Helper parked truck by drill on other side of shot berm. Then crawled over berm to get to the drill. As EE was going over the berm, fell and broke arm. Helper should have walked around to opening on berm instead of going over it.
On 8/30/10 employee, while dismounting off of the LULL forklift stepped on a rock approximately 3" and rolled his left foot causing him to fall and land on his left arm. The impact hurt his foot and arm requiring him to get it checked out by a doctor.
