Operator was informed that medical treatment was provided on 6/6/2014 for injuries sustained to the shoulder when employee slipped on ice. The employee no longer works at this mine.

Mostrando los 22Walking to lock out main mine fan door, thick ice under the snow. Employee slipped and fell twice while traveling to the fan door causing pain in his right shoulder.
Employee was hanging MRS cable and slipped causing him to cut his finger on the corner of a roof strap. He was transported to Summersville Memorial Hospital where he received 6 sutures. He received a full release.
Employee had exited out of his truck to perform a post shift vehicle inspection. He stepped on the edge of pavement and gravel meet; his ankle twisted. X-rays revealed a fracture of the left ankle. Employee reported un-witnessed accident, stating he slipped on bathhouse floor, he finished shift and went to doctor next day, employee had MRI on 07/16/2012, no tears or deformities were found, however a cyst was found. Operator has determined no accident occurred, submitting for MSHA review, as Occupational Illness
Slipped on water line and grabbed on "rock box", laceration on left palm, transported to SMH and received 3 stitches.
Employee was walking beltline when he twisted his ankle due to muddy conditions. He was transported to surface and transported by ambulance to Greenbrier Med. Center. x-rays reveled a fracture to left ankle
Employee was walking through #4 entry at brk 21 when he stepped in sloppy muddy conditions and fell down. He stated his ankle "popped" when he fell. He was transported to surface and taken by ambulance to Summersville Hospital. X-Rays were negative, released with instructions to keep right foot elevated until swelling went away.
Slipped on steps of coal truck while exiting the cab. Fell to ground striking right elbow and back. Transported to Summersville Regional Hospital. No witnesses to the accident.
Two co-workers jumped on employees back pushing him to the ground, (horseplay) resulting in 3 broken ribs for the employee.

EE was moving a cement pallet out of his way when he slipped/tripped over some plastic wrap that was on the mine floor. EE pulled some ligaments in his lower back. He did not go off of work until 10/16/2009.
Employee was hanging a chain hoist to hold the belt head roller and stepped off of an 8" H-beam and twisted his right ankle.
EE was removing the bolter cuthead. After removing the cuthead, ee was trying to untangle the cable and tripped over gob. He caught himself with his left arm straining his shoulder. EE continued to have pain in his shoulder & went to his family doctor on 1/10/08. His doctor diagnosed him with strained ligaments & took him off work for physical therapy.
Employee put the #2 section forklift on charge at the end of his shift and was walking to the man-trip to ride outside when he slipped and fell. He stated that the bottom was uneven and he stepped in a hole. X-rays show spiral fractures of the tibia and fibula bones.

CLIMBING UP LADDER OF CINDER TRUCK. FEET SLIPPED AND HE CAUGHT HIMSELF BY HIS HANDS ON THE RUNG OF THE LADDER. HE HUNG BY HIS ARMS AND THEN LET HIMSELF DOWN TO THE GROUND.
EE HAD PARKED THE ITZ8 ENDLOADER AND WAS CLIMBING DOWN THE LADDER ON THE SIDE. AS HE PLACED HIS LEFT FOOT ON THE GROUND AND PUT WEIGHT ON THE LEG HE FEELS HE INJURED THE SHIN AREA OF HIS LEFT LEG. BEGAN MISSING WORK 1-21-00.
