Employee was attempting to repair a support on a float barge when EE's foot slipped and EE fell over onto hip causing a bruised hipbone.

Employee was standing on front platform helping a mechanic bolt a light bracket on the lift cylinder of a D11 bulldozer after they had replaced Hyd. hoses. The employee lost balance and handhold causing EE to fall. Employee received a cut on the head that required stitches.

Employee was getting a fuel hose to fuel a loader and slipped on ice and snow pack, employee stuck hand out to catch self. Employee's hand landed up against the ladder bending index finger back and fracturing it.

Employee was climbing down off Highwall Miner and either slipped or missed a step and fell to the deck and then onto the ground. Employee broke employee's leg right above employee's ankle in the fall.

Operator noticed that a culvert was plugged up on one end so ee got off machine to go and shovel it out. On ee way ee twisted ankle on uneven ground on the pit floor. Operator worked the rest of the shift but the next morning ankle was swollen so ee called off work and went to the doctor.

EE was using a socket to tighten a lid on a Hyd. filter lid and the wrench slipped causing the EE to lose his balance and fall.

Employee was replacing teeth on a 992 loader, he was carrying one over to the loader and slipped and fell hitting his shoulder on the ground. He has worked with a pain in his shoulder, now he has decided to have it repaired. Doctors have told him that he has a small tear in his rotator cuff.

Employee was standing on the stinger of a 657 scrapper power washing. The hose of the power washer become tangled under the tire of the pan and EE pulled on it. When it came loose it caused him to lose his balance and fall backwards about 4'6".

Employee was climbing onto Trac Hoe and slipped. He cut his leg on the track. He received 5 stitches.

Correction to E- Document 781351 EE was to report to work on Thursday night , but was unable to because his knee was bruised from previous days Accident.

Employee jumped from his mechanic's truck to put a fire out and when he returned to his truck he noticed his knee was hurting. He went to his doctor and his doctor told him to be off for 2 weeks.

EE was climbing onto a Komatsu 475 trackhoe to start it when he hit his knee on the track. He reported it to us and he said it wasn't bad enough to go home. He never missed a day till 1/14/08. He has went to doctors and they done a minor surgery on it. EE will be off for couple weeks.

Employee was working on 4400 TK bed. He was carrying a piece of steel, slipped on wet bed (from dew) and tried to catch himself from falling with his left hand. It bent his little finger backwards. Doctor said it tore tendons and for him to take two days off.

Was helping lube man change oil in dozer,needed a tool that was on hood of dozer ,climbed up to reach tool and slipped, on the way down hit his arm on a door that was open . went to hospital , he required 7 stiches

Rainy, wet, muddy. Employee was climbing the ladder on his lube truck when his foot slipped off of the ladder causing all his weight to be on his right arm. IT shot a pain down his arm.

Operator claimed he fell off a grader tire, falling down between tire and frame. This was on 09/13/06. Our Personel Director informed me last week that he now needs surgery. He never missed a day until now.

Stepped down from ladder onto frozen uneven ground, causing ankle to turn over, sprain.

Leaving at quitting time - EE was walking up a grade to his car at a fast pace to get out of the rain when he slipped in some mud and fell.
