Was in the process of coupling trailers back together. He turned to walk back to his hauler and foot rolled over in a rut and he felt a pull in his left knee.

Employee was getting out of scoop deck. He had his right foot on the ground and was removing his left leg to plant on the ground. When he twisted his right knee popped. He continued working until 6/11/2015 when he had surgery on his right knee.

Walking through door on bottom, fell through door and sprained left foot/ ankle.

EE was washing the road grader when his feet became entangled in the wash down hose causing him to trip and fall on his right knee. This resulted in ee not being able to return to work on his next scheduled shift.

Employee was reclaiming some belt framing. He stepped off of a supply trailer onto the ground and twisted his left ankle. This resulted in a sprained left ankle, that did not allow him to return to work.

Walking down a belt line and stepped in a hole and fell and twisted his hip and lower back. This was on 2/24/2014 it was a reported only accident. On 3/5/2014 he went to see a Dr. an x-ray was completed and the results were negative. He was still having some pain so a MRI was completed on 3/25/2014 that revealed a fracture to his hip. Placing him off of work.

Employee was walking into 3D header and stepped hole twisting his right knee, on 10/15/2013. He went and had x-rays completed on 10/15/2013 and had negative findings. He then saw a orthopedic surgeon on 10/31/2013, which he placed him off of work until 11/04/2013.

Employee was exiting his ram car, when his foot landed on uneven ground, as he spun out of the ram car his foot remained planted as his body turned causing a spiral fracture to his left lower leg.

Employee was in the process of walking to hang the miner cable when he stepped in a hole. Which caused his ankle to roll and the employee fell awkwardly on his leg.

Injured employee was in the sitting position with right leg under him, left leg out straight. He went to get up using his right leg, as he got up his ankle rolled he lost his balance and fell back on his ankle.

Employee was getting out of a scoop and lost his balance, he hit his head on the rib. This caused him to sprain his neck. He began to miss work on 10-29-2010.

Employee was walking beside the section feeder when he stepped over some coal and rock, the employee slipped and bumped his knee on a rock. Employee continued working until he was seen by a physician on 8/4/2010, at that point he was placed off of work and is awaiting surgery for his left knee.

Employee stepped onto mine belt that had mud on the surface of it and caused him to slip and land on his knee. Employee was prescribed physical therapy as a result of this occurrence.

Employee was moving a cable, when he took a step back, lost his balance and fell down. This caused him to twist his left ankle.

Employee was working on a golf cart when he slipped and cut himself above his right eye. This resulted in a laceration and he received four sutures.

Employee was walking when he tripped and fell causing him to fracture his left arm.

Employee was walking in the #7 entry to his roof bolter and stepped in a small hole and twisted his left knee.

Employee was exiting the shower and slipped on a step.

Employee was preparing pins to put up. He placed them on the ground beside the bolter. He then walked up the side of the bolter and tripped over the pins causing his knee to twist.

Employee was standing on a piece of metal he was cutting. When metal fell he fell also about 4 feet.

Employee was in the process of moving an electrical receptacle when he slipped and fell on a bolt that was protruding from the receptacle causing a laceration to his left hand, which required sutures.

EE was steam cleaning a loader using a Landa Pressure Washer. He climbed on the tire and slipped. He landed on his back.

Employee was hanging belt chains on a new belt set-up when a piece of rock fell and caused him to hit his face against the belt structure.

Workman was removing a coal hopper from it's support structure when the walkway he was standing on broke and caused him to fall down approximately 13 feet onto a concrete slab. The fall caused fractures of both ankles.

Employee was attempting to straighten out a roof bolt. The bolt twisted around, causing the employee to lose his balance. As he fell forward, he cut his right ear on the roof bolt plate.

He was on a ladder getting supplies from the attic and fell off the ladder. He recieved a sprained ankle.

Employee was getting off the 3500 shovel at the end of his shift. It was dark and he misjudged the distance from the step to the ground causing him to fall and hurt his ankle.

WALKING ACROSS THE UNIT, STEPPED IN A HOLE TWISTED RIGHT KNEE.

WHILE LOADING BULK TRUCK EE SLIPPED AND FELL ON TOP OF TRUCK.

EE WAS WALKING FROM OLD ROOM SET-UP TO NEW ROOM SET-UP WHEN HE STEPPED ON SOME LOOSE COAL WHERE A X-CUT HAD HOLED THROUGH. AT THIS TIME HIS FOOT SLIPPED AND HE TWISTED HIS KNEE.

EMPLOYEE HAD BEEN WORKING ON #3-D10 CATERPILLER BULLDOZER. HE WENT TO GET A 5 GALLONG CAN OF HYDRAULIC FLUID OUT OF THE BACK OF HIS TRUCK. AS HEWAS LIFTING THE CAN OUT OF THE BED OF THE TRUCK, HE LOST HIS BALANCE AND FELL SIDEWAYS INTO A HOSE REEL THAT WAS MOUNTED ON THE SIDE OF THE TRUCKBED. HE RECEIVED A SMALL LACERATION ON HIS ELBOWAND RECEIVED TWO SUTURES ON HIS ELBOW.

EE WAS BENT OVER TO PICK UP A 3 X 5 HEADER BOARDTO LEVEL BELT AS HE TURNED AROUND HE TWISTED HIS KNEE.

EE WAS OPERATING HAUL TRUCK HAULING MUD AT PIT. HE COMPLETED SHIFT & PARKED TRUCK. AFTER DESCENDING HALFWAY DOWN THE LADDR, HE LET GO WITH HIS RIGHT HAND & REACHED BACK UP TO GET HIS DINNER BU CKET OFF THE PLATFORM. WHILE REACHING FOR HIS BUCKET, HIS FEET SLIPPED OFF OF THE LADDER TO THE GROUND. DURING THE FALL, HE INJURED HIS LEFT SHOULDER AND A LEFT TOE.

TWO EE WERE GOING TO DRIVE BACK TO THE RECOVERY #4 PLANT, TO SEE ABOUT GETTING A COAL SAMPLE. THE ROAD WAS ICED OVER AND THE TRUCK BECAME STUCK ON THE ICE AND WOULD NOT MOVE. INJURED PROCEEDED TO GET OUT OF THE TRUCK TO SEE WHAT HE COULD DO, WHEN HE SLIPPED ON THE ICE AND FELL, LANDING ON HIS RIGHT ELBOW AND WRIST.

EMPLOYEE STATES: WHILE CLIMBING DOWN LADDER FROM BULK TRUCK MY FOOT MISSED THE LAST STEP, FELL AND TWISTED MY ANKLE.

EE WAS CRAWLING OVER A PILE OF ROCK WHEN HE SLIPPED & TWISTED HIS KNEE. THE COMPANY IS STILL ON GOING AN INVESTIGATION OF THIS ACCIDENT

THE LEFT HOIST ROPE HAD BROKEN ON THE 2550 DRAGLINE & BECAME ENTWINED AROUND THE BRIDAL CABLE, MED SIEVE & THE BOOM. A CO-WORKER HAD POSITIONED A D10 BULLDOZER SO THE EE COULD USE THE TRACTOR TO STAND ON & TIE A 1" HEMP HOPE ONTO THE BROKEN WIRE ROPE. WHILE IN THE PROCESS OF TYING THE ROPE, THE CABLE CAME LOOSE & STARTED TO FALL. EE NOTICED IT FALLING & DISMOUNTED TRACTOR, HURT ANK
