Mostrando los 23Employee tripped while walking down a flight of stairs. Employee was diagnosed by an Emergency Room physician as having a concussion from hitting his head during the fall, as well as a Distal Bicep Tear which will require surgery.
While unhooking the D-ring rope from the back deck of the Rubber Tire Dozer, the employee lost his grip on the grab bar and fell approximately 4' to the ground. The employee landed on his left arm when making contact with the ground, and reported pain in his left wrist.
Employee was pulling on a rope that was attached to a road sign, the employee was trying to place the rope over the ball hitch of his pick up truck so he could move the sign to another part of the mine. As the employee was pulling on the rope trying to get the sign close enough to the ball hitch of the pickup he slipped and his head struck the flat bed pickup.
After completing his shift, the injured miner was walking through the parking lot to get in his car pool. When he passed between two parked cars, he slipped and fell. The fall caused soft tissue injury to the employee's left knee and shoulder. The employee was given first aid treatment but was not able to return to his next scheduled shift on January 2, 2014.

A Maintenance Inspector falls off the back of a work vehicle. He lands on the ground, breaking one rib on his left side. He had stepped on an open tool box lid, which then shut, causing him to lose his balance.
A mechanic was working off the rear platform of an 830B Caterpillar Rubber Tire dozer. He was changing a light bulb. As he turned to his left to move toward the boarding ladder, his left foot contacted a tow hook that had been welded on top of the rear bumper. He fell approximately 4.5 feet, slightly fracturing his left elbow.
A Haul truck operator parked her truck for fuel and service. She left the cab and was going to descend the truck stairway to isolate the truck. She stepped on a pile of coal on the edge of the deck and lost her balance. She fell forward onto the stairway landing rolling her right ankle over. In doing so, she fractured the distal end of her right fibula.
Employee slipped on last step hitting her head. Received a laceration on the right eyebrow about 1/2 inch long. Wound required 7 sutures to close it.

THE EE WAS DIAGNOSED WITH A CONTUSION TO THE LEFT RIB.
OPERATOR OF CAT 16G MOTOR GRADER WAS EXITING THE CAB AND DESCENDING DOWN THE LADDER. THE EE'S RIGHT FOOT SLIPPED FROM THE LADDER CAUSING LOSS OF FOOTING. OPERATOR FELL TO THE GROUND STRIKING O N OLD GUARD WITH BUTTOCK.
A MAINTENANCE EE WAS TAKING A DRIVELINE OUT OF A 789 CAT HAUL TRUCK. THE EE STEPPED FROM THE CENTER CROSS FRAME MEMBER TO ONE OF THE BRAKE CANS WITH HIS RIGHT FOOT. THE EE THEN STEPPED ONTO TH E TRANSMISSION HAVING WITH HIS LEFT FOOT. THE EELEFT FOOT SLIPPED FROM THE TRANSMISSION HOUSING TWISTING THE LEFT KNEE.
EE WAS RETURNING A LADDER TO REAR OF SERV TRK INDOING SO HE STEPPED ON REAR BUMPER OF TRK TO REPOSITION THE LADDER & HIS FOOT SLIPPED OFF THE BUMPER STRIKING THE GROUND CAUSING A FRACTURE TO L EFT HEEL
