EE was walking on a concrete paved area, when the EE slipped on a small, thin patch of wet raw mix. His left foot came to a stop on dry concrete causing his left knee to overextend. EE reported as a precaution. EE reported discomfort recurred and worsened after a couple of months. EE went to the doctor for evaluation, and returned to work the same day on modified duty.

Mostrando los 24Employee slipped and fell into a sump hole injuring shoulder and back. Company was notified this was a medical injury on 12/19/2012.
Employee had sampled a waste fuel storage tank. He purged material into a bucket, and as he was walking, tripped on a pipe and splashed material that was in bucket into his right eye. Employee was sent to local doctors office to have eye washed out. Doctor administered tetanus shot and prescribed ointment for eye.
QC Tech was traveling on FM2 grated platform to obtain a grinding aid sample. Uneven grating caused him to fall. He landed with his weight on his right arm and right knee. Currently, his knee is just scraped, but his elbow and forearm are in alot of pain
Employee was walking east of Punch Clock on a ground-level concrete walkway that has a handrail. It was raining heavily at the time. Employee's left leg slipped. Right arm caught handrail & temporarily stopped fall. Left leg continued to move & struck bottom portion of handrail at the knee.
Working under structure, slipped and placed all weight on right knee. Caused knee pain.
Employee was pulling plastic standing on a metal bar to get better leverage for removing plastic. While pulling plastic out of the pan, his right foot slipped off the bar. While stepping down with left foot, contacted ground, rolling ankle.
3rd party driver for customer delivering fuel quality waste to Systech was walking back to his truck along a sloped gravel area. He lost his footing and apparently broke his ankle. Driver has over 5 years driving experince. Currently (Coal City Cobb 972-923-7508) for 3 years 11 months. As he is not a Systech employee, will not be able to confirm return to work status.
EE was on top of the Finish Silo's for a ground level fall injured while measuring E Pocket canister. He slipped from a cement line that he was standing on to measure the pocket. Twisted right ankle.
Employee was inspecting work in kiln. As he walked he stepped onto bare kiln shell and slipped, landing on his shoulder.
Employee was crossing a work area that was being cleaned and slipped on wet floor that resulted in a fall causing a sprain to his back, hip and elbow.
Employee was going restart a compressor. He was in a hurry and lost his balance. He stumbled forward and tried to run forward to catch himself. When he reached the point to where there is a 4" step down in the floor, he lost control even more. He ran into a switchgear cabinet causing a laceration to his right elbow that required stiches.
Employee was walking along the railroad tracks checking rail cars when he stepped into a hole and twisted his right knee. He seen a doctor, but was released without any treatment. In February, it was determined that he needed surgery. The surgery was on 2/17/06, and he returned to work on his next regular scheduled day of 2/20/06.
After taking slurry sample, injured was exiting the slurry tank pit w/sample in left hand and right hand used for handrail. When he reached the top set of stairs, he fell forward striking his left knee on the bottom edge of the doorway cutting his knee open. He received stitches for his wound. Addendum: Leg that wasn't cut swelled up & EE admitted to hospital.
INJURED & OTHER EMPLOYEE WERE ON BREAK IN THE GREEN AREA NEXT TO THE PAVILLION. THEY BEGAN WRESTLING ON THE GRASS. THEY BOTH FELL TO THE GROUND & INJURED'S LEFT LEG WAS BROKEN IN THE PROCESS.
EE WAS PULLING A DRUM ALONG THE CONVEYOR AWAY FROM THE DUMPER. HIS FEET SLIPPED OUT UNDER HIM.AS HE FELL HE PUT HIS ARMS BACK TO CATCH HIMSELFAND BROKE HIS LEFT WRIST. THE FLOOR MAY HAVE BEE N DUSTY FROM DRY CORNCOB THAT HAD BEEN USED TO CLEAN UP A SMALL SPILL.
SLIPPERY SURFACE WHEN MOPPING AND CLEANING AGENT FUMES. EE BRUISED HIP AND BUMP TO HEAD.
EE WAS REMOVING DOW SCREW FROM KILN FLOOR. WALED AROUND SCREW TO REHOOK AND SCREW MOVED TO THE SOUTH AND KNOCKED HIM OFF KILN 2 FLOOR.
CONTRACTOR EMPLOYEE WAS CLIMBING AN ACCESS LADDER ON A RAIL TANK CAR. THE EMPLOYEE WAS CARRYING A ROPE IN ONE HAND AND MISSED THE RUNG WITH THE OTHER. HE FELL APPROX 9 FT AND STRUCK BACK ON BO LT IN FENCE RAIL. HE SCRAPED HIS BACK. PHYSICIANAPPLIED STITCHES AND PRESCRIBED PAIN KILLERS.
EE WAS STEPPING OFF FORK TRUCK TO OPER A DOOR. EE TWISTED ANKLE LEFT.
EE WAS PULLING A TORCH CART, SLIPPED AND FELL BACKWARDS, CART FELL ON LEFT LEG, TWISTING KNEE AND HIP. EE DID NOT REPORT UNTIL 11-17-00, WENT TO DR AND WAS PLACED ON LIGHT DUTY, SPRAIN TO KNEE .
EMPLOYEE WAS PATCHING HOLE IN CHUTE TO UPPER RECLAIM BELT. STEPPED ON A CLINKER AND ANKLE ROLLED. SPRAIN TO RIGHT ANKLE.
EE WAS WALKING TO CHECK OUT A MOTOR-SLIPPED ON A ROCK ON RAMP AT THE NORTH DOOR OF RAW MILL. SPRAIN TO LOWER BACK. WENT TO CHIROPRACTOR.
EE WAS WALKING OUT OF THE MILL BUILDING AND SLIPPED ON THE ICE. POOR WEATHER CONDITIONS. EE DID NOT GO TO THE DR UNTIL 2-25-00. WENT TO CHIROPRACTOR/DIAGNOSIS GRADE 1 SPRAIN TO RIGHT SHOULDER.
