Mining near a fault line, the intersection of #3 Entry fell approximately 10 to 12 feet high. There was no damage to ventilation or equipment on the section. No miners were involved in the fall, so therefore no injuries.

EE reported Wednesday 1/3/07 by telephone to mine operator that on monday he had tripped on miner cable and injured his back.
The fall was in the return at 55217. Out by #2 drive in the return.
Employee lifted the cable reel to the TD2-43 roof bolter and strained his back.

Roof fall in no. 1 entry 70 ft inby survey 2012. The fall was approx. 19 ft wide, 4 to 5 ft high and 30 ft long. The fall occurred just inby the break which had one cut taken out of it and fell all the way to face.
A rock fall was discovered during the weekly examination of the return area. The fall was 3500 feet from the working section. The fall was at SS 550 approaching the #3 seal. The area will be dangered off and a walkway to the #3 seal will be timbered off for safe access to the #3 seal.
Had roof fall inby survey 179. Roof fall was approx. 18' wide x 30'long, thickness was 2.5' to 4.5'. There were 4' glue bolts used in this area of mines. This fall was found by EE on pre-shift exam.
The injured party was pinning top when a small piece of draw rock fell. He moved to get out of the way and hit his elbow on the side of the pinner. Just bruised and a very small cut. Two days later it was swollen because of infection. He had to spend time in the hospital.

Bolter operator was lowering drill head when steel suddenly came loose and struck him. A piece of rock fell with the steel. Operator was struck.
EMPLOYEE WAS RIDING A 3-WHEEL RIDE. EE STOPPED, WALKED AROUND THE FRONT OF THE RIDE AND WAS GOING TO UNLOAD SOME TANKS. WHEN EE UNLOADED ONE TANK, THE OTHER TANK ROLLED OVER ON THE PEDAL AND THE 3-WHEEL RIDE TOOK OFF AND KINDLY TWISTED HIS LEG, CAUSING THE INJURY (TWISTED KNEE).
PICK UP BOX OF RESIN AND SAID TWISTED HIS BACK.
THE ROOF FELL, IT WAS A MUD SEAM, AND IT SLIPPED OUT.
EE WAS TRAVELING #2 BELT ON THREE WHEEL BUGGIE PIECE OF DRAW ROCK FELL STRIKING HIM ACROSS LEGS CAUSING INJURY TO LEFT KNEE AND RIGHT THIGH.
EMPLOYEE TRIED TO PICK UP A 21SC TIRE TO LOAD ON HIS RIDE. TIRE ROAD HIM BACK WHICH HE SAID TWISTED HIS BACK.
EE WAS OPERATING STANDARD SHUTTLE CAR. HE LOADED& WAS ON HIS WAY TO THE DUMP WHEN HE CAME IN CONTACT WITH THE CORNER OF THE RIB IN THE NUMBER2 HEADING GOING INTO THE BREAK TOWARD THE NUMBER 3 HEADING.
THE ROOF FELL APP 3 TO 4 FT IN THE RETURN. WE ARE NOT GOING TO CLEAN IT UP. WE JUST BUILD CRIB AND SET TIMBER AROUND IT.
