Employee was collecting a tool by the Ore Apron Feeder. As employee was going down a section of stairs, employee slipped and landed awkwardly rolling employees' ankle. Employee visited doctor and was diagnosed with a severe ankle roll. Walking boot was prescribed. Employee returned to work with no lost time.

Mostrando los 20When the ammonia line was charged, during the plant start-up process, the compression fitting holding the flow meter in the pipe failed releasing ammonia into the building. As the operator was evacuating the building the operator dropped from a mezzanine deck to the floor below. This drop caused lower-back pain.
Individual descended a flight of stairs, stepping on a hose at the bottom and lost footing. EE landed on back side. On 9/14/16 the individual reported discomfort in buttock. On 9/28/16 ee was examined by PCP and prescribed an order for an exam and treatment by a Physical Therapist. EE attended 1st PT session the following week.
Employee stepped on stainless steel lid covering an inspection port. The lid slipped off the opening, causing the right leg to fall into the opening resulting in a laceration above the knee on the back of the thigh, and a contusion on the upper shin. Treatment required stitches.
Employee was descending from D9 Dozer when foot slipped on icy track. The employee's leg hit a bar and sustained a laceration to the inside right shin area (L=3cm; W=.5cm; required 5 stitches).Cold slick conditions caused a slipping hazard.
EE was descending the ladder, one foot already on the ground and the other on the lowest rung, had just unhooked his fall protection harness and lost his footing and slipped.
The employee was on the lower cat walk at the thickener area to repair an auto valve. When stepping across the pipes to the valve, his foot slipped between the pipes causing his right knee to hyperextend, spraining the knee. Incident went unreported until 12/23/2011
Upon completion of a pipe replacement the EE slid from the pipe rack to the cat walk. Once standing he twisted to move forward and felt a pop in the knee.
Employee was making his usual rounds, began to climb down the staircase into Thickner Tunnel #8 and fell to the cement below. Fractured L1, L2, L3, Sacrum and skull.
Employee entered the lunchroom to get a paper cup, slipped sideways on the wet floor (janitors were mopping at the time), fell forward, hitting her right shoulder on the Pepsi machine, right orbit (area surrounding eyeball) against the soda dispenser(metal). 2 Stitches required by physician to under eye area.
Coming down the stairs in between A & B hydrolyzer, toe caught on cement landing, twisting right knee.
Subject & co-worker working with vent pipe removal. Vent pipe improperly secured. Subject was struck by moving pipe which then caused him to fall to the floor. (Same level fall)
ICY SURFACE & SNOWING OUTSIDE. EE WAS IN PROCESSOF WALKING TOWARDS THE BACKHOE TO FUEL IT UP. HESLIPPED ON ICE, TRIED TO CATCH HIMSELF, & SLIPPED AGAIN TWISTING HIS RIGHT KNEE. HE SAW DR 12/18 /01 WHO GAVE EE RELEASE TO WORK. KNEE CONTD TO HURT & EE WAS REF'D TO ANOTHER DR. 1/22/02. HE RET'D TO WORK WITH RESTRICTIONS: REF'D TO ANOTHER DR FOR 2ND OPINION 3/14/02 HAD SCOPE DONE &RTW
EE WAS GOING DOWN THE METAL LADDER WHICH IS BOLTED TO THE WALL IN THICKNER TANK#8 & ABOUT 2/3 OF THE WAY DOWN THE LADDER HE SLIPPED & LANDED ON THE GROUND WITH RIGHT HAND UNDER HIM. THIS RES ULTED IN A FRACTURE OF THE NAVICULAR BONE OF THERIGHT HAND
EMPLOYEE WAS CLEANING THE IMPACT CRUSHER ON A WET MUDDY DAY. THE MACHINE WAS OFF AND HE NEEDED TO SPIN BLOWBARS WHICH IS USUALLY DONE BY PUSHINGONCE WITH THE FOOT. HE SLIPPED DUE TO MUDDY COND ITIONS AND HIS RIGHT FOOT BECAME CAUGHT BY A BLOWBAR. THIS GAVE A CRUSH INJURY TO HIS FOOT CAUSING A SMALL FRACTURE TO FIRST METATARSAL AND LARGE JOINT CAPSULE.
EE WAS STANDING WITH ONE FOOTON A LADDER AND PIPE AND THE OTHER ON A JACK LEG WHILE INSULATING A PIPE ABOVE THE EXTRA STORAGE TANK IN SOLVENT EXTRACTION. HISHSOES HAD SOME OIL ON THEM AND HE S LIPPED ENDING UP WITH BOTH LEGS BETWEEN TWO RUNGS ON THE LADDER. HIS FEET DIDN'T QUITE TOUCH THE FLOOR SO HE HAD TO PULLE HIMSELF LOOSE TO GET DOWN. BRUISES AND ABRASIONS. (SEE ATTACHED)
EMPLOYEE WAS STANDING ON ELEVATOR PADDLES TO THE 633D SCRAPER ABOUT HALF WAY UP REPLACING RETAINING PINS IN THE ELEVATOR CHAIN.EMPLOYEE LOST FOOTING ,POSSIBLY DUE TO SOME OIL ON HIS SHOES,AND FELL 4 TO 5 FEET TO GROUND TWISTING LEFT KNEE WHEN HE LANDED.
AN EMPLOYEE STGEPPED OFF OF A CATERPILLAR D8 DOZER ON UNSTABLE GBROUND LOST HIS BALANCE & FELL CAUSING INJURY TO LOWER PART OF HIS RT.ARM INITIALLY WAS CONSIDERED ONLY A SPRAIN AFTER 48 HRS.DI SCOMFORT CONTINUED & HIS ARM WAS X-RAYED INDICATING A SLIGHT FX.OR CHIP TO THE LOWER RT.ARM A LEATHER BRACE WAS PUT ON HIS ASRM FOR SUPPORT EMPLOYEES DUTIES WERE NOT INTERRUPTED NO LOST TIME
EMPL STANDING WITH ONE FOOT ON TOP OF TRACK OF DOZER AND THE OTHER FOOT ON THE PUSH ARM,HE ATTEMPTED TO OPEN ENGINE COMPARTMENT DOOR AND LOST HIS BALANCE.AS HE FELL BACKWARD HIS FOOT CAUGHT AN D HE LANDED ON THE GROUND ON HIS LOWER BACK.FALL WAS 5\.
THE EMPLOYEE ALLEGED THAT WHILE LIFTING A HYDRAULIC JACK ONTO TWO 12X12 WOODEN TIES HE LOST HIS FOOTING ON LOOSE ROCK, CAUSING HIMSELF TO LOOSE HIS BALAC HE FELL FORWARD ONTO THE JACK CUTTING HIS LEFT EYE BROW. HE WAS WEARING SAFETY GLASSES. HE REQUIRED 5 SUTURES.
