Employee was bending a bolt over the tire of a bolter. The bolt rolled over causing EE to lose EE's balance. EE fell to the ground causing a sprain to EE's right thumb.
Employee was carrying a box of glue beside the scoop. EE lost balance and fell hitting head on the scoop bucket causing a laceration above eye which required stitches.
EE was sitting on bucket operating the continuous miner. As EE was getting off of the bucket to move the miner cable, foot slipped, causing EE to overextend groin muscle.
Employee was on a step ladder hanging fiber across 5A belt. The ladder slipped out from under EE and EE from the ladder. EE was about 3 ft. off the ground. When EE landed, EE turned right ankle causing a sprain.
Employee got off of the scoop to roll up the curtain to finish cleaning the face area. EE stepped off of some gob and heard a pop and felt pain in the right leg below the knee. The X-ray was negative for a broken bone and EE was referred to a specialist.
EE was standing on a shuttle car cable with conversion valve out. As EE put the valve back in the cable took up while EE was standing on it, and it flipped EE over on right shoulder. X-rays showed no breaks and EE is home waiting an MRI.
Walking past bolter in back entry on #5 entry on 5 section and tripped on gob and hit t-bar flab hurting nose. Came back next shift but was treated with ice to nose area.
Employee was carrying a half bag of dust as they were rock dusting around section power center. As they stepped over chain at power center they felt a pop in left knee.
Employee was preparing to hang a ventilation curtain extension when EE stepped in a hole twisting right knee.
Employee was walking across section when stepped in a hole (indenture) in mine floor. Doctor took employee off work on 06/20/16 pending the results of any MRI.
Employee was in the process of installing permanent roof support as he leaned forward to straighten bolt he lost his balance causing him to fall forward, mashing the tip of his right index finger between the roof bolt plate and the boom of the bolter. He was taken to local ER and referred to a hand specialist. Surgery was needed to repair the damage to his finger.
Employee was moving Vent curtain and stepped in a hole twisting knee.
Employee was in the process of tramming a continuous miner, as he twisted on his feet in place to watch the tail of the miner he felt pain in his right knee. Employee was seen by a local physician and was removed from work pending MRI results.
While in the process pulling a rock dust hose employee ran head first into a cable bolt, causing him to fall, landing on his right shoulder. Employee continued to work until 7/26/2013 at which time he was seen by a specialist and removed from work pending results from an MRI.
He was getting in deck of roof bolter from the side. He put his legs in with his hands on top of the bolter deck. His hands slipped off hitting lower back area on the back rest of the bolter deck.
Was on a ladder that was leaning against a block wall. The ladder kicked out and he fell approx. 4 ft. to the ground landing on his feet. He originally thought he had twisted or sprained his ankle - later found to have fractured bone in foot.
At 7A BELT HEAD ON 7 SECTION. HE SLIPPED, AND HIT RIGHT FOREARM ON BOTTOM BELT STAND CAUSING A LACERATION THAT REQUIRED 12 SUTURES.
While in the process of exiting a rail mantrip at the battery station, employee twisted his left knee. He was seen by a local physician whom placed off work pending MRI results.
Employee was walking around the front of 1C tail piece, when he stepped into some uneven bottom causing him to slip and twist his left ankle.
Employee was cleaning shop when he stepped over a crib block his knee buckled.
While installing permanent roof support EE tripped and fell on a roof bolt plate that was laying on the mine floor, resulting in a laceration to his left leg that required sutures to close.
Got out of scoop. He felt pain in lower back.
While in the process of dismounting supply motor employee felt pain in his left knee. He stated that he has a history of knee problems. On 12-23-11 an MRI was performed. On 1-6-12 he was seen by a specialist and was informed it was a new tear. This report was delayed to determine if this was a new or old injury.
Employee was in the process of untangling a miner cable and water line when he tripped and twisted his left knee. He was taken to a local hospital; x-rays were performed with negative results. Injured employee returned to work and continued to work, but pain worsens. On 6-28-2011 a MRI was performed. On 7-11-11 the specialist removed him from work, to repair the ACL of his knee.
Employee stated he was walking from employee parking lot to shop when he tripped on water hose and fell. Employee did not report this to Safety Dept until 12-13-10. Is doing therapy on left calf muscle.
Employee stepped onto pump to untangle pull chain. When he stepped off the pump, his foot slipped causing him to fall striking his right side and back on the base of the pump. his doctor removed him from work on 6-1-10.
Injured employee was bending a roof bolt when he slipped and strained his lower back.
While walking toward a personnel carrier, injured employee tripped over a rock. He fell, causing a strain to his ankle. Employee saw a specialist on Wednesday, August 12th. The doctor took him off work on this day.
Injured was walking towards the section when he stepped into a pot hole in the mine floor causing injured employee to loose his balance and fall landing on his right knee.
This report is being filed late due to first believing this event to be sports related. It is now believed this could have been work related. He states that he became entangled in a roof bolt cable, lost his balance and fell to the mine floor.
Injured was standing on the side of a conveyor belt tail piece reaching for some shear pins that were on top of a belt feeder. His foot slipped which caused the employee to loose his balance and fall. He landed on the side of the tail piece of his lower back resulting in a contusion. He visited his family dr on 3/30/09 and was removed from work.
Injured ee was cleaning gob off the track and twisted his knee around backwards.
Employee was in the process of hanging miner cable when his feet got tangled up in the cable causing him to fall to the mine floor. When he fell, he tried to catch himself. This resulted in a bruised right wrist. The employee continued to work and completed his shift. EE did not return for next shift.
Injured EE was in the process of removing a cover lid from a continuous miner to place on mine floor, when EE stepped on some loose bottom, resulting in EE spraining his left ankle. Injured EE continued to work until 12/22/08, when his family doctor removed him from work.
Employee was repairing a conveyor chain when he slipped and fell to the mine floor. He was transported to a local emergency room where x-rays revealed a fracture to the spineous process of the L 3 vertebrae.
Injured was working on the cable reel of a shuttle car. When the cable reel freed up the cable caught his right leg. This caused him to fall and injured his right knee.
Employee stepped into a hole in the mine floor and twisted his right ankle as he was getting off a rubber tired 4 wheeler.
Injury ee was walking around the front of the section feeder when he stepped on some loose coal causing him to loose his balance and fall on his right foot, he was seen by his family dr and removed from work 11-13-07.
EE slipped and fall, striking his right on an open utility knife that was he had laid on the tail piece where he was working. The laceration required two stitches to close. NOTE: the knowledge that he had received two stitches was not brought to the attention of the Safety dept. until 11-19-07
Employee stated that he was assisting in replacing a 6" pipe. He stepped on a piece of angle iron which bent and his foot slipped. This resulted in his foot going between two pipes, causing a possible strain to his lower back and right thigh. Did not start missing work until 7/30/07. Doctor has him off work pending an MRI.
Employee was cleaning around an underground conveyor belt drive when he slipped and fell, twisting his right knee. He did not receive medical attention until 3/4/07 when he visited the local E.R. where x-rays were performed but did not show any damage. He was advised to follow up with his family physician and was taken off work for further testing.
Employee was removing material off a idle conveyor belt when he suddenly tripped causing him to fall resulting in a hairline fracture of the left wrist.
Employee was working on a section belt line tailpiece when suddenly water started spraying from the end of the 4" water line lying next to the belt line. While moving suddenly out of the way to keep from getting wet, employee stumbled and struck his right hand against the side of the feeder frame causing a fracture to the ring finger.
The employee was in the process of moving by hand an electrical receptacle when he slipped and fell to mine floor. During the fall he hit against another electrical receptacle with his right hand causing a laceration to the right thumb. The laceration took 12 stitches to close.
EE was attempting to remove a large rock off a bundle of roof bolts he placed his left foot on the front part of the scoop bucket and his right foot next to the bundle of roof bolts as he was starting to lift the rock his left foot slipped causing him to twist and fall on his left knee.
EE was walking along the rear of a fletcher roof bolter and was unable to lift his leg was due to preexisting arthritic condition. EE was seen at local hospital & ER. Dr advised EE there was evidence of arthritis in the back of knee area. He is following up with his family DR.
EE STATED THAT HE WAS ATTEMPTING TO HANG A CONTINUOUS MINER CABLE ON THE COMPANY NO. 002 SEC. WHILE PUSHING AGAINST THE CABLE TO SECURE IT AGAINST THE RIB, HIS KNEE GAVE WAY AND THE KNEECAP WAS OUT OF PLACE. EE STATED THAT HE SET THE KNEECAP BACK INTO PLACE.
THE EMPLOYEE WAS WALKING ACROSS THECONTINUOUS MINER SECTION WHEN HE STUMBLED AND FELL. DURING THE FALL, HE FELL ON HIS PICK HAMMER THAT HE HAD ATTACHED TO THE SIDE OF HIS LIGHT POUCH. THE PICK HAMMER CAUSED A PUNCTURE WOUND TO THE RIGHT LEG, THICH AREA THAT REQUIRED 6 SUTURES TO CLOSE.
EE stated that he was getting out of the deck of a 488 scoop. He pulled himself out using hand hole cut outs on the canopy. In the process of exiting the deck he placed both feet on the back frame of the scoop deck. When he was out of the deck with both feet resting on the back of the deck getting ready to step onto the ground, his left foot slipped causing his left leg to hit
WHILE STANDING ON TOP OF CONTINOUS MINER THAT WAS PARKED HANGING A VENTILATION CURTAIN, EE STEP HIS LEFT FOOT BETWEEN THE END OF THE CONVER CHAIN AND THE BRACE AT THE END OF BOOM, LOST HIS BALANCE AND FELL WITH HIS LEFT FOOT STUCK CAUSING BOTH FIBIA & TIBIA TO BREAK
