EE was in the process of working on batteries for a battery hauler. He was stepping from the ground onto the batteries when he twisted his right knee. He didn't start missing work until March 16, 2007 to have surgery on his knee.

Employee stated he was coming down the steps from the bath house (it was drizzling rain, steps were wet, handrail wet also) when he slipped on the third step from the bottom causing him to fall to the second step from the bottom.
EMPLOYEE WAS WALKING FROM THE UNIT MAN TRIP. WHEN HE ENTERED THE BELT ENTRY HE STEPPED IN A HOLE CAUSING HIM TO SPRAIN HIS ANKLE.
Employee was loading the bolter when he picked up a box of glue and twisted his knee which caused him to fall and hit the rail on the side of the pin trailer. He worked until approximately 10:00 a.m. when he notified his supervisor he was wanting to go out of the mine.
While unhooking a chain, he was in the scoop bucket, coal slack and wet, he twisted his knee. He missed the first day due to the accident on Feb 17, 05. He is scheduled to see the doctor on Feb 28, 05.
EE twisted knee 1-10-05 after going to Dr and not missing any work it was decided to do some cleaning of knee. Diagnosis was osteoarthritis of knee, but an MRI showed tear which was repaired on 3-3-05 while knee was being checked. EE's first day to miss was 3-3-05 original injury was 1-10-05.
EE STATED, "HE WAS PULLING ON A CURTAIN THAT APPEARED TO BE UNDER SOME GOB". WHEN HE PULLED ON IT, THE CURTAIN CAME LOOSE CAUSING HIM TO FALL BACK ONTO HIS BACK. THAT IS WHEN HE FELT PAIN IN HIS LOWER BACK.
EE WAS WALKING WHEN HE TRIPPED OVER A CABLE CAUSING HIM TO FALL ON HIS LEFT HAND (THUMB).
WHILE WALKING TO HIS VEHICLE, EE STEPPED IN A SMALL DITCH CAUSING HIM TO TURN HIS ANKLE, THE FIRST DAY MISSED IS 4-17-03.
EE EXITED HIS RAM CAR TO HANG A CURTAIN. AS HE STARTED TO RE-ENTER THE RAM CAR, HIS RIGHT HAND SLIPPED CAUSING HIM TO HIT HIS RIBS.
EE SLIPPED ON A MUDDY FLOOR IN THE WAREHOUSE. HE GRABBED FOR SOMETHING TO CATCH HIMSELF & CUT HIS ARM ON EDGE OF SHELF.
EE WAS IN THE PROCESS OF STACKING PIN PLATES ON BOLTER. HE REACHED DOWN TO PICK UP ANOTHE PIN PLATE WHEN HE FELL INTO THE CORNER OF A PLATE ON THE STACK.
WHILE SHOVELING BELT TAILPIECE, EMPLOYEE TURNED TO WALK AWAY WHEN HE TRIPPED ON CHAIN TIE DOWN OF TAILPIECE. IT CAUSED EMPLOYEE TO HIS HIS KNEEON A ROCK.
WALKING INTO BATH HOUSE FROM THE OUTSIDE TO START WORK, EE TRIPPED IN THE DOORWAY. HE FELL, CATCHING HIMSELF, JARRING HIS BACK AND NECK. (FIRST DAY MISSED--5-22-00)
