Employee slipped on the mine office steps and fell on his right arm causing a contusion.

Employee stated he twisted around and fell while installing roof bolts and strained his lower back. He was suspected of drug use and was asked by the superintendent to take a drug test. He decided to quit.

EE slipped on the mine floor while making a fireboss run. He pulled a ligament and received a contusion on his right ankle.

Employee was walking on the section after building a stopping when he slipped and broke his right ankle. He went to the hospital and they put a plate and screws in it.

EE was pulling waterline when he tripped and fell. He caught himself with his left hand and stoved his left shoulder.

Injured was shoveling on #6 belt when he slipped on gob. He fell on a rock and hurt his right knee.

The employee was pulling belt and tripped. He fell on a belt rail bruising his left rib and right knee.

The employee was cleaning the #1 belt tailpiece when he fell backwards. He landed on an anchor pin and injured his lower back. He went to the hospital and they diagnosed the injury as a lumbosacral strain.

EE was climbing down the ladder on the endloader. He slipped off the bottom step, felt burning in lower abdomen. Went to dr. after work and found out he had a hernia.

Employee was hanging curtain standing on an oil can. It tipped over causing him to strain/pull a muscle in his back.

Employee stumbled against a pallet of rockdust injuring his tailbone area.

HANGING LINE CURTAIN, FELL FROM LADDER INJURING LOWER BACK.

LAPPING BELT ONTO SCOOP; SLIPPED & FELL OFF SCOOP BUCKET FRACTURING HIS ARM.

WHILE CLIMBING ON LOADER, THE EE SLIPPED ON OIL SPOT, HIT HIS HEAD & FELL TO THE GROUND ON HIS RT. SIDE. THERE WAS KNOT ON THE RT. EYEBROW, HE ALSO COMPLAINED ABOUT HIS RT. SHOULDER & ARM.

OFFICER GOT OUT OF HIS VEHICLE TO RELEAVE HIMSELF. FEET SLIPPED ON ICE/SNOW COVERED ROAD & HE FELL. HE INJURED HIS HIP WHEN HE FELL.

HELPING ON MINER CABLE, FEET GOT CAUGHT IN MINER CABLE AND HE TRIPPED AND FELL.

STEPPING OFF PUSH ARM ON DOZER, LEFT FOOT HOOKED IN TREAD PLATE AND TWISTED LEFT KNEE. EXPECTED TO RETURN ON 8/23.

WET & RAINY CONDITIONS. SLIPPED & HIT HEAD.

WHEN EE WAS DISMOUNTING COAL TRUCK HIS FOOT SLIPPED FROM ATTACHED LADDER. THIS CAUSED HIM TO FALL BACKWARD ONTO THE GROUND. THUS, RESULTING IN CONTUSIONS TO HIS RIGHT SIDE & A FRACTURE TO HIS RIGHT ARM.

DRIVER WAS STANDING ON FENDER OF COAL TRUCK AND FELL OFF WHILE TRYING TO CHANGE A AIR FILTER.
