Miner was turning water pressure back on at 2 block 2 east at the end of the belt spur (after installing booster pump). His hands slipped off handle and fell over backwards hitting the back of his head on the edge of a supply car causing a laceration that required staples. No problems with mining methods, ppe, training or compliance issues.

Injured replaced a piece of grating on slope catwalk at 2780, then walked down catwalk and a separate piece of grating failed and he fell striking his right shoulder on the concrete floor below. Started missing work much 11/23/15 for persistent shoulder pain. No problems with mining methods, training, ppe or compliance issues.

While carrying a 2" air pump in the bleeder walkway near 21W tap he stepped on a section of slick line and fell backwards, landing on his back. The location where he tripped was dry. He is experiencing localized pain in the upper lumbar and has been referred to a specialist. No problems with training, PPE, mining machines or compliance issues.

Employee was carrying come-a-long on belt move when he tripped on chain and fell striking his right shoulder. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved

Employee was injured on 10/04/2015 and continued to work until 11/03/2015 when doctor changed treatment and took him off work. He was reinstalling guarding when he turned and slipped hyperextending his right knee. All personal safety items were being used. It did not involve compliance issues, no equipment involved. It did not involve job proficiency or attitude.

Employee was helping shearer operator spin the shearer drum when the operator fell into him causing him to fall striking his neck on the panline. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Individual was dismounting 24 track mobile not concentrating, caught his foot on the vehicle frame or safety chain causing him to fall (sprain / strain) to the left foot. This did not involve any compliance issues. No equipment involved. This did not involve proficiency or attitude. All personal safety items were being utilized.

Walking through the dust chute on Metz Bottom and fell, spraining his right ankle. He is unsure whether he tripped, or if his problematic surgically repaired knee gave out and caused the fall. No problems with mining machines, training, PPE, or compliance issues.

Employee had cleaned 15 tail and was walking back to his mantrip, when he stepped over the rail he rolled his right ankle, resulting in a sprain. He continued to work until 9/22/15, when he missed 3 days due to the injury. No problems with training, ppe, mining machines or compliance issues.

Employee was attempting to pull cable slack by putting cable on shoulder when he went to put cable on shoulder he stepped in a rut and twisted his left ankle. There were no compliance issues. Employee was pulling cable in mud on the 8 south section. While pulling cable employee claimed he stepped in rut injuring his left knee.

During a part 50 audit and citation 9123767 being issued, this report is being filed. Employee while working on the 3rd floor of the prep plant tripped causing a foot strain and contusion. Cause of the injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Individual was not concentrating when stepping down into the pit area of the motor barn. The right knee buckled when stepping on the first step, experiencing pain from the hip to the ankle. It did not involve any compliance issues, no equipment involved. It did not involve job skills training or attitude. Personal safety items were being utilized.

While servicing a set of scoop batteries, the ee slipped off the top of the batteries and struck his right knee against the battery case. Injury caused by failure of ee to recognize slip potential while on top of wet battery case. EE was instructed to service while standing on the ground. EE had on the appropriate PPE. Became reportable on 7/24/15 when EE began missing work.

Employee was dragging bolter cable when he tripped falling on left shoulder. Employee did not start missing work until 8-4-15.There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.

Individual was dragging a 1" wash down hose to clean the 1st. floor of the prep-plant floor. Due to lack of concentration EE slipped and experienced pain in the right groin and right lower back areas. This did not involve compliance issues, no equipment involved. All personal safety items were being utilized.

Employee was hosing in sample building and when he was walking down the step he missed a step rolling his left ankle. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved.

While walking in the Sugar Run mine rescue training room the miner tripped on the leg of a desk falling and striking his face on another desk resulting in a laceration to his upper lip. Received 20 sutures. No problems with training, PPE, mining machines or compliance issues.

There was a water hole in front of the 1 East feeder which was being cleaned by dumping a buggy of material in the water and loading out with a scoop. The injured went to turn feeder on and walked through material (instead of around). Felt pain in left knee trying to pull foot out of mud. No problem with mining machines, training, PPE or compliance issues.

The employee stated that while dismounting a compactor, he jumped off the last step and when he landed on the ground he felt a pull in his abdomen. He has a history of hernias. The accident occurred because the employee jumped off the last step instead of maintaining 3 points of contact as he dismounted the compactor. He had on his required PPE.

The employee stated that as he was walking around a shuttle car, he stumbled, tripped and fell landing on his right shoulder. The accident occurred because the employee was not observant of the mine floor conditions in the area he was walking. A contributing factor may have been the new boots he was wearing for the first time on this day.

Pole came off wire at 33 block 1 North haulage and 220 jeep halted. B43 was following and slid more than 1 1/2 blocks before colliding with 220. Injured was standing in 220 passenger compartment and fell over striking right shoulder on frame of mantrip and receiving contusion. No problems with training, PPE, mining methods, or compliance issues.

At the end of day shift the employee was dismounting a 15 man mantrip passenger compartment on 2 track Metz bottom, and felt a pop in his left knee. Accident reported 3/17/15. No problems with mining equipment, training, PPE or compliance issues known at this time.

The employee stated that while hanging longwall jumper cables, he stepped backwards, stumbled and tripped over broken pieces of blocks that he has previously moved out of his walkway. He stumbled & twisted his left ankle. The accident was caused by the employee's failure to move the materials completely out of his walkway. He had on his appropriate PPE.

Individual was walking along 5-West Belt Line when stepping on loose coal, due to lack of concentration tripped striking the left knee on the ground. There was no equipment involved, it did not involve job skills training or attitude. Personal safety equipment was being used. Individual has previous injuries to his knees. Continued to work until 3/09/2015.

EE was walking down entry he stepped on a rock with a injured ankle & fell further injuring his ankle. The accident occurred because the EE did not ensure a walkway free of extraneous materials. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills/proficiency/training or attitude. No PPE or protective devices on equipment involved.

Individual was not concentrating when dismounting BT-3 Battery ride. When stepping out of vehicle the left knee gave out causing him to roll the left ankle and fall on the left knee, (fractured knee cap). It did not involve any compliance issues, no equipment involved. It did not involve job skills, training or attitude. Personal safety items were being utilized.

Individual was stepping off the front of the Fletcher Twin Boom Bolter. due to lack of concentration twisted the left foot and ankle, experiencing an abrasion to the outer foot. This did not involve any compliance issues, no equipment involved. It did not involve job skills or attitude. All personal items were being utilized.

Employee was helping set up for mining in #2 entry dragging tubes when he tripped and fell falling on right shoulder. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Individual stepped off a track rail due to lack of concentration catching the left foot on a track tie causing the left leg to extend backwards causing strain to the calf muscle. This did not involve compliance issues, no equipment involved. It did not involve job proficiency, training or attitude. All personnel safety items were being utilized.

While opening an airlock door, the employee lost his balance and fell to the ground. The accident occurred because the employee was not paying attention to the task he was performing. The employee did not violate any rules, regulations or policies. He had on his appropriate PPE materials. He has a middle of the road attitude.

Employee was walking through supply yard and slipped on patch of ice hyper extending right knee. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Individual was walking, crossing over outrigger on the section tailpiece when catching his left foot on the outrigger causing him to fall, striking the right elbow. This did not involve any compliance issues, no equipment involved. It did not involve job skill, proficiency, training, or attitude. All personal protective items were being utilized.

The employee claims that while walking along the belt, he stepped in hole in the pavement, tripped and fell, injuring his left foot. The accident was caused by the failure of the employee to observe, recognize the bottom conditions and take the appropriate preventative measures. The employee was in compliance with rules, regulation and policies.

When filling the water buffalo with water, pressure was increased to the hose and the individual lost balance and falling on scoop blade striking the upper left leg and shoulder area. This did not involve any compliance issues, no equipment involved, it did not involve training or attitude.

Employee was walking the off side of 2 belt around the water separator checking the bed rails and slipped, falling on his posterior. Medical examination revealed a compression fracture and disc impingement to a thoracic vertebrate. No problem with mining methods, training, ppe, or compliance issues.

Employee had finished dragging rock dust hose at 99 block #8 entry 9 South when the hose energized unexpectedly, striking him in the groin. This caused him to fall down, twisting his left foot and tearing some of the calf ligiments. No problem with mining machines, methods, PPE, training or compliance issues.

The ee stated that while carrying roof bolting material, he stepped on uneven bottom and twisted his left ankle. The ee has previously experienced a non-work related severe ankle fracture which required the insertion of a metal plate, pins and screws to re-stabilize the ankle. The ee failed to take the necessary precautions as to his foot placement while working.

Employee was walking up the track and went to get out of the track heading and tripped over the rail and hit his forehead on the cross tie. Required 3 stitches.

Employee was walking around machine and tripped caught hand on roll of mesh and fell. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because the employee was not aware of his surroundings and tripped.

Employee was walking the track at 1 West checking tracking readers when he twisted his ankle resulting in a fracture. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Helping load drive motors for 24-D and scoop operator was against motor. Employee walked around scoop towards right rib, slipped and grabbed scoop with left hand. Felt something pop in left shoulder area. No problems with training, PPE, mining machinery, or compliance issues.

Individual was dragging a door kit alongside the supply train on 13 WHG section when he twisted his ankle on loose coal/rock. He was removed from work by Med Express Fairmont, Fairmont, WV for swollen ankle and pain.

Employee was walking along the stage loader on the 2 west longwall when he fell and tried to catch himself injuring his right index finger. The injury resulted in a fracture and laceration. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude.

Individual tripped over a track switch throw striking the lower right leg. Trained on walking about mine, Individual continued to work until 10/08/2014 due to restrictions from doctor.

Using cross over at 3D transfer over 11 belt when the bottom 3 steps gave way and he fell to mine floor. Weakened steps were hidden by deep rock dust. Contusions to neck, shoulder and back. Referred to physical therapy for shoulder pain on 9/9/14. No problems with mining methods, machinery, PPE, training or compliance issues.

While dismounting a diesel motor near the end of track in 23D the injured's metatarsal bones in his right foot felt like they shifted. Employee heard pop and felt pain. No problems with mining methods, machinery, training or compliance issues. Diagnosed with metatarsal sprain.

While assisting with a belt move, the employee claims to have tripped and fell. In falling, he struck his knee on the edge of a cement block. The injury did not involve any aspect of compliance with rules, regulations, any mining equipment, nor any personal protective equipment.

At the end of afternoon shift, the injured was showering in the women's bathhouse at Sugar Run. She was barefoot and fell out of the shower, twisting left ankle, striking right knee on floor and hitting head on the bench. Sustained sprained ankle. No problem with training, mining machines, PPE or compliance issues.

Injured was climbing down off of 22D middrive when he lost his footing and his left leg went under the wing on the middrive, twisting his left knee. There was no problem with mining methods, machinery, training, PPE, or compliance issues.
