Mostrando los 14Employee was walking in the maintenance shop and while reaching for a box of rags the employee tripped over a wooden pallet causing the employee to fall to the ground. Employee fell on wrist causing an injury.
Employee was getting on the plant elevator when his right foot slipped, causing him to lose his balance. He twisted his left knee which caused him to buckle and fall to the ground.
He was cutting weeds on the side of a hill and his right foot slipped on the wet mud and he twisted the lower part of his back causing injury to his lower back. He has had a history of back problems due to a past accident.
EE WAS MOUNTING THE D-10 DOZER (MAINTAINED 3 POINT CONTACT)WHEN HIS LEFT FOOT SLIPPED ON THE MUDDY TRACK & WENT TOWARD THE DOZER BODY BETWEEN TRACK & B0DY. HE TRIED TO CATCH HIMSELF WITH HIS LEFT HAND & BENT BACK HIS MIDDLE FINGER. THE SAFETY DEPT WAS NOT MADE AWARE OF THIS ACCIDENT UNTIL 8/19/05.
EE went up a 12 foot fiberglass ladder to perform work on ceiling area. After assesing how to tie off EE started back down the ladder, he lost his balance and fell about 8 feet landing on his back. He was transported to the hospital via ambulance where it was found that he had two cracked vertebrae and was suffering from a concusion.
EMPLOYEE WAS GOING OVER A BANK TO THE FOREIGN COAL BELT TUNNEL. HE LOST HIS FOOTING AND FELL LANDING ON HIS WRIST.
STEPPED OVER A LARGE PIECE OF GRATING TO GET IT UNSTUCK & STEPPED AT AN ANGLE ON THE END OF ANOTHER WHICH MADE IT SLIP OFF THE BEAM - HE FELL IN HOPPER & CAUGHT ROPE GOING DOWN & ROPE CAUGHT R IBS ON RIGHT SIDE.
TEE WAS HELPING LOAD A VC 48 DRYER ON A FLAT BED TRUCK. HE SLIPPED WHILE STEPPING DOWN AND FELL BACKWARDS, USING HIS LEFT SIDE TO CATCH HIS FALL.
EMP WAS WALKING FROM THE JOB SITE TO THE BATH HOUSE. AS HE REACHED THE BOTTOM STEP HIS FOOT TURNED CAUSING HIM TO FALL ONTO THE HAND RAIL. WENT TO HOSPITAL THE NEXT DAY AND AN XRAY CONFIRMED A FRACTURED RIB.
EE WAS STANDING ON STEEL PIPE WHEN HE SLIPPED AND FELL BACKWARDS. HE GRABBED AN ANGLE BRACE TO STOP HIS FALL. AS A RESULT HE DISLOCATED HIS RIGHT SHOULDER. THE EE WAS WEARING A SAFETY BELT AND HE WAS TIRED OFF AT THE TIME OF HIS FALL. HE STATED THAT HE GRABBED THE ANGLE AS A REFLEX OF FALLING.
WAS GETTING OUT OF COAL TRUCK, SLIPPED AND WHEN STEPPED ON PAVEMENT TWISTED ANKLE. LEFT ANKLE.
EMPLOYEE WAS AT THE CAR HOIST CHECKING THE GEAR DRIVE AND DISCOVERED IT AS "GOBBED OUT". HE DECIDED TO TAKE THE FIRE HOSE AND CLEAN IT OUT. AT THE ANGLE HE WAS STANDING, THE WATER WAS SPRAYING BACK IN HIS FACE. HE MOVED OVER TO THE EDGE OF THE HOIST & CLEANED IN THAT AREA FOR A GOOD WHILE. ACCORDING TO EE, THE PRESSURE INCREASED, HE TOSSED HOSE, JUMPED ABOUT 4' & FOOT WENT INTO HOL
WHILE WORKING ON THE OFF SIDE OF A BELT ON A SCAFFOLD BOARD ONE END OF THE BOARD WAS LYING ON A PIECE OF ANGLE WELDING TO CHANNEL ON SIDE OF BELT. THE ANGLE BENT AND DROPPED THE SCAFFOLD AND HE FELL TO THE FLOOR. THERE WAS 24" BETWEEN BELT AND WALL TEN FEET HIGH.
