On 6/1/2017 Employee stated while washing the bottom floor of the plant - they drug the hose out the door, misjudged the single concrete step and fell on their left shoulder. On June 2, 2017 employee was examined and diagnosed with a torn muscle in their chest. Employee continued to work full duty until 8/9/2017 when surgery was performed to repair.

Employee was stepping out of battery transport when he struck his head on the mine roof, knocking his hard hat off. As he reached out to grab for his hard hat his momentum carried him forward and he lost his balance. His feet caught on the lip of the deck and he fell out of the cab. As he fell he attempted to catch himself with his right arm breaking his Humerus.

Employee was walking beside the double boom roofbolter when he tripped on some loose rock. As he fell he tried to catch himself with his right arm. He fell onto the floor pulling something in his right shoulder.

Employee tripped and fell while changing batteries on his ram car. Employee claims that he felt something pop in his back when he fell. Employee was taken to hospital and follow up with doctor. He continued to work until 10/24/2012 when he was taken off for bulging disc of the L4 and L5.

Employee was cleaning in the unit 4 tail area, when she heard a rib crack. She took off running and fell down onto the ground. She landed on her right arm leaving a small cut on it. She was released to come back to work but did not come back because she is experiencing dizziness.

Employee and 3 other co-workers were working on 242 miner, removing tram reducer, when the roof behind them started working. Employee stated he was bumped into by one of his co-workers knocking him into the front of the miner. As he was falling he reached to catch himself with his left hand. His left hand landed on a sharp rock which caused a laceration that required 11 stitches.

Employee had just got off of the dozer and was walking on coal yard to the road. The ground was crusty (frozen), ground broke through and he lost his balance and fell on his right shoulder. Employee continued to work until 5-18-2012 when he went off for rotator cuff surgery.

Employee was stopped and ask by another employee to hang miner cable. He shut off the scoop and upon exiting the scoop his right knee popped, employee stated that his right knee was burning and it hurt to straighten it out. Employee continued to work until 1/12/2012 when he had surgery for a torn meniscus of the right knee.

Employee was walking toward the face, slipped on the mine floor, fell, and struck his forearm on a sharp rock. The employee finished the shift, but later went to a doctor who applied 9 sutures to close the wound. The employee was released back to work without restrictions.

The employee (a Mine Examiner) states he was walking on the back side of the 4th North belt when he tripped over a water line coupling, fell, and struck his forehead against the belt structure. The injury required 12 sutures.

Employee is a roof bolter and stated he had a rock fall on his ATRS. He was preparing to clean the rock, turned toward the rear of the machine, when a rock fell from between the bolts and struck the back of his head and shoulder. He fell forward and landed on his left hand, fracturing the tip of his small finger and requiring 2 sutures.

Employee walked through a curtain and stepped in a depression in the floor and twisted her ankle. Employee was released for restricted duty on 01/12/2011, then fully released for regular duty on 01/17/2011.

Employee was helping the mechanics replace shuttle car cable. Injured was pulling cable off a new spool when his foot became entangled in a loop of cable on the floor. He tripped, stumbled, lost his hard hat, and struck the top of his head on the coal rib causing a laceration of his scalp requiring staples.

Employee states he was beside the feeder standing a timber up when he slipped and fell striking his elbow against the feeder or the ground. He was treated at a medical center and taken off work with a contusion.

Employee stated he had lifted up the Cont. Miner trailing cable to get it over against the rib out of the roadway more. As he was walking the cable over he stumbled on some loose gob and fell toward the rib. He stuck his arm out to catch himself from falling into the rib and felt sharp pain in his shoulder.

Employee states he was exiting the cab of a scoop when his right foot slipped in mud and reported pain in his right knee. Employee has continued to work, but with pain. Employee was taken off work on December 1 for arthoscopic evaluation and according to the operative report, a torn Chondral Flap was repaired.

Employee stated he was helping out with hanging and moving the Roofbolter cable. When he went to turn around to walk another direction he heard and felt his knee pop causing pain in his left knee. He continued to work until 8/5/2010 until taken off by a doctor for an MRI.

Employee stated she was getting out of the cab of a Ramcar, when she apparently got her leg caught on something, which caused her to fall to the ground. She really didn't know what happened. She was treated in the ER for a leg injury and transfered on to other medical facilities. Injury was a severe (Hematoma) that had to be drained.

Employee was getting into the cab of a diesel scoop when he got his foot tangled up when he stepped in and fell foward, striking his mouth on a handle next to the cab. He was taken to medical services where he recieved 5 stitches in his lip.

Employee stated that he was exiting his Ramcar when his left foot got hung/caught in the car controls. This caused him to slip and spread his legs apart (like the splits), straining his hip and also a muscle in his groin area. He has continued to work but was taken off by his doctor starting 9/25/2010.

Employee stated that he was carrying pin boards to the scoop when he stepped in a uneven place in the mine floor and fell on his knee. He continued to work for a couple hours and asked to be taken to Med. services. He then was taken off by his family doctor.

Employee stated he was using a 3/4" ratchet and socket to remove a nut on a steering jack on a Ram Car. The ratchet slipped internally and he lost his balance and tried to compensate with his other leg. He said he felt he had twisted something in his right knee. Employee started missing on 1/31/2010.

Employee stated he was walking over to the left side of the section, when he stepped in a hole twisting his right ankle. He also fell on his face and blooded his nose. Did not miss any work but was put on transitional duty for 1 day.

Employee stated he had got out of the scoop and had started walking, when he stepped in a tire rut and twisted his knee. Employee continued to work until 11/30/2009, when the doctor took him off work for possible surgery.

Employee walking into x-cut off the travelway to get rock dust for bolter, the dust was thick and a pallet was cover with dust. The employee caught his right foot on the pallet and twisted his right knee and fell to the ground.

Employee was examining the CO monitor areas and the conveyor tail area. He grabbed a belt hanger chain and stepped up on the frame to look at the monitor. When he did this his foot slipped and he twisted his knee. He started missing work on 11/04/2009.

Employee was pulling on the miner cable with wet gloves. While pulling on the cable his hands slipped off the cable causing him to lose his balance and fall backwards on his back and straining it. Employee is off awaiting an MRI.

Employee was unloading a big bag of rockdust when the corner of the bag gave way causing the employee to fall backwards onto the scoop. This caused a injury to the lower lumbar area.

Employee stated he maybe stepped on a uneven place in travel road which caused him to fall to the ground early in the shift. He also stated later that he was building a stopping, tripped & fell backwards hurting his back. Injury he claimed is his knee which shows Osteoarthritis, Floating particles and Giant benign Deg. subcortical Geode which is an Illness. Didn't just happen.

Employee stated he was carrying a 1/2 bag of rockdust walking backward dusting the face area. As he twisted around he stepped in an uneven place on the floor, stumbled and this caused his leg to buckle under him straining his knee. He was transported to Medical facility. Was taken off work on 03/31/2009.

Employee is a examiner and stated that while making his route which is walking he slipped going under an underpass and had pain in left and right ankle. Started missing work 6/6/09.

Employee stated he had gotten out of his Shuttle Car to walk away, when he stepped on uneven ground and sprained his ankle. Employee started taking off work on 03/06/2009.

Employee stated that he was getting out of the cab of the roof bolter and tripped over a curtain. He fell backwards and hit his right elbow on the machine canopy and also fell on his right leg. Employee did not start missing work until 10/04/2009.

Employee stated he was walking around a power box when he apparently tripped over a 3/4'' water hose. He stated he fell to the ground but tried to catch himself with his hand. His shoulder was dislocated. Investigation showed that approx. 6 mos. ago he dislocated this shoulder at home. Doctors report stated this could happen again, will need surgery. Occup. Illness

Employee stated he was walking beside a shuttle car carring 2 empty oil cans, when he tripped/slipped on a piece of rock laying on the floor. He fell on his left shoulder and had pain aftwards. He continued to work until he was taken off on 02/05/2009 for rotator cuff surgery.

Employee was going after bit for the cont. miner, tripped over the cable and fell on his right knee.After investigation and a MRI was done , it showed that this was not an injury. The knee had a cyst behind the knee joint behind the posterior cruciate ligament. Also significant osteoarthritis, areas of synovitis, and metallic densities evidence of prior surgery.

Employee stated he was walking underground on the section when he stepped in a small hole and twisted his knee.

Employee stated he was walking toward the face when he slipped on a flat slick rock. He lost his balance and recovered from falling but twisted his knee. He continued to work until 9/22/2008 when he was taken off to have surgery.

Employee has had a bad left knee and possibly favoring it, as he was walking across the unit doing his examination, he stepped in a tire rut and twisted his other knee. Employee continued to work until April 20th, 2008 and since had surgery on his knee.

Employee stated he was taking a roll of line curtain to the face, when he stepped in hole and twisted his Right knee. Employee was transported to medical services ER and saw his regular doctor on 11/06/2007. He scheduled off the next three days and started missing on 11/10/2007. Results of MRI was recieved 11/16/2007 from Dr.

Employee was in the surface shop installing a transmission in a underground mantrip. The transmission became hung and wouldn't go in any farther so he was pulling at it trying to get it to free up. All of a sudden it came loose and caused the employee to fall backwards, twisting his knee and striking his elbow on a metal cover. He started missing 9/19/2007.

Employee stated that he was walking up to the face area to stagger out the roofbolters. He stepped in a hole on the mine floor and twisted his right knee. He did not go to the doctor until the next day.

Employee stated he was operating a Mini-Trac and was sitting on a piece of curtain in the seat. He went to get out of the cab, when his foot slipped on the piece of curtain and he fell. As he fell he tried to catch himself with his right arm and hurt his shoulder. He continued to work until April 1st, 2008 when he was taken off for surgery.

Employee stated that he was on top of the cont. miner and had been servicing the head hing pins. As he stepped off he put his left foot down on the ground, slipped and struck his right knee on the top cover of the miner.

Employee stated he was examining the slope belt and as he was walking down the slope he slipped on a piece of pipe laying on the concrete floor and fell, twisting his back.

Employee stated he was preparing to set a water pump at a sump at 171 X-cut. The bank of the sump gave way, causing the employee to fall in the sump twisting his ankle.

Employee stated he had climbed upon the raw coal stacker and did some work and was climbing down. As he reached the last step and started to step onto the ground, which was a longer step, he twisted his knee when his foot hit the ground. Employee didn't miss any work until 06/05/2007.

Employee stated that he was walking from his ramcar to go help the miner operator when he slipped/tripped over a piece of far-clay and fell to the floor. He stuck his hand out to break his fall and apparently strained his wrist. Employee started missing on 05/23/2007

Employee was rock dusting beltline. He was walking backward and tripped over a roller in the walkway.

Employee stated he was bolting an area that had a slicky side slip in it . He went to put his drill steel in the chuck and a huge rock fell hitting the canopy and fell toward the face. When he heard it fall he lunged and dove backwards away from it and was not hit by the rock. When he got up off the floor he had a lacerated ear lobe.
