOLTS. GRATING WAS NOT YET SECURE D AND SLID OUT FROM UNDER HIM CAUSING HIM TO FALL TO NEXT LEVEL. FALL BROKE BOTH WRIST AND CAUSED SMALL LACERATION TO RT CALF. NO OTHER INJURIES.
WHILE CARRYING MATERIAL DOWN SLOPE EE LOST FOOTING & TWISTED RIGHT KNEE
EE WAS CLIMBING DOWN THE LADDER FROM THE LOADER CAB WHEN HE SLIPPED & FELL ON THE LAST 4 STEPS. HE CAUGHT HIMSELF ON THE LADDER'S HANDRAIL BRUISING HIS LEFT SHOULDER & ELBOW. HE WAS EXAMINED B Y THE COMPANY'S AUTHORIZED PROVIDER. X-RAYS WERE NEGATIVE. MEDS & PHYSICAL THERAPY PRESCRIBED. HE WAS IN A HURRY TO USE THE RESTROOM & WAS RUNNING DOWN THE LADDER NOT USING 3 POINTS OF CONTAC
EE WAS CARRYING A PIECE OF ANGLE IRON WHEN HE TRIPPED AND FELL. HE BENT HIS THUMB CAUSING A STRAIN TO THE TENDON ON HIS RIGHT THUMB.
