EE was climbing on dozer. He stepped in grease and his foot slipped. He caught himself injuring his shoulder.

Foot slipped & fell against handrails

Stepping off of gravel pile - twisted right ankle

Mud was on steps of service truck causing EE to slip when getting in truck, hitting his face on the bottom door jam, knocking his front tooth loose.

Mounting 922C endloader. Slipped when he lost his footing, catching himself with handrails resulting in pulling his right shoulder.

Operator was dismounting a drill. As he put his left foot on the ground, it went into a void, or hole, in the bench causing his foot to turn toes up. This stretched the back of heel and back of calf. He still had his other foot and both hands on the ladder, preventing him from putting his full weight on his foot.

EE was walking around his truck, due to a previous accident, his left knee gave out. He fell to the ground, hitting a crib block with his left knee.

EE stated that he was standing on a berm and stumbled which resulted in falling approx 3' onto his shoulder. Shoulder jammed on left side & right shin had small abrasion.

EE was operating truck #107, hualing coal from pit to stockpile area. EE stopped his truck on hualroad to urinate, as he was standing he lost his balance and fell to the ground.

Mechanic was working on lights on 301 D10N when operator EE went to get off of dozer to use the bathroom when he slipped and fell on push arm of dozer. NOTE: He missed 6/14/05, worked thru 6/15/05 and missed 6/16/05 and 06/17/05

EE WAS OPERATING HYDRO SEEDER. HE STATED THAT HE WAS CLIMBING GRADE WHEN MACHINE POWERED OUT. EE PLACED THE MACHINE IN NEUTRAL AND IT STARTED ROLLING BACKWARDS. EE PANICKED AND JUMPED OUT. THE MACHINE ROLLED APPROX. 40 FT. AND STOPPED. EE HAS A SLIGHT HAIRLINE FRACTURE OF HIS TIBIA.

EMPLOYEE SAID HE FELT SOMETHING PULL IN HIS BACK AS HE CLIMBED THE STEPS UP TO HIS 777C ROCK TRUCK AT THE BEGINNING OF SHIFT. PAIN WORSENED AS DAY PROGRESSED. AT 10:30 HE WAS TRANSPORTED TO SA ME GENERAL DIVISION BY COMPANY PERSONNEL.

FUELER MISSED BOTTOM STEP ON FUEL TRUCK LADDER THUS FALLING AND EXPERIENCED LOWER BACK PAIN.

EMPLOYEE SLIPPED OFF LADDER ON CATERPLLER 777C ROCK TRUCK. FELL TO GROUND FRACTURING HIS SCAPULA (SHOULDER BLADE). EMPLOYEE WAS ATTEMPTING TO CHECK WATER LEVEL IN RADIATOR.

EMPLOYEE PERFOREMING MINOR REPAIRS TO HYDRAULIC TANK ON DIDN BULLDOZER SLIPPED & FELL OFF IDLE MACHINE EMPLOYEE PUT DOWN HIS LEFT HAND TO CATCH HIMSELF & SUSTAINED A FRACTURE TO HIS LEFT WRIST DOING SO
