On 12/10/2009, the employee was walking toward the power center in his work area. He stated that he tripped on a rock in the walkway and fell on to his right side. He continued to work at his regular job until 12/17/2009, when he started missing work due to pain in right leg.

Employee was in the process of cleaning out a shelter hole along the track haulage. His left foot was placed on a coal shelf on the mine bottom by the shelter hole. As he reached down to pick up a large piece of coal toward the back of the shelter hole, his left foot slipped causing him to hyper extend his knee. The employee started missing work on 10/10/09 due to this event.

Employee was walking down the steps at the Prep Plant. She slipped on the last step twisting her left knee. She has continued to work until 10-2-09 when she started missing work for a torn meniscus.

Employee got finger caught between frame of scoop bucket and hydraulic ram in scoop bucket, resulted in fractured distal phalanx right middle finger.

Employee was removing safety chains from between supply cars on slope bottom when he slipped striking his right jaw on something (undetermined) resulting in fracture to right jaw. Incident was not reported until 10-15-2008, began missing work on 10-16-2008.

On June 2, 2008 employee was lifting one end of steel beam when his artificial left hip came out of socket causing him to fall on his left side injuring his left hip and groin area. He was being evaluated by a doctor until June 20, 2008. On June 20, 2008 he was evaluated and diagnosed as being unable to return to work.

On 4/30/2008 the employee was pushing over an old cribbing that was set. In the process, he stepped on some material and slipped twisting his right knee. He has continued to work until 7/14/2008 when he started missing due to having surgery on his knee.

On 3/17/2008 the employee was stepping down the steps of a ladder on a belt cross-over. His foot became caught in the step causing him to fall to the bottom twisting his back. He continued to work until 3/26/2008 at which time he started missing due to back pain.

Employee was crawling across a conveyor belt cross-over platform. The left handrail broke causing the employee to strike the left side of his face on the frame of the platform.

Employee was helping to hose down the floor surrounding the stacker belt tailpiece. As he went to shut water hose off, he slipped on the expanded metal flooring straining his right shoulder and left knee. Employee has continued to work until 4/21/2008 when he started missing work due to pain from his injuries.

On 1/19/2008 the employee was stepping down off the stageloader at the longwall. He felt something pop in his left knee. He has continued to work and started missing time on 5/9/2008 to have surgery on the left knee.

On 11-23-07 employee was dragging rockdust hose and fell backward on his right elbow. He has continued to work until 1-18-2008 when he started missing due to pain in right shoulder.

On 10/10/2007 the employee was climbing up on dozer at the Prep Plant. He slipped and strained his right shoulder in the process. He has continued to work until 10/29/07 when he began missing for pain in Rt. shoulder and neck area

On 9-12-07 the employee was walking in entry and stepped in mud with his left foot. As he attempted to take his next step his left foot remained stuck causing him to twist his left knee. He continued working until 10/31/07 at which time he started missing work due to pain in left knee.

EE fell while cleaning bottom of thickner at the prep plant. He strained his left shoulder in the process. EE started missing work on 9/10/07 due to left shoulder pain.

Employee was charging a scoop. He tripped on a water hose and fell striking his lower right leg on a charger cable. He began missing work on 9/7/07 due to pain in right leg.

On 7/21/07 the employee slipped and fell while working in slope. He continued to work until 9/12/07 at which time he started missing work due to pain in rt shoulder.

On 7/10/2007 the employee was walking along a beltline. He stated that he slipped in some mud twisting his right knee. The employee has continued to work until 8/31/2007 at which time he started missing work due to surgery on his right knee.

On 6/09/2007 the employee fell while removing an acetylene tank out of a mantrip. He fell to the mine floor striking his right shoulder. The employee has continued to work until 8/30/2007, at which time he started missing work due to pain in right arm, shoulder and neck.

Employee slipped crossing over track at the slope belt tail. He fell on his back and his SCSR impacted his left side. Employee was diagnosed on 12/11/2006 with two fractured ribs. He started missing time on 12/13/2006.

Employee was standing on the drill head. He slipped and fell catching his middle finger and ring finger of his left hand between the lift jack and arm of the bolter.

Employee was cleaning out rock bin and slipped striking head on side of bin. Employee received sutures to forehead.

Employee reported on 06/10/06 that on 06/08/06 he slipped spreading legs apart and felt pain in groin area. He was diagnosed with a hernia. He started missing time on 06/15/06.

Employee was walking and twisted his right knee. Employee did not start missing time untill 5/25/06.

Employee slipped cleaning a windshield catching himself with his left arm. Later he felt pain in his shoulder.(Employee started missing time on 12/27/2005).

Employee slipped falling down steps at the load out.

WALKING IN #2 ENTRY OF 6-B SECTION. TRIPPED AND FELL TWISTING HIS LEFT ANKLE.

LOST HIS FOOTING COMING DOWN FROM TOP OF MOTOR. FELL AND CAUGHT HIMSELF WITH LEFT HAND CAUSING HIS LEFT THUMB TO BE OVER-EXTENDED.

EMPLOYEE WAS CLEANING AT OIL STORAGE AREA AT SLATE BIN. HE REPORTED HE SLIPPED AND FELL ON LEFT SHOULDER

WALKING ALONG BELTLINE CARRYING A BAG ON ROCKDUST. EMPLOYEE SAID HE STEPPED ON UNEVEN BOTTOM AND FELT PAIN IN RIGHT KNEE.

EMPLOYEE WAS CROSSING OVER A BELT, THE HAND HOLD BROKE CAUSING HER TO FALL BACKWARDS, JARRING HER RT. SHOULDER & TWISTING HER LT. ANKLE.

EMPLOYEE WAS WALKING IN # 2 ENTRY OF 6B SECTION. HE STEPPED ON A ROCK IN A WATER HOLE CAUSING HIM TO LOOSE HIS BALANCE TWISTING HIS HIP.

EE WAS OPERATING THE SHEARER. ROCK FELL STRIKING TOP OF SHEARER AND EE MOVED OUT OF THE AREA. IN DOING SO, EE TRIPPED OVER THE RELAY BAR FALLING ONTO LEFT SIDE

EE WAS AT SLOPE CLEANING. FELL, STRUCK SHOULDER. EE MISSED 11 MONTHS LAST YEAR WITH SIMILAR PROBLEM TO SHOULDER

EE WAS WALKING TO MANTRIP IN SOUTHWEST MAINS SECTION. EE TRIPPED ON A ROCK FALLING DOWN INJURING SMALL FINGER OF RIGHT HAND

Employee was exiting a motor as he steped out the motor moved causing him to fall. EE felt pain in lower back and shoulder.

EMPLOYEE CLIMBED UP ONTO A SUPPLY CAR TO UNHOOK CHAIN, AS HE STEPPED DOWN HE LOST HIS BALANCE & FELL FROM THE SUPPLY CAR.

EMPLOYEE WAS STEPPING DOWN FROM OVERDENSITY PUMP, FELT PAIN IN RIGHT KNEE/HIP.

While walking down panline employee tripped on shield toe, falling onto a shield base.

WALKING UP #2 ENTRY ON THE 4-B SECTION TOWARD THE MINER, RIGHT FOOT SLIPPED OUT TO THE SLOE IN A RUT CAUSING THE MINER TO FALL AND TWISTED RIGHT KNEE.

Employee was walking, stepped in a hole with right foot, felt pain in right side of body.

AS EE WAS WALKING UP A HILL, HIS KNEE GAVE WAY AND HE FELT PAIN IN HIS LEFT KNEE.

Employee stated he was walking to uncouple supply cars and felt apain in left knee.

EE STEPPED DOWN OFF A MOTOR & STEPPED ON A ROCK CAUSING HIM TO FALL STRIKING KNEE AND TWISTING KNEES. EE DID NOT START MISSING WORK UNTIL 1/20/05.

EMPLOYEE WAS DIAGNOSED WITH A PATELLA SUBLAXATION THAT HE BELIEVES IS DUE TO TWISTING HIS KNEE WHILE WALKING THRU A MUDDY AREA.

Employee was walking and he slipped on a piece of wood in the walkway, causing him to fall onto his left knee.

EE WAS STANDING ON TOP OF A SUPPLY CAR HELPING TO UNLOAD CEMENT BLOCK. A CHAIN WAS ATTACHED AROUND 2 PALLETS OF BLOCK. AS HE WAS WALKING ACROSS CAR TO GET IN THE CLEAR HIS FOOT GOT CAUGHT IN HOLE OF THE CAR DECKING CAUSING HIM TO FALL TO THE MINE FLOOR STRIKING HIS MOUTH ON A ROCK.

EMPLOYEE WAS CLIMBING DOWN LADDER FROM A FRONT LOADER AND MISSED THE BOTTOM STEP. HE FELL TO THE GROUND TWISTING HIS LEFT KNEE. HE WAS DIAGNOSED BY PHYSICIAN ON 3/31/04 FOR AN ACL TEAR ON LEFT KNEE.

EMPLOYEE PUSHING CONCRETE BUCKET, TRIPPED OVER A TIMBER AND TWISTED KNEE.

EE WAS DIAGNOSED WITH A TORN MENISCUS IN HIS RIGHT KNEE WHICH HE ALLEGES TO BE WORK RELATED.
