Employee was tied off while cutting a piece of angle iron. The object they were tied off on broke causing employee to fall approximately 6' to 8' feet. Employee bruised their spline and has lower back pain.

Stepped down onto a shaker box and slipped and twisted left knee. Wet surface.

Injured was in process of opening 3" metal ball valve, when foot slipped on wet fabr-form ditch liner, causing EE to fall injuring palm of left hand.

Injured started down steps from 1st level of No. 1 refuse bin when foot slipped causing employee to jerk suddenly and sprain lower back. Injured was not maintaining 3 point contact and step was wet from rain. Note: Employee works three 12 hour shifts per week.

Employee was exiting the trucker auger and slipped on steps due to ice and snow. He put his left arm out to try and catch himself. He injured himself when he fell across a railing.

A security guard was asked to run off fishermen from a creek located near mine property. In the process of trying to contact fishermen she fell down creek bank on her rear hitting her hands and backside on some large rocks.

Employee slipped and fell while working on replacing steel beams. Fall arrest equipment consisting of full-body harness, twin-leg lanyard and horizontal ratline arrested the fall, but his right arm bumped a piece of angle iron close to his work, causing a mild sprain to his right shoulder area.

Injured was fueling D8T dozer at #2 switchback fuel tank, fell off tracks/platform of dozer. Injured fell about 70 inches onto ground and landed on right hip.

Employee was carrying a sample bag of coal and while walking he tripped over a large chunk of coal and fell on his lower back/hip area.

Injured reported to work sick at his stomach and feeling nauseated. Injured was in load out when he said he felt sick and stepped outside & threw up. At this time he got light headed, passed out & fell backwards striking his head on metal step causing laceration to back of head.

WHILE ON STEP LADDER CHANGING 4 FT FLUORSCENT LIGHT BULBS THE STEP LADDER FOLDED AND HE FELL ONT OP OF FILE CABIENT. HE FELT PAIN IN LOWER BACK, LEFT LEG, LEFT WRIST.

WALKING BACKWARDS USING PUSH MOWER-STEPPED IN HOLE AND FELL.

POURING NAPTHA FROM 55 GALLON DRUM INTO 5 GALLON BUCKET. SLIPPED AND FELL CAUSING INJURY.

EE AND CO-WORKER WERE ON THE TOP WALKWAY/PLATFORM LEVEL AT THE GRAVITY TAKE-UP (COUNTERWEIGHT) WHEN HE STEPPED OFF OF HANDRAIL DOWN ONTO MAIN WALKWAY PLATFORM, & STEPPED BACKWARDS, FALLING THR OUGH A 24" X 30" OPENING AND LANDED ON THE WALKWAY PLATFORM (APPROX) 14 1/2' BELOW. AMBULANCE TRANSPORTED HIM TO HOSPITAL WHERE HE WAS ADMITTED OVERNIGHT FOR OBSERVATION.

INJURED CLAIMS THAT WHILE GETTING OFF OF BOBCAT LOADER THAT HIS FOOT SLIPPED CAUSING HIM TO FALL IN BUCKET CAUSING PAIN TO NECK & BACK.

INJURED WAS TURNING OVER 55 GAL. DRUM OF COAL TO COLLECT SAMPLES WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL BACKWARDS HITTING BACK AGAINST ANOTHER BARREL.

STEPS WERE BENT AND WET, EE FELL ALLEDGEDLY HURTING LOWER BACK, NECK AND RIGHT SHOULDER.

INJURED CLAINS TO HAVE SLIPPED AND TWISTED BACK WHILE CLEANING ON VORSIEVE SCREEN WITH A PIECE OF 2" PVC PIPE WHILE WORKING ON 3RD FLOOR OF PREP. PLANT THE ALLEDGED INJURY ACCURED ON 3-31-98 A ND WAS NOT PROPERLY REPORTED TO SUPERVISOR UNTIL4-13-98 WHEN INJURED VISITED EMERGECY RM AT HOPSPITAL.

INJURED WAS EXITING SAMPLE BUILDING WHEN HE TRIPPED OVER DOOR SATCH CAUSING HIM TO FALL TO THE GROUND HITTING HIS CHIN ON GROUND CAUSING INJURY.

INJURED CLAIMS TO HAVE BEEN GOING DOWN STEPS BETWEEN THE 4TH AND 3RD FLOORS OF PREP PLANT WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL ON THE SIDE OF HIS LEFT FOOT. INJURED WAS WEARING RUBBER MET ATARSAL BOOTS.

THIS TIME STILL UNDER DR CARE PROBABLE LOST TIME 4 TO 6 WEEKS MAY HAVE TO HAVE KNEE SURGERY.

INJURED CLIAMS TO HAVE BEEN WALKING DOWN STEPS AND FOOT SLIPPED IN SNOW. INJURY HELD ON TO BOTH TO KEEP FROM FALLING WHICH RESULTE DIN MOST OF HIS BODY WEIGHT BEING SUPPORTED BY SHOULDER AND NECK INJURED FELT PAIN IN NECK AND SHOULDER AREAAT THE TIME.

INJURED WAS GOING DOWN STEPS FROM 4TH FLOOR TO 3RD FLOOR OF PREP PLANT WHEN HE MISSED THE 2ND STEP FROM THE BOTTOM CAUSING HIM TO FALL ON THE CONCRETE FLOOR. INJURED STATED THAT HE FELL ON THE SIDE OF HIS LEFT FOOT CAUSING THE INJURY. INJURED WAS WEARING METATARSAL GUARD BOOTS(LEATHER).
