He rec'd surgery consult on right shoulder & assigned no work status 3-15 while on paid investigatory leave for a separate matter
Speed/Haste - Employee failed to maintain three points of contact as instructed. Employee failed to recognize foot was caught in step when dismounting equipment. Employee suffered bruise to torso and strain/sprain in his shoulder. Employee has made full recovery and returned to work.
While the employee was conducting construction quality assurance for a project at the mine, he walked forward a few feet down the slope and stepped onto articulated concrete block mat (which had some gravel on the surface) and he slipped falling backward and landing on his left shoulder, causing a contusion and ruptured rotator cuff.

He opened the spring latch door to the man-lift and used his hip to hold the spring gate open as he backed into the basket. Below a welder was preparing to board the shovel. HE WAS INITIALLY TREATED CONSERVATIVELY. HE WAS REFERRED TO A SPECIALIST. HE HADSURGERY ON 8/1/01.
EE WAS WORKING IN THE MINE DEPARTMENT AT THE FUEL ISLAND WHEN SHE SLIPPED AND FELL CAUSING HER LEFT ANKLE TO TWIST. SHE SLIPPED WHEN SHE STEPPED OFF THE LAST RUNG OF THE FLEX LADDER LOCATED IN THE FRONT OF THE HAUL TRUCK SHE WAS OPERATING.
